My mother is 51 years old, and over the past one year, she has been having cough problems. This seems to happen every two days (sometimes before food, sometimes after). On some of these occasions it is just cough, but on others it leads her to throw up (not necessarily food, sometimes only phlegm). One of the previous doctors she had consulted, recommended she take an endoscopy of esophagus, but she is not comfortable with that. Do these symptoms have anything to do with menopause? is there an alternative to endoscopy? if endoscopy is a must, can it be done with anesthesia? kindly advice. Someone I know from work has consulted with you for their mother who had similar symptoms, hence seeking your guidance.

DNB (ENT), MBBS
ENT Specialist, Bangalore
Hi . These symptoms don't appear to be related to menopause. It sounds like she has acid reflux disease or laryngopharyngeal reflux disease. There is a simple way to rule this out. Give her an antacid like tab sompraz 40 mg or tab neksium 40 mg empty stomach once in the morning for a week. If her symptoms improve for 2-3 days with over 50% recovery then the diagnosis is likely and she can continue the medications for 3 weeks. If there is no change in her condition then she is recommended to undergo an endoscopy which can be performed under anesthesia if requested to the gastroenterologist.

The condition is termed as Metrorrhagia also known as the dysfunctional uterine bleeding is a problem that women suffer from. Metrorrhagia is common, with patients who are near to their menopausal phase. Basically, this condition is characterized by bleeding episodes (mainly spotting but can lead to severe bleeding) outside the menstrual phase. Metrorrhagia is considered a symptom of a possible health problem but shouldn’t be handled as a serious health problem right away. One should not panic. Some of the main cause are----------- • Endometrial hyperplasia. This condition is said to be a result of abnormal levels of estrogen and decreased levels of progesterone. This abnormality in hormonal levels would then prompt excessive endometrial cells. • Uterine cancer. This is considered a common cause of the metrorrhagia. Increasing age is related to uterine cancer, making postmenopausal women as victims of the uterine cancer. Tumors such as those found in ovarian cysts can result to abnormal off-menstrual bleeding. • Ruptured polyps. There are cases where a woman is unaware that she has cervical polyps. Somehow, after sexual contact, it will be identified. Postcoital bleeding should not be taken lightly, as this may indicate for presence of polyps in the cervix or endometrium. • Cervical dysplasia. This condition is a pre-cancer stage that involves vaginal spotting as a symptom. This may be indicative that cancer development is in progress. • Cervicitis. This is known as infection of the cervix. Bleeding during the infection would indicate prompt attention as this may progress to a chronic problem. • Abortion or miscarriage. This is a common cause of vaginal bleeding. There are cases where after abortion, fetal parts are left that can cause for vaginal infection and bleeding. • Infection. Sexually transmitted infection/s is a probable cause of metrorrhagia. Syphilis is known to cause vaginal bleeding. Those who have Lyme disease are prone to metrorrhagia as they become susceptible to infections. • Hormonal imbalance. There are underlying conditions such as hyperthyroidism/hypothyroidism that cause hormonal imbalance. • Coagulation problems. Women who have an underlying coagulation (blood clotting) problem such as those prone to bleeding are highly susceptible to metrorrhagia. • Use of contraception. The use of devices that can traumatize the vaginal or uterine lining can cause bleeding. IUD is a common device to cause metrorrhagia. Treatment depends upon the cause of Uterine bleeding and for that a thorough physical examination is required. So my advise is to immediately consult a gynaecologist so as to diagnose the underlying cause at the earliest and treatment given accordingly.

I am 22 and had sex for the first time a week back. After the second or third time I initially felt like my bladder was full and wanted to urinate all the time but that went away in a day. Just to be sure I took Neil-72 the next morning even though we had used condom. I had sex again 3-4 hours later after taking the pill but this time the condom broke. So I took Neil-72 immediately and once again within three hours just in case because I was really scared about pregnancy. It's been three days now and I haven't had any side effect from the pill except that my vagina itches. I very rarely feel pain while passing urine though there has been a strong medicinal smell from it. And it has been a little cloudy too. However, I could urinate after sex. I had the problem of vaginal itch in the past when I was young but haven't had it in years until now. My partner had sex only twice with one girl but that 4 years back. Last period- 16th Dec- 21st Dec Sex- 24th Dec- 1st Jan (5 times in total) Next period due- 16th Jan. Could this vaginal itch and the medicinal odour of urine be the side effects of the pills (and the overdose)? Or is this UTI or something else? Should I get a check up? Thank you

As far as your bladder cpmolaints are concerned then you may take homoeopathic medicine Staphysagria 200 two pills in half cup of water only one dose. Follow up if required.

You would need to see a Gynaecologist as you would need medicines prescribing to help you. If these do not work surgery may have to be considered

For family planning, what option can be considered as most effective and convenient and for whom - husband or wife ?

For family planning, what option can be considered as most effective and convenient and for whom - husband or wife ?
If family is complete , husband can have a blood less no stitch vasectomy done and go back to work on same day. in a woman , key hole surgery for family planning though looks like one stitch surgery only but is as traumatic as appendix surgery.she needs at least rest for 15 days. IUCD if fitted in hygeinic conditions suits a lot of women but it is scary for most women ,courtesy vans and untrained personels fitting women with it in rural areas oral pills are for short time only-3-6 months low dose like ovral ( mala d has 3 times the dose !)

Why Having a Pet Improves Your Mental Health?

Why Having a Pet Improves Your Mental Health?

Pets can be amazing companions and help you cope with your life problems. There can be times when you might feel low and lack motivation to do anything; a pet can change that scenario and bring about a positive change in your life. They keep you engaged and help you take your mind off from various problems in your professional and personal life.

Here are few reasons why having a pet can significantly improve your mental health:

  1. Petting lowers stress levels: Petting your furry friends is a major stress buster. It has been observed that petting a dog or a cat releases the hormone called oxytocin, which is associated with lowered anxiety and stress levels. When you connect with your pet and engage yourself in grooming and petting, it naturally reduces your blood pressure and lowers the level of cortisol, thereby, producing a calming effect.
  2. Pets reduce loneliness and isolation: Pets can constantly keep you company, if you don't like being lonely. Dogs and cats can make friends easily and due to their intuitive nature, they can sense it when you are feeling low. They refuse to leave you alone and provide your mental support. Pets also help you socialize especially if you take them out for a walk. They help you interact with other pets and their owners.
  3. Pets are great listeners and they keep you engaged: Pets can distract you from your problems and stressful situations, which plague your mind. You and your furry mate can spend a wonderful time playing games or by simply clicking pictures together. Pets are also good listeners and hence you can vent out your feeling without the fear of being judged. They help you heal and get over certain things which would otherwise be a lot more difficult to overcome.
  4. They love you unconditionally and keep you happy: Seeing your pet welcoming you after a hard day at work can instantly lift your mood and make you smile. They love you no matter what mistakes you made and keep you energized, positive and motivated. Caring for your pet and reciprocating their love can make you less self centered and more motivated towards your goals.
