If you are worried about the average time to conceive, the first thing you need to do is relax. Chances are, you will get pregnant in time. But comparing yourself to others and worrying about why you haven't conceived yet puts too much pressure on yourself and your partner to fit unrealistic expectations. I have posted my question several times but still didn't get a proper answer. I am 29 years old having hypothyroidism. But my tag is under control. My lower pelvic ultrasound scan was ok. I had irregular periods as I was diagnosed with thyroid 2 years back. But my periods became normal since January this year. But from July 24 again I am not having periods. Can you help me Regularly I am exercising also. Please help me out.

lybrate-user, If hypothyroidism is under control then periods usually become normal, provided that is the only cause. Sometimes there may be other hormonal issues also involved, then one needs to investigate further to find out exact cause. If hypothyroidism is in control then TSH should be between 3 to 4 m U/L. If the value is higher than this then irregularity invariably persists. You have not mentioned the current TSH level, which will give an idea about the control of hypothyroidism. Thanks.

Infertility is a condition where a couple, trying to conceive in a natural way is not able to do so even after one year of trying. It is also a matter of worry for patients who are over the age of 35 years and those who are not able to conceive naturally even after 12 months of trying constantly. So here are the causes, risks and treatment for this condition.

There are many causes of infertility. It may be down to a condition present in one or both partners. As per a medical study, 20% of the cases of infertility are due to males, while 40% to 50% cases are due to problems in the female partner. 30% of the cases suffer from infertility due to problems in both males and females.

Ovulation and fertilisation are the main elements of the conception process and any condition in the female or male can upset either function, which can make it difficult to conceive. While abnormal sperm production and transport may be a common condition that leads to infertility in males, other factors like side effects of cancer treatments and too much exposure to toxins and chemicals can also play an important role in this regard.

For females, the causes of infertility may range from ovulation disorders, uterine and fibroid tumours, uterine and cervical anomalies, damage to or blockage of the fallopian tubes, endometriosis, primary ovarian insufficiency and pelvic adhesions. Also, factors like thyroid, cancer and medication leading to temporary infertility may be at play.

There are a number of factors that put a male or a female at risk of infertility. Being in your mid 30s usually has an adverse effect on the ovulation and production of well functioning sperm. Further, smoking and tobacco are among the main causes of infertility in either partner. Excessive alcohol consumption may also lead to this problem. Being overweight or underweight can also make a person infertile. Further, not getting enough exercise and routine activities out of your day, can lead to this condition as well.
Male patients can go through a treatment for any existing infections, hormonal injections and medication, surgical options like vasectomy reversal and art or assistive reproductive technology. Female patients can turn to medication that will stimulate ovulation, intrauterine insemination, and surgical processes to correct a uterine septum and remove endometrial polyps, or even a hysteroscopy surgery.
If these methods fail, the patient or the couple can always turn to methods like in vitro fertilisation. For other mild reasons, lifestyle changes along with a stress free environment can help in increasing the chances of conception.

I am 37 year old. Missed my periods by 11 days. Same happened back in jan 2015 and it was not pregnancy. Consulted my gyne n she did few harmones test all came normal. She prescribed me medicine to induce periods. Now again facing same issue. What could be the reason?

Hi good u consulted your gynac. it could be due to any fever or common cold, or anaemia if your working n in mum, large no of female encountering menopause at such early age, Ayurveda says its vat disorder and to managed by with through investigation like thyroid, usg abdomen till then have 1)kumari asav 4 tsf twice a day 2)ashwagandha tab 1 at bed time jira n ajawayan added warm buttermilk after food at noon.

To get pregnant you need to have fallopian tubes patent which is checked by hsg test and semen should be adequate quality which is checked by semen analysis. These both tests are done before the treatment of any infertile couple. Once the above tests are normal, then treatment with clomiphene citrate is started with monitoring of egg. You need to consult a good gynecologist who would start proper treatment for u.

Dear lybrate-user, you will have to wait out for the next periods but since you have taken pills at the right time so should not be a problem. You can repeat a pregnancy test 10 days later from the 1st day you took the pills. Best regards.

It can also happen in those who physically strain like long journey travels exercises or carrying of heavy weights etc.. What is Aeroplane Ear?

Pain in the ear, popping ear, blocked ear during air travel is the most common medical problem faced by flyers. Aeroplane ear is the better known name for Barotitis media or Aerotitis media. Aeroplane ear or Barotitis media is the condition where there occurs damage to the ear due to difficulty in adjustment of the air pressure between Middle ear and External ear

Why do your ears get blocked in an Aeroplane?

I have to go into a bit of physics for a proper explanation. Please excuse the science Lecture! As you know according to Boyle’s Law : The pressure and the volume of an enclosed mass o gas are inversely proportional. When the plane rises in the air, there is a progressive decrease in air/atmospheric pressure. Basically at 18,000 ft above sea level, the air pressure is half that at sea level.During the rise through the atmosphere , as the pressure falls ,a particular mass of air in an elastic structure expands .Similarly during descent, the air contracts and the volume of cash decreases.

So what happens in the ear during Flying?

Imagine your ear is made up of 3 rooms.The outermost part of the ear which connects the entryway with the other rooms is the external ear.This is the part involving your ear canal. The ear drum separates your external ear from the middle ear, where the small bones (malleus, incus, stapes), which help transmit sound live.The ear drum is somewhat flexible and can move with change in volume of air in the middle ear. Two windows oval window (covered by stapes footplate) and round window (covered by round window membrane) connect your middle ear and the inner ear.

The inner ear has your hearing and balance apparatus It’s called the labyrinth. A tube connects your middle ear with the part of your oral cavity behind the nose (nasopharynx) .It’s called the Eustachian tube.Easily the most important structure which helps keep up the normal air volume inside your middle ear. It’s through your Eustachian tube that air enters or exits to equalise air pressure between outside atmosphere and your middle ear.

What happens during Ascent?

Air pressure outside falls. Middle ear pressure falls. Air in your middle ear expands (following Boyle law) This excess air pushes the tympanic membrane outwards to its natural limit.This is normally followed by escape of air through your Eustachian tube. Movement of the ear drum may be restricted due to scarring (previous infection).This may cause ear pain during ascent.

What happens during Descent?

Air pressure outside rises. Middle ear pressure rises. Air in your middle contracts and volume decreases (Boyle law). The ear drum moves inwards. (retraction). The opening of your Eustachian tube must be opened to allow air from the nasopharynx to enter and adjust the volume.The Eustachian tube will open with swallowing movement. If this mechanism fails (if you fall asleep, or have a cold), or is delayed, the increasing pressure difference closes the naspharyngeal end of Eustachian tube.When the pressure difference is more than 90 mm,Hg, your tubal dilator muscles are unable to open the Eustachian tube and it stays locked.The ear then feels blocked.

Barotrauma occurs during flying due to:

1) Very rapid air pressure changes.Sudden ascent/descent. 2) Blocked Eustachian tube.

It may also occur in:

Hyperbaric oxygen chamber
Decompression and compression chambers

Who is more likely to get Aeroplane Ear?

Anybody who has some nasal or Nasopharyngeal problem which blocks the Eustachian tube.
Acute infection in the nose.
Chronic infection in the nose
Nasal allergy
Vasomotor Rhinitis
Malformation of nose
Nasopharyngeal tumour.
Sleeping during ascent or descent
Small narrow Eustachian tube; infants toddlers.

How will you know whether you are developing aeroplane ear?

Symptoms ( most common)

Discomfort in the ear
Ear Muffled hearing
Blocked ear/fullness of ear
Severe complaints

Excruciating pain in ear
Severely blocked ear
Ringing, buzzing sound in ear (Tinnitus)
Severe Hearing loss
Spinning sensation (Vertigo)
Bleeding from ear

What will your doctor See?

An ENT surgeon may do an otoscopic examinations to see your ear drum.

Signs on otoscopy:

Invaginated/retracted ear drum

Congested ear drum

Solitary or multiple blood filled blisters on your ear drum


Signs of serous otitis media

Ruptured ear drum, with blood clot.
Otoendoscopy may also be done where you will also be able to see these findings along with your doctor.

Are there any long term complications ?

Rarely the ringing sound in ear or tinnitus may be permanent.

If inner ear barotrauma occurs hearing loss may not improve

What other tests can you do?

Pure tone audiometry; to check your hearing and rule out inner ear barotrauma Tympanometry:To check the air pressure in the middle ear.and find out if there is any fluid.

Aeroplane ear:How to prevent it during flights

Don’t fall asleep during flying, specially during ascent or descent.
Avoid flying if you are having a severe cold.If emergency then use a Nasal decongestant spray just before takeoff and landing.Also take a decongestant orally, before getting on the .Dont use over the counter sprays .They may cause even more problem in the long run.
Take plenty of chewing gum so that you can actively help in Eustachian tube opening during flight.The chewing and swallowing movement helps to open Eustachian tube.
Try Valsalva maneuvre.This is a pressure equalizing technique which helps to pop the ear.You can try this if repeated swallowing and yawning doesn’t help.
Pinch your nostril, take a mouthful of air,close your mouth and try to gently direct the air into the back of your nose.Imagine you are trying to blow your nose gently. You should feel a pressure buildup.But do not let the air out.If you hear a pop you will know the ear is unblocked.

What can young children do to prevent aeroplane ear?

Very young children have difficulty during fflying.Often you will find them irritable crabby , pulling at their ears and may also cry incessantly. This is because the Eustachian tube in infants and very young children are not mature enough to handle the extreme change in pressure.
But you can help them too.

Dont let them fall asleep.
Play with them.
Carry a few pacifiers so that the suck and swallow reflex constantly works.
Talk to your baby doctor whether a decongestant oral medicine is advice able for your child.
A plain saline nasal drop given just before take off and landing helps to increase the swallowing movement and equalise pressure effectively.
What can you do to unblock the ear post flight?

Continue the Valsalva maneuvre/pressure equalizing technique
Normally it’s self healing and gets better within a few hours to few days.
If symptoms are uncomfortable and persists , or the ear doesn’t pop you should visit a doctor specialised in ear diseases
If your nose is blocked , avoid forceful nose blowing and take steam through your nose.This helps to relieve nasal congestion and open up the Eustachian tube.

What your doctor may advice?

Nasal spray; decongestants/steroids
Oral decongestants
Oral antihistamines
Non steroidal anti inflammatory drugs for relieving pain
Once medication have improved Eustachian tube function Valsalva maneuver helps to force open the blocked tube.. Surgery is rarely needed.

If fluid in the middle ear persists even after 3 months myringotomy (incision in the ear drum) is done to drain the fluid.

If the perforation in ear drum does not improve after 3 months tympanoplasty to repair the ear drum may be needed. I hope next time you are flying you will be carrying some chewing gum or hard candy with you.

Bad diet
A balanced diet helps you maintain your weight and keeps you energized. Eating a lot of junk food can make your blood sugar level fluctuate. When the sugar level drop below normal, the person is likely to feel tired and drained out.

Anemia is a condition where the body suffers from a lack of iron. Without enough iron the red blood cells in blood cannot carry oxygen to all parts of the body. This makes the heart work faster to pump blood and can make you tire easily.

Caffeine is said to wake the system up and boost energy levels. However, like sugar, caffeine make energy levels fluctuate. As the level of caffeine in the body reduces, the person will begin to feel tired. Not drinking enough water along with your coffee can worsen this situation.

Water is necessary not only to keep the body hydrated, but also to keep it energized. This is because the loss of fluid in the body can result in lowering the blood volume. This makes the heart work harder to pump oxygen to the brain and other parts of the body.

Fatigue is often not diagnosed till very late. There are many reasons for this. Primary amongst these are:

1. Age: We assume that we are feeling tired only because we are growing older. However, the cause of fatigue may have nothing to do with age.

2. Gradual onset: We are often not aware of how tired we are until we reach a point where we are completely drained out. Fatigue has a very gradual onset and is rarely noticed in the early stages.

3. Misinterpretation: Complaints of feeling tired are often mistaken for lack of motivation and laziness.
The easiest way to prevent fatigue is to maintain a regular waking and sleeping schedule and to eat a healthy diet. Also, try and be physically more active.

Hi . We can wait upto 1 week after the expected delivery date for labour pains to start on their own but that has some risks involved. Waiting after the dates can result in decreased liquor of the baby, causing stress to baby, which in turn will make the baby pass stool in uterus, if the baby inhales the stool then she will having lung infections, will not cry after birth and may need nicu admission. I usually do not wait beyond 2-3 days after crossing the dates for this very reason. I suggest you visit your doctor and get the baby examined by nst (non stress test) and ultrasound to see for liquor. If both are fine you can wait for 2 days. These tests need to be done alternate day after the expected dates to ensure that the baby is doing well inside. Your doctor will advise the best after seeing the reports. Menses can be missed either because of hormonal changes or even stress or even if you have put on some weight in recent times then also menses can be missed. All these problems are temporary and menses will come on its own. You can wait for another 15 days before you can take tablet deviry to get menses. In the meantime, keep repeating urine pregnancy test every week till you get your menses.

Hi lybrate user, saffron doesn't make baby fair. Colour of baby depends upon genetic condition of the parents. Saffron has add on effect on growth and development of the baby. It will improve blood supply to babies which will improve growth of the baby. It also help to immunity also.
