Avoid alcohol. Normally alcohol in moderation is well tolerated but for people prone to acidity even a small amount of alcohol can cause discomfort. Avoid smoking .The nicotine in the tobacco affects the opening and closing of the food pipe allowing back flow of the normal stomach acid into the food pipe. Eat small meals. Instead of 3 big meals, have 5 small meals every 3-4 hours. Eat slowly. Don’t sleep on a full stomach. Try to have an early dinner. Make sure there is a gap of 3 hours between your last meal and bed time. Wear loose-fitting clothes. Avoid certain known foods which cause acidity like, coffee, tea, colas, and citrus fruits, oily and spicy food. If your weight is high – losing a few kilos will reduce acidity too. Keep a gap of 2 hours between exercise and meals. Drink at least 3 litres of water every day. Try meditation and yoga to minimise daily stress.

Avoid tea/coffe alcohol, smoking spicy food. Life style changes and food habbits can solve your problem easily. Chew your food properly. Avoid outside food. Walk about 3-5 kms daily. There should be gap of 2hrs after dinner and sleep. Small and frequent meals is of help. Keep your blood sugar controlled. Raise your bed about 4" from headend side. Get your gastroendoscopy done if it persist, consult doctor. Edit answer.

For painful piles and fissure avoid constipation by drinking plenty of water, decrease meat intake and dal, have good green leafy vegetables. That should prevent recurrence of the problem but risk will always be there. Regarding infection examination would be required.

8 Types Abdominal and Pelvic Surgical Incisions

F.I.A.G.E.S , MNAMS (Membership of the National Academy) (General Surgery) , DNB (General Surgery), MBBS
General Surgeon, Delhi
8 Types Abdominal and Pelvic Surgical Incisions

At the time of a surgical procedure, while making an incision a doctor has to take care of a number of factors before making an incision. Considering a number of factors, different types of incisions have come into fore, such as

  1. Midline Incision: It’s the commonest incision and is done along the linea alba (fibrous structure running through the mid of the abdomen). These are preferred, especially in diagnostic laparotomy as it permits a wide access to the abdomen.
  2. Pfannenstiel Incision: It is transverse in nature, extending from the umbilicus to the pubic-symphysis. It is generally employed for abdominal hysterectomy of benign nature and caesarean section.
  3. Chevron Incision: It is an incision under the rib-cage and is done on the abdomen. It starts from beneath the ribs on the right abdomen and extends till the other mid axillary line. Thus, the entire abdominal width is incised for proper reach into the liver. The incision can be up to 2 feet.
  4. Kustner Incision: It is transverse in nature and extends from the symphysis pubis till the iliac spine (anterior). This type of incision takes time to perform. A Pfannenstiel incision offers more exposure than a Kustner incision.
  5. Lanz Incision: It is a variation of the more common mcburney-incision (also known as Gridiron’s incision). It is generally used for open appendectomies. There are quite a few variations for this type of an incision.
  6. Gridiron’s incision: It is done for appendectomies. It is an oblique short incision which is done in the lower right quadrant in the abdomen.
  7. Kocher’s Incision: It is oblique in nature, extending from the abdominal upper right quadrant and is generally used for performing an open cholecystectomy. Gallbladder, biliary tract and certain liver operations can be suited for a Kocher’s incision. This however is different from the same named incision used for the thyroid gland surgery.
  8. Cherney Incision: It is transverse in nature. It allows a great range of exposure for the pelvic sidewall. It is less painful than a midline incision. It allows for the greatest pelvic exposure and hence is a widely preferred and practiced incision.

I have high sugar levels around 275 (random) due to chronic pancreatitis. What should be my ideal diet and also suggest me some fruits. How can I increase my weight.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr Abhishek, Diabetes with pancreatitis, needs to be treated with Insulin Injections because there is hardly any insulin production. All the permitted fruits like papaya, orange, sweet lime, water melon, apple, pear, pomegranate and guava etc are good enough. But only about 100 g a day. As regards adding weight you will have to first achieve a strict blood glucose control with fasting closer to 100 mg and PP 150 to 160 mg and HbA1c%<6.8. Then you might look at taking some protein supplement and regular exercise to build muscles. Thanks.

I suffer from pain in my lower abdomen whenever I have meals. It lasts for short time but occurs consistently. Mostly occurs after breakfast. Medicines do no good! All the tests (examinations) show that there is no problem at all. What should I do?

MBBS
General Physician, Cuttack
1. It could be due to gastric/duodenal ulcer, h. Pylori infection, gerd (gastro oesophageal reflux disease). 2. Take bland diet. Avoid spicy food, 3. Take one capsule of omez daily in the morning before break fast for 4 week. 4. If no relief, do upper gi endoscopy after consulting gastro enterologist and follow the advice and treatment.

Hi, How long have you been taking alcohol? First and foremost I would advise you to restrict alcohol intake and simultaneously meet a gastro-intestinal (GI) specialist, in person, with no further delay. He or she would be able to clinically examine you properly and may require you to go through a few diagnostics including a liver function test and USG abdomen as deems appropriate. You may also expect a referral to a neuro physician for further clinical evaluation should it require. Your ECG report being normal is indicative of no issues as such on your cardiac front. Pain in your sternum could be a referred pain owing to gastritis or issues, if any, with your liver. However, to be on the safer side, a cardiologist consultation would rule out any issues with your heart as well. As regards your gastritis issue, your GI specialist who you meet in person, shall also advise appropriately in this regard. At the same time, however, it is important that your diet and lifestyle be streamlined. You need to take more of wheat grain cereals, fresh fruits & leafy greens and ensure that the food is well cooked, bland yet tasty & easily digestible. Avoid hot, chilly, spicy & oily & greasy hard to digest foods. Also, restrict intake of tea, coffee and other high calorie foods like carbonated sugary drinks, packaged juices, sweets, pastries et al. It is good to take smaller amounts of food in regular intervals rather than taking three large meals. Sitting in ?Vajrasana? post every meal, helps digest better. Drink plenty of fluids, at least 8 ? 10 glasses of water daily and avoid taking water intermittently while eating as this will hamper effective digestion of food. Brisk walk for at least 30 to 45 minutes, 5 to 6 days a week, unless you have any other health issue or advised otherwise by your doc. Amla, ginger, mint, tulsi, ripe banana, cold milk etc also help arrest acidity & associated issues including gastritis. At the same time, you must stay relaxed and de-stress yourself through either prayer, yoga, meditation, deep breathing exercises, socializing etc all. Hope this helps. All the very best.

There is lot of flatulence later in the day. I always feel to go to toilet again and again. Is the incompleteness due to fissures. Is there any complete cure for fissures.

Dear , yes your troubles are due to anal fissure which is due passage of hard faeces in constipation which again is not a disease but a lifestyle problem-sedentary life. Here are some suggestions to help you. 1. Increase fluid intake 2.
