Hello, thanks for your query and welcome to lybrate. I am dr akshay from fortis hospital, new delhi. You will have to elaborate your problem as to a detailed discussion of your back pain which means duration of pain and mode of onset. I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias ( electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem. Ideally since it is a 2 year story I would advise to get: 1) x ray ls spine - ap & lateral( flexion/extension) views 2) mri ls spine and then send all reports across to me. Do not hesitate to contact me if you need any further assistance. Thanks & regards Dr Akshay Kumar Saxena

Hello, thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Please tell me duration of pain and what was the initiating factor for it? what medications you generally take and what has been your treatment history till now for existing problem. I would like to know whether you have got your inflammatory profile done till now and whether calcium and vitamin d testing/supplementation has been done. A lot of knee pain in young adults is due to a condition called as chondromalacia wherein intensive physical therapy, calcium and vitamin d supplementation, short course of anti inflammatory medications and avoidance of certain activities initially will help. Also please let me know what all exercises you are doing on a daily basis. Do not hesitate to contact me if you need any further assistance. Thanks & regards

Hello. Maintain a right standing and sitting posture, keep your back always straight (even while sitting or standing), avoid carrying weight on back. Hold or carry weight divided equally in both the hands or shoulders (to avoid over straining on one particular hand or shoulder). Always select a chair with proper back support and hand-rest. (Excessive sitting or standing is to be avoided as well don’t carry much weight on shoulders). Apart from these, stress could be the major cause of pains! Exercise: Practice Shavasana or Corpse Pose as it looks like a dead body. If practiced correctly and with full efforts, can relax every body part, relaxing not only the body but also the mind: Spread the legs one to two feet apart, the toes are turned outwards, the heels facing each other, a comfortable distance apart. Bring the arms a little away from the body, palms turned upward. Relax the neck and allow it to turn to the side if it is more comfortable. Close the eyes and focus the attention on the body, breathing normally. Begin focusing each body part and relaxing it, then moving on. Keep the mind focused on relaxation, the breath should be normal. Relax the whole body. Management: -Sit straight with a back-rest or back-support. -Stretching or activities that place additional strain on the back are discouraged. -Sleeping with a pillow between the knees while lying on one side may increase comfort. -Lying on your back with a pillow under your knees would also relieves pain and no pillow under your head. -Some people seem to benefit from the use of ice or heat. Take care, do not use a heating pad on ‘high’ or place ice directly on the skin. Medication: Take homoeopathic medicines - Arnica 200 and Rhus Tox. 200 - take both of them thrice daily for 1 week. Also, apply Schwabe's Topi-Arnica and Topi MP Gel on the affected area in circular motions gently thrice a day. Get back to me thereafter.

I have a shoulder dislocate. And whenever I throw any heavy stuff or in force. I get pain . How do I fix it? Any exercise for me? Please help

You appear to have torn a ligament (glenoid labrum) in your shoulder. You need to get an MRI done for your shoulder. Considering your young age, you need key hole surgery for your shoulder (Arthroscopic Bankart repair) to prevent further dislocations and further damage.

For frozen shoulder exercises are the treatment. Severe pain can restrict your shoulder exercises, so take anti inflammatory drugs like Ibuprofen round the clock and once pain is relieved start exercises. If pain is not relieved with medication you can get an injection called Suprascapular Nerve block done by a Pain specialist to get good pain relief and start exercises.

I have last 2 years back pain what should I do?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist,
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have issues with your lower back since 2 years and ideally we should get it thoroughly investigated in the form of x rays, mri and a thorough clinical evaluation to proceed further. But if you have localized mild to moderate back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.
