Most people use the term piles if they see any bold in motion, which may be just mild from minor injury in the excretion pathway resulting from constipation, or fistula or gastroenteritis and see in blood is NOT piles, in all cases. Piles are haemorrhoids that become inflamed. Haemorrhoids are masses, clumps, cushions of tissue in the anal canal - they are full of blood vessels, support tissue, muscle and elastic fibers. Piles can be of various sizes and may be internal (inside the anus) or external ones (outside the anus). Typically, internal piles occur from 2 to 4 cm above the opening of the anus. External piles (perianal hematoma) occur on the outside edge of the anus. The internal ones are much more common. Symptomatic haemorrhoids affect at least half the population at some time in their lives before the age of 50. In the majority of cases, piles are effectively treated with over-the-counter medications, a good fluid intake, and by following a diet high in fibre. In severe cases, the piles may have to be surgically removed. About 10% of patients who go and see their doctor about piles eventually require surgical intervention. Internal haemorrhoids are classified into four grades. Grade 1 - there are small inflammations, usually inside the lining of the anus. They are not visible Grade 2 - larger than grade 1 haemorrhoids, but also inside the anus. When passing a stool, they may get pushed out, but return unaided Grade 3 - often called 'prolapsed haemorrhoids'; these appear outside the anus. The patient may feel them hanging out. They can be pushed back in if the patient presses with their finger Grade 4 - these cannot be pushed back in and need to be treated by a doctor. They are large and stay outside the anus all the time. External haemorrhoids are called perianal hematoma. These are small lumps that are located on the outside edge of the anus. They are very itchy and can be painful if a blood clot forms inside (thrombosed external haemorrhoid). Thrombosed external haemorrhoid requires medical treatment straight away PILES OCCUR when the blood vessels around the anus and in the rectum will stretch under pressure and may swell or bulge. Inflamed veins (haemorrhoids) can develop when pressure increases in the lower rectum. This may be due to: Chronic constipation9 Chronic diarrhea10 Lifting heavy weights11 Pregnancy12 Straining when passing a stool. 13 The tendency to develop haemorrhoids may also be inherited. The risk of developing piles grows with age Treatments for piles In the majority of cases, piles resolve on their own without the need for any treatment. Treatments can help significantly reduce the discomfort and itching that many patients experience. Diet - piles can be caused by too much straining when doing bowel movements, which is the result of constipation. A change in diet can help keep the stools regular and soft. This involves eating more fiber, such as fruit and vegetables, or switching your cereal breakfast to bran. Water is the best drink, and the patient may be advised to increase his/her water consumption. Some experts say too much caffeine is not good. Simple things you can do yourself to help prevent piles: 10 Try not to strain when you go to the toilet Avoid laxatives Exercise. Ointments, creams, pads and other OTC medications - there are some over-the-counter (OTC) medications which help soothe the redness and swelling around the anus area. Some of them contain witch hazel, hydrocortisone, or some other active ingredient which can relieve symptoms of itching and pain Surgery - used for particularly large piles, or grades 3 or 4 haemorrhoids. Generally, surgery is used if other procedures were not effective. Sometimes surgery is done on an outpatient basis - the patient goes home after the procedure.

CGO, BHMS (Silver Medalist)

I am losing hair for past few months. Now I have noticed some bald spots on my head. Can you please help me with this?
Hi, hairfall has many causes: 1) anemia 2) tying your hair tightly 3) imbalanced diet or a diet low in protein 4) stress 5) physical or emotional shock. 6) post typhoid, hypothyroidism. Tips to avoid hairloss: 1) do not comb on wet hair. 2) oil your hair regularly. 3) destress and sleep 8 hrs daily. 4) do not blow dry or experiment with hair like hair straightening. 5) balanced and high protein diet. 6) use homoeopathic jaborandi hair oil by sbl pharmacy. Bald spots on the scalp means you are suffering from alopecia areata which is an autoimmune disorder where your own body cells damage your hair follicles. This requires proper treatment. You may consult me privately for my complete hairloss package. You may consult me privately for.

How To Make It Through Alcohol Withdrawals

Fear of Alcohol Withdrawals
One justification that alcoholics will use for not giving up the abuse is their fear of alcohol withdrawals. The process of entering sober living can involve an uncomfortable few days, but the rewards that come afterwards make it well worth it. The reality is that alcohol detox symptoms are rarely that uncomfortable and some people experience very little in the way of unpleasantness. The experience that the individual has with withdrawals can depend as much on their expectations as anything else.

Alcohol Withdrawals Defined
The word withdrawals or withdrawal are most commonly used when describing symptoms that people experience when they abruptly discontinue or decrease their intake of medications or recreational drugs. These symptoms begin to occur whenever the level of these chemicals in the bloodstream falls to a certain point. Alcoholics can experience the beginning of withdrawal symptoms as part of their hangover. This is why many of these individuals will feel ill until they manage to consume their first drink of the day. Another way of describing withdrawal symptoms would be to say that they are painful physical and psychological symptoms that occur with the discontinuance of addictive substances.

Symptoms of Alcohol Withdrawals
The most common symptoms that people are likely to experience when withdrawing from alcohol include:

* Irritability
* Feelings of anxiety and nervousness
* They often experience a feeling of fuzziness in their cognitive functioning ? they are unable to think straight.
* Inability to sleep or even get comfortable in the bed. When they do fall asleep they may experience disturbing and vivid dreams.
* Nausea and vomiting.
* They feel shaky and easily startled.
* Headaches
* Body aches
* Shaking or trembling
* They feel tired and lack motivation to do anything.
* Cravings for alcohol
* Mild symptoms of depression.
* Sweating or clammy skin.
* Inability to concentrate.
* The feeling that something bad is about to happen.
* Mood swings.
* Changes to vital signs such as increased heart rate or elevated blood pressure.
* Upset stomach

Symptoms of alcohol withdrawal usually begin about 5 to 10 hours following the last drink. They reach their peak between 48 and 72 hours and can persist for a few weeks into recovery. In most instances the symptoms of alcohol withdrawal are mild, but there can be more severe symptoms known as delirium tremens (DTs) ? these can be fatal.

Dangers of Delirium Tremens
Those individuals who have been heavily abusing alcohol for many years are at risk of developing the DTs when they give up alcohol. Up until a few years ago the mortality rate from these severe withdrawals was as high as 35%, but it currently stands at about 10%. The risk of death is still there so it is vital that anyone who is likely to experience delirium tremens should be medically supervised during withdrawals. The symptoms of the DTs can include:

* Convulsions similar to clonic tonic seizures where the individual loses consciousness and shakes violently.
* Intense hallucinations that will usually involve moving animals such as rats and snakes. Some people will also experience the hallucination that insects are crawling on their skin.
*Confusion associated with the DTs can be quite severe.
* Changes to vital signs are likely to be more extreme than with usual withdrawal symptoms.
* People can become highly anxious and agitated.

In most instances the symptoms of delirium tremens will appear within the first 72 hours. If the individual makes it past this time without experiencing the DTs they will less at risk of them ? although not entirely risk free. A physician can use a special assessment to judge the likelihood of people developing this severe form of alcohol withdrawals. Those who intend to stop drinking are advised to have this assessment.

Causes of Alcohol Withdrawals
Those individuals who abuse alcohol develop a tolerance for this drug. The body is wonderfully adaptive, and it can adjust functioning to accommodate alcohol abuse. It does this so well that the sudden removal of alcohol from the bloodstream is a shock to the system. It has to struggle to regain normal functioning, and the withdrawal symptoms occur while this is happening.

Alcohol withdrawals also usually involve psychological symptoms that can be just as uncomfortable as the physical ones.

Dangers of Expecting the Worst in Alcohol Withdrawals
One of the best known motivational speakers of the twentieth century, Normal Vincent Peale, once claimed that:

> If you paint in your mind a picture of bright and happy expectations, you put yourself into a condition conducive to your goal.

The expectations that people have about the future does have an impact on what they will experience. It means that if they are expecting their withdrawal symptoms to be unpleasant they probably will be. Their fear of what is going to happen will mean that they are aware of every single niggle or discomfort, and they can easily blow these out of proportion. This is because when people go looking for trouble they are almost certain to find it.

The Buddha is reported to have given a good explanation for why this happens. He uses the example of a man being hit by an arrow. The pain associated with this event is going to cause discomfort, but the way the person reacts to it can make things worse. If they panic and expect the worst then this is like being hit by a second arrow. Their mental attitude has led to a doubling of their pain. The exact same thing can happen with withdrawal symptoms if people are not careful about their expectations.

Importance of Distraction When Dealing with Alcohol Withdrawals
Distraction is a powerful tool for helping people make it through the early days of recovery. This is because the worst thing that the individual can do is remain focused on their discomfort and cravings. Distraction means turning away from the original focus or interest. When people do this it means that they can at least temporarily escape the stimulus that was making them uncomfortable. Examples of the type of distraction that people might use in early recovery include:

* Watching television ? comedy shows can be a particularly good form of distraction.
* Listening to music, but this should not be anything that reminds the individual of drinking.
* Going for a walk can be a great distraction, and the fresh air also tends to make people feel better.
* Vigorous exercise is another effective solution, but this may not be appropriate for those who are in early recovery and have not exercised in a long time.
* Spending time with other people.
* Playing computer games, though this can have its? own complications for compulsive behavior.
* Reading a book is another option but some individuals may not have the concentration for this in early recovery. Audiobooks may be an effective solution for those who have trouble reading.

Advice For Making It Through Alcohol Withdrawals
Most people will only have mild withdrawal symptoms that will not be any worse than an episode of influenza. It is the knowledge that the individual is able to stop the symptoms by drinking again that makes them more difficult to cope with. In order to make it successfully through alcohol withdrawals it is helpful to:

* It is important to keep in mind that the unpleasant symptoms are only temporary, and the rewards for sitting them out are important.
* The worst thing that people can do is to wallow in their suffering. It is best to keep the mind distracted as much as possible.
* If people have the right mental attitude they may experience very little in the way of symptoms when they give up alcohol.
* Those individuals who expect the worst may be more likely to struggle with their withdrawal symptoms.
* If there is any suggestion that the individual may be progressing towards delirium tremens they will need to medical assistance right away.
* When people feel like they are about to give into the cravings they should delay for an hour. The cravings are likely to lessen during that time and if not they only need to commit to one more hour ? by just keep on committing to one more hour the individual makes it through this stage of weakness.
* It can be helpful to write a letter at the start of the process where the individual writes in detail about their reasons for wanting to quit, and their expectations for a future life in sobriety. If they feel like their motivation is beginning to wane they can look back on this letter for inspiration.
* It can be helpful to remember that many millions of people have successfully made it through the withdrawal process. There is no reason for why the individual cannot do the exact same thing.
* The companionship of other people can be a great help at this stage of recovery.
* The individual will greatly increase their chances of making it through the detox process by entering rehab. Here they will have all the resources they need tom make the process go as smoothly as possible ? they will also be protected from temptation.
* A physician can prescribe medications that will ease discomfort during withdrawals. In most instances such drugs will only be available in a medical setting or when the individual is going through detox in rehab.
* There is no requirement for people to feel miserable during this period of adjustment. Laughter is one of the most effective medicines on the planet and is highly recommended.

How To Lose Fat From Hips and Thighs

How to lose fat from hips and thighs

Dietary changes –

  • Check the oil that you are using. Vegetable-based and olive oils are good for you. Cut down on ghee, butter, cheese, paneer and margarine.
  • Limit your carb intake. Restrict yourself while eating potatoes, white rice, sago, pasta and other products made of refined flours.
  • Use only skimmed milk and skimmed milk curd.
  • Give up on all the fried foods until you get that shape. Nothing will come easily. Avoid fries, pakodas, samosa, fried momos, chips, and other fried snacks.
  • Increase your fibre intake. Eat whole cereals, oats, broken wheat / daliya, brown rice, brown breads, fruits with skin and a lot of green veggies. If possible mix bran into your flour.
  • If you are a non-vegetarian and enjoy fish, you will love this diet. Eat fish upto 4 times a week. Bake it, grill it but don’t fry it. You can have other lean meats up to 2 times in a week. And egg whites for breakfast are a good idea.
  • 2 tsp of triphala churn, along with ginger water is excellent way to help you liver to break down fats speedily and effectively.
  • Cut the intake of wheat. Extra carbs always end of on hips and thighs. Include other cereals like jowar, bajra, and mix these flours with horse gram flour and soya flour.
  • Go very light with your dinners. A plate full of salad made out of veggies, sprouts, fruits along with soups will help. Fill in with a very small portion of brown rice or daliya. In short have a fiber and protein rich dinner and go low on carbs.
  • Eat dinner around 7.30 pm and having a small walk after it will only add up to your goal.
  • Consume as much of water as you can and eat different varieties of melons for better results.

How to lose fat from hips and thighs through exercise:

  • Walking is the best exercise for your hips and thighs. Walking up to 4 km per day will make your goal appear closer and closer.
  • If I ask you, “how to lose fat from hips and thighs”? you would probably answer cross trainer or the cycle at gym. Frankly you are not helping yourself if you are doing these to lose fat from hips and thighs. These exercises will only help you in tightening your muscles and then bulking them up.
  • If you have been walking already, but don’t see much result, then you might need to increase the distance by a km and the intensity of walks from the 30th minute onwards. Also check your diet to ensure you are following the tips above.
  • Lunges and squats will help you too. Remember to check your posture while you do these exercises. Wrong posture can cause pain.
  • Running and jogging, hiking and trekking will help you too.

How to avoid sleep disturbance?

MBBS
General Physician, Mumbai
How to avoid sleep disturbance?
Ideal sleep duration is maximum six to seven hours a day.

The following tips help one to avoid sleep disturbance:

A) have bath regularly in evening after coming from work or before going to bed.
Early to bed and early to rise (awake) is a nice process for natural sleep cycle.

B) do not engage mind in active work or stressful talking at night before bed time.

C) do not engage mind in too much of computer work in night before bed time (for example, do not do stock market work in night).

D) do not read or view illicit or pornographic material at any time preferably.

E) do not take heavy meals in night. Best is to have heavy breakfast, lighter lunch and lightest dinner to maintain stomach rhythm which affects sleep. Vegetarian diet is best for good sleep.

F) do not over engage mind in television in night.

H) do not smoke cigarette, chew tobacco, tobacco containing masala, or similar intoxicants which can disturb rhythm of mind and disturb sleep cycle.

I) no tea or coffee in night (preferably cocoa and caffeine containing drinks should be totally avoided for best results).

J) listen to relaxing music and even then if you do not get sleep, then hear soothing stories or nice talks.

K) read books which not only calm the mind but also nourish the thinking process.

It is best not to depend on sleep medicines for sleep disturbance as in long run, they lead to dependency and increased disturbance.

Never rely on alcohol for sleep. Some people use alcohol to help them sleep, but that's not a good idea. At first, drinking alcohol may make you sleepy and help you fall asleep. But when you drink alcohol, you are more likely to wake up later in the night and have a hard time falling back asleep. This can leave you feeling tired the next day.
