How to Surgically Treat Cleft Palate

Cleft palate or palatoschisis is a common genetic abnormality that leads to a horde of problems and is presently a growing challenge to medicine practitioners. The major developmental stages affected due to this particular irregularity include feeding, speech development, dentition and maxillofacial growth which are rather important to the normal overall developmental pace of an individual. Even though the cleft palate deformity was defined centuries ago, no fixed management algorithm exists for patients suffering from the condition in the present day scenario.

Cleft palate may be successfully fixed using reconstructive surgery. Multiple specialists are involved in the reconstruction surgery including plastic surgeons, otolaryngologists, nutritionists, oromaxillofacial surgeons and speech pathologists. Some hospitals also consider psychological therapy for the patient and the family to help get through the emotional trauma and the issues faced due to developmental backlogs.

The treatment for cleft palate usually begins around 9 to 12 months of age. If left untreated, it may cause major deformities. It takes about some years before the whole procedure is completed although it depends on the type and severity of the deformity.

The process involves the administration of anaesthesia after which the palate repair closes the inner, middle and final layers and at the same time realigning of the palatal muscles in a technique called anintravelarveloplasty is conducted. This ensures that the muscles are adjusted in a normal position which facilitates the best functioning of the palate during feeding, swallowing and speaking. It is possible that the child might require more than one surgery to completely close the palate.

Some of the risks involved during the process include:
1. Abnormal reactions to the medications
2. Bleeding
3. Problems in breathing
4. Need for more surgery
5. Infection

Although complicated and time consuming, cleft palate must be given immediate attention to avoid serious developmental issues. The reconstruction surgery and therapy combined ensures a normal development for the child in the longer run, given the surgery was done at the correct time. The child would be required to remain at the hospital for about 5-7 days. Complete recovery takes a time period of 4 weeks. lybrate-user, Thanks for the query. If the milk intake is inadequate, infant may not pass much stools. Now a days other than breast milk nothing is to be given in first few months. It is better to consult your pediatrician. But do not give sugar water for now. Breast milk is the only nutrition for the baby. Thanks.

Sometimes small amount of vomits may occur even after burping. It is common in this age. Try to place a folded towel under her shoulders so that the regurgitation of milk is reduced. IPV is mandatory, yes. But you can receive it whenever available. 2 doses within 2 years of age is sufficient for immunity.

She is a high risk baby with stormy neonatal period. What you need to know is that your baby is likely to have neurological deficit for long term. The magnitude of neurological dysfunction and severity will be evident later on. But the most positive part is that brain has huge potential of neuronal plasticity during initial period of life, say first 2 years. With developmentally supportive nursery and good physiotherapy and occupational therapy you baby can perform her best. So you think about the positive side and try your level best.

You should not feel guilty and it is common for for many women to go for work. Yes you can express milk and give it. If you keep in fridge it should not be boiled but keep outside for one hour before giving it. Now is the time to introduce homemade semi solids as well, and feed when you come back.

To build his immunity give him nutritious diet which includes high protein with supplements minerals and vitamin. Also give iron zinc & folic acid. Probiotics also help build immunity.

Hji some kids are fussy eaters. Try to inculcate good eating habits. Don't introduce wrong foods. If she is active and then skinny you don't need to worry much. Whatever is being made at home should be served. Take care.

General instructions •Take meals in frequent intervals (ie every 2-3 hours) •Do not skip the meals •Take munakka, makhana, khajoor, anjeer, chuhara, nuts with milk •Take compulsory milk and milk products like yogurt, shakes, smoothies, sweet lassi, paneer, khoya etc. •Add soya flour, chana flour in your wheat flour chapatti. * take protien rich foods likes - paneer, soya, kala chaana, besan, egg, lobia. Some weight gaining snacks are: •Paneer tikka •Soya chaap •Tofu sandwich •Green mung sprouted sandwich •Paneer vegetable tikki •Cheeku shake, banana shake, anjeer shake.

If it is a long lasting problem, she may be suffering from allergic rhinitis. A clue to that would be other family members including parents or grandparents also suffering from some type of allergy. If so we would need anti allergy medicine, either orally or as nasal sprays.

In small children atleast for first 3 months it is better to avoid going out and visitors. Other than that maintenance of good hygiene and every 3 hrly breastfeeding will be sufficient.

Irregular,painful periods are normal in adoloscents. If periods are scanty,irregular,painful associated with weight gain,mood swings consult a gynecologist who will ask for USG abdomen,blood tests.This may be a case of PCOD.
