If you are a regular smoker, motivational therapy with a trained psychotherapist or a psychiatrist and enormous will power will help you quit. If you are a diseased smoker, consult a de addiction psychiatrist at the earliest. He may give you anti-smoking medications and de addiction treatment.

Hi, carbonated water is water into which carbon dioxide gas under pressure has been dissolved. Some of these have additives such as sodium. Most carbonated water is sold in ready to drink bottles as mineral water and carbonated beverages such as soft drinks. Whether homemade or store-bought, soda water may be identical to plain carbonated water or it may contain a small amount of table salt, sodium citrate, sodium bicarbonate, potassium bicarbonate, potassium citrate, potassium sulfate, or disodium phosphate, depending on the bottler. By itself, carbonated water appears to have little impact on health. While carbonated water is somewhat acidic, this acidity is quickly neutralized by saliva. Carbonated water may increase irritable bowel syndrome symptoms of bloating and gas due to the release of carbon dioxide in the digestive tract. There is tentative evidence that carbonated water may help with constipation among people who have had a stroke. Some carbonated waters have added sodium, so they may be an issue for those on low-sodium diets. Carbonated soft drinks such as colas do have health risks. Carbonated colas have a correlation with slightly decreased bone density in older women. Soft drinks are about 100 times more erosive to teeth than plain carbonated water. A soft drink (see terminology for other names) is a drink that typically contains carbonated water, a sweetener, and a natural or artificial flavoring. The over-consumption of sugar-sweetened soft drinks is associated with obesity, type 2 diabetes, dental caries, and low nutrient levels. Many soft drinks contain ingredients that are themselves sources of concern: caffeine is linked to anxiety and sleep disruption when consumed in excess.

Hi, make changes to your computer screen, such as: place the screen 20-26 inches away from your eyes and a little below eye level. Regularly clean off dust and fingerprints from the screen. Smudges on the screen can reduce contrast and increase problems with glare and reflections. Choose screens that tilt and swivel. Consider using a glare filter over your screen. Make changes to your work environment, such as: change lighting to reduce glare and harsh reflections. Use an adjustable chair. Place a document holder next to your computer screen. Make changes to your work habits, such as: try the 20-20-20 rule. Every 20 minutes, look away about 20 feet in front of you for 20 seconds. Post a note that says" blink" on the computer as a reminder. Take regular breaks from computer work. Take care of your eyes with these steps: apply a washcloth soaked in warm water to tired, dry eyes (with eyes closed). Use artificial tears to refresh your eyes when they feel dry. To help prevent dry eyes while working indoors, use an air cleaner to filter dust and a humidifier to add moisture to the air. Management: give sufficient rest to your eyes with proper sleep. Keep yourself hydrated with intake of 10-12 glasses of water in a day. Eat healthy especially vitamin a rich food (green vegetables, carrots, yellow fruits and vegetables) and avoid junk food. Hold tap water in your flexed palm, dip your one eye closer to the water level, shut and open your eyelids often. Do the same with your other eye. Repeat this whole process 1-2 times more in a day. Give your eyes proper rest. Can keep slices of cucumber or potato or used tea bags over the closed eyes for sometime. Medication: instill 2 drops of euphrasia eye drops in each eye twice daily.

Dear Mr. Lybrate-user, kidney stone upto 3mm diameter is expelled through urine by administering homoeopathic mother tincture berberis vulgaris q, 30 drops, thrice daily after meals, in a cup of water for 15 days. If the size of the stone is more than 3mm then it would be better to go for surgery.

Most probably you are suffering from oa of knee. Knee surgery-total knee replacement (tkr) is quite safe & gives uniformly good results. We have well equipped center in delhi. Do ask for detailed treatment plan. Surgery should be done when patient is still active & will be able to take benefit of surgery. Do not ignore. It could be beginning of a serious problem. Kindly show me x- rays before a final decision can be taken. You will be thoroughly investigated before surgery is under taken. Risk in surgery is less than the risk in crossing road. Common Types Of Skin Incisions

Ideally, surgical incisions are primarily designed to allow easy access to the desired areas in addition to possibilities of extension, if required. Further, they should be designed to split muscles rather than cut them, with minimal scarring along with the aim of boosting regenerative and healing qualities.

Incisions can be classified under these three groups; Vertical, Transverse and Oblique incisions.

  • Vertical incision

1. Midline incision: Almost all abdominal incisions can be performed using this technique. Starting from the midline of the abdomen, it can extend all the way down to the umbilicus.

Advantages:
a) Minimal blood loss
b) Minimal nerve injury
c) Minimal muscle injury

Disadvantage:
a) Midline scar

2. Paramedian incision: This technique provides more laterality to midline incisions, allowing access to such lateral organs such as spleen, kidney and adrenals.

Advantages:
a) Easy access to lateral structures
b) Closure between incisions in anterior and posterior sheath is more secure
c) Rectus muscle remains undivided

Disadvantages:
a) More time consuming
b) Difficulty in extension
c) Can result in atrophy of the muscle

3. Mayo-Robson incision: This incision is typically a paramedian incision, but which bend towards the xiphoid process and consequently allows for a larger and wider opening.

  • Transverse incision

1. Transverse incision: This type of incision is made just above the umbilicus, and can dissect either one of the rectal muscles.

Advantages:
a) Least amount of pain and damage
b) Muscular segments can be reattached
c) Easier access to upper GI structures

Disadvantages:
a) Limited lateral access
b) Higher risk of wound infections

2. Subcostal incision: Also referred to as the Kocher subcostal incision, this type of incision starts from the midline and runs parallel to the costal margin. A double Kocher incision is known as a rooftop of Chevron incision and allows for access to the esophagus, kidney, stomach and liver. The Mercedes incision is yet another variant, characterized by a vertical incision from the rooftop incision to form the shape of a Mercedes sign.

Advantages:
a) Heals faster
b) Less risk of post-operative complications

Disadvantage:
a) Lengthy and time-consuming

  • Oblique incisions

They are also known as Thoracoabdominal incisions, these incisions may either be situated in the RUQ or LUQ. They provide entry to the liver, lungs and spleen, as well as to the stomach and esophagus.

  • Laparoscopic incision

This technique comprises of tiny cuts in the skin that allow laparoscopic tools and instrument access into the abdominal cavity

I often get tonsillitis every 2 months. Before 2 years I took b12 injection n la tablet n got sore throat or tonalities rarely, may b 3 times in one n half year but since last 3 months getting it again n again. When I take antibiotics or b folcin plus tab it heals but again it come after month. I have met many doctors but no one have permanent solution for this.

Mr. lybrate-user, A person with diabetes has to take a balanced diet, which includes carbohydrates, fats and proteins. Rice is one of the carbs, it can be taken by a diabetic person. In south India, where rice is the staple diet (Kerala, TN, & AP, South Karnataka), it is consumed by every one. Foods to be avoided are sweets, deep fried foods, junk foods, pastries, bakery items, other maida items, sweetened aerated drinks, sweet corn containing products, highly processed food products and high fat milk products. Total caloric intake has to be based on ideal body weight, extent of daily physical activity and blood glucose levels. Fruits like papaya, water melon, pomegranate, orange, sweet lime, apple, pear and guava can be eaten. Thanks.

Sir, I am a diabetic and high blood pressure patient. I take Istamet tab.(50/500) twice daily and Telma H/Lipril 5 1 tab. Each morning and evening. I am suffering from constipation for a long time. I had fissure problem. Now fissure problem is much less but fistula-in-ano has recently been operated. I take diabetic diet and fat free salt restricted diet. But constipation problem is there. I have to take cremaffin, lactihep, mucibael, softovac SF etc. Regularly. Please advise.

Mr tapash kumar, try and increase fiber content in your diet by consuming more salads, flax seeds (25 g, roasted) or fenugreek seeds (roasted or powdered). Fruits which are permitted like papaya, apple (with skin), pear and guava. Drink enough water. Normally these measures help in overcoming constipation. If nothing works then the laxatives which you are already taking.

All the 4 points are true as of now in my case. I often shout on others realizing later that they were right. Feeling alone inspite after thinking that I wish I would be at home for some relaxment. Have lots of friends but always looking for true friend. This is called phobia. There is logical reason but person fears about particular things. There is no permanent in the world. No one is pure. We have to go as per the recommendations of the nature. We do not think too much about future. If think it may turned at our point no it is baseless thinking. Because of your internal frustration and no clarity about yourself may cause for shouting. First create a strong self esteem. Then everything looking very positively. If you are not settled in one area your thoughts may come baselessly. So first develop a strong self confidence. Do the work very confidently. Feel always everyone is positive. If some are negative then leave. Feel and enjoy every movement of life in a positive manner.
