What is cancer? What is the main reason of cancer? Is cancer is curable? What all medicines we have?

DNB, MBBS
Oncologist, Faridabad
Dear sir, cancer is caused by abnormal uncontrolled proliferation of cells, usually caused by some abnormal mutations or change in genes which control the cell growth. This is usually caused by some carcinogens like tobacco, pollution, pesticides, radiation exposure etc. If diagnosed one has to get it treated by surgery, chemotherapy, radiotherapy etc and cure depends on the stage of presentation, histopathology and biology of disease.

Ek choti jaisi gilti ha mere breast me jo bhadi hoti ja rhyi ha or dard bi hota hai. Please help

PGDMLS, PGDHHM, LLB, PhD Surgery, MS - General Surgery
General Surgeon, Gurgaon
Generally this is due to a condition called fibroadenosis. This is a harmless condition and is related to hormonal changes. It is usually more, a few days before the start of monthly menstrual period and becomes less after the periods. You might be able to feel a lump on catching with all the fingers of your hand but no lump when you try to feel it with the flat surface of the fingers / hand. Breast support with a tight fitting bra might give some relief. Medication can be of some help but that should be taken only after a physical examination by a general surgeon.

Mr. Lybrate-user, both can be tackled at the same sitting, check on your urologist. If the stones are very small, once you undergo turp for your enlarged prostate, stones will pass out, if not, at the same sitting, they can remove the stones doing a cystoscopy.

What is prostate? What are the reasons for its enlargement, and what is the treatment.

PDDM, MHA, MBBS
General Physician, Nashik
What is prostate? What are the reasons for its enlargement, and what is the treatment.
The prostate is a gland in men. The prostate grows larger due to an increase in the number of cells (hyperplasia). However, the precise reason for this increase is unknown. A variety of factors may be involved, including androgens (male hormones), estrogens, growth factors and other cell signaling pathways. Once the diagnosis of prostate enlargement is made, your doctor may not recommend immediate treatment if symptoms are mild. Likely, one or more exams will be conducted per year to be sure that you are not developing any complications from prostate enlargement. Should your symptoms become more severe, both medical and surgical treatments are available.

I am 23 years old female I am having lump in right side of my breast and it has increase in size! I don't have any pain in it the lump can move every where in breast. It is movable. Is it cancerous or non cancerous?

MD - Obstetrtics & Gynaecology
Gynaecologist, Mumbai
It looks like it is fibroadenoma, it should be removed only if painfull and increasing rapidly in size after initial investigation like mammography and sonography of the breast and/or FNAC. Fibroadenomas are not cancer and don’t increase the risk of developing breast cancer. They occur because of an increased sensitivity to the female hormone estrogen. It is very common can occur in numbers. Most are about 1 to 3cm in size but occasionally, it can grow to more than 5cm. Most fibroadenomas stay the same size. Some get smaller and some eventually disappear over time.

DNB, MBBS

Dear smoking is a silent killer. It does not happen overnight but slowly it gets absorbed in lungs and blood and not only damages lung but also other parts leading to cancers of head and neck, lungs, gastrointestinal tract like intestinal, rectal, bladder and kidney and even leukaemia. Annual health screening will help you apart from if you have any specific problem then you can meet doctors earlier also.

Sir, I am rahul Raj. I am 20 Yr old. Nd my mausi has blood cancer. What is the process of cure of these diseases.

MS ( General Surgery)
Oncologist, Mandsaur
Blood cancer treatment & therapy options Treatment for blood cancer depends on the type of cancer, your age, how fast the cancer is progressing, where the cancer has spread and other factors. Some common blood cancer treatments include: • Stem cell transplantation: A stem cell transplant infuses healthy blood-forming stem cells into the body. Stem cells can be collected from the bone marrow, circulating (peripheral) blood and umbilical cord blood. • Chemotherapy: Chemotherapy is the use of anticancer drugs designed to interfere with and halt the growth of cancer cells in the body. Chemotherapy for blood cancer sometimes consists of giving several drugs together in a set regimen. It may also be given before a stem cell transplant. • Radiation therapy: Radiation therapy may be used to destroy cancer cells or to relieve pain or discomfort. It may also be given before a stem cell transplant.

Red to have full details about your problem before any sound advice can be given. There is high recurrence rate of this cancer even after treatment. It is mostly the local problem which troubles a person. Distant spread related problems are rare.

Family History of Cancer? - Opt for a Preventive Health Check

Family History of Cancer? - Opt for a Preventive Health Check

The rising incidences of cancer have made it important for us to be vigilant about our health. It has been proven medically that if cancer is detected at an early stage -stage I & II , then the chances of cure and treatment along with full recovery are very high. For early cancer detection & for people at greater risk of cancer, PHC-Preventive Health Care plays a major role. Preventive Health Check is particularly important for the people who have a family history of cancer.

Hereditary Cancers:

Almost all cancers are caused by damage or mutation in their genes, which, may be acquired from environmental exposure, dietary factors, hormones or through normal aging. In 90% of cases, these genes are not passed from parents to children. These are known as sporadic mutations and the cancers that they cause are called as sporadic cancers. But 5-10% of cancers are caused by gene mutations which are inherited from one or both parents & passed on to the children .These are hereditary or germline mutations & these cancers are known as hereditary or familial cancers.

People /children, who carry gene mutations have a higher risk of developing cancer at an earlier age or at some point in their life time.

Some of the common such cancers, which run in families, are:

  • Breast cancer
  • Ovarian cancer
  • Colo-rectal cancer (cancer of large intestine )
  • Prostate cancer
  • Lymphoma
  • Melanoma
  • Retinoblastoma

Breast, ovary & colo-rectal carcinoma have well established gene mutation studies.Mutation of BRCA 1 & BRCA2 Genes is responsible for 85% of hereditary breast cancers.

Estimated life time risk for developing breast cancer in woman with BRCA1 & BRCA2 mutation is 56-87% & risk for developing-bilateral /contralatral breast cancer is about 20-40%. These statistics make a preventive health check for women with breast cancer history in their family a necessity. Mutation in these genes confers about 20-40% increased life time risk for developing ovarian cancer. Hence the presence of gene mutation for breast cancer can trigger ovarian cancer.

However as stated above, other environmental and lifestyle factors can also cause breast cancer other than genetic BRCA1 & BRCA 2 gene mutations.

  • Increasing age
  • Early menarche
  • Late menopause
  • Nulliparity
  • First birth after the age of 30
  • Atypical lobular hyperplasia or Atypical ductal hyperplasia
  • Prior breast biopsies
  • Long term postmenopausal estiogen replacement
  • Early exposure to ionizing radiation

Other Familial Cancers:

Familial cancer syndromes associated with colo-rectal cancers are familial adenomatous polyposis (FAP).It is an autosomal dominant inherited syndrome with more than 90% penetrance, manifested by hundreds of polyps developed by late adolescence .The risk of developing invasive cancer is almost 100% .So such patients should start getting colonoscopy at an earlier age- say by 20yr of age & should undergo Total Collectomy on development of significant polyps .

Other familial cancer syndromes are hereditary non -Polyposis Colo Rectal Cancer (HNPCC), Gardner's Syndrome, Turcot's Syndrome, Peutz-Jeghers Syndrome, Juvenile Polyposis.

Cancer Assessment & Conclusion:

For breast cancer risk assessment, we have the GAIL model & IBIS model, which take into consideration ,all the risk factors, apart from BRCA1 & BRCA 2 gene mutation. These models calculate the over all life time risk of development of breast cancer for any lady. For this, you have to consult a specialist experienced in area of hereditary cancers, cancer risk assessment, genetic counseling & testing.

To conclude, all people with cancers in their family should undergo genetic testing & counseling, to detect high risk individuals should take adequate measures & treatment.

Water in lung does not mean you have cancer. It can be due to some infections also. Please consult a pulmonologist (chest specialist) for proper diagnosis.
