Joint Injuries and Disabilities
Hello friends! I’m Dr. Kamal Bachani. I’m practising Orthopedic Surgeon in Delhi for the last twenty years and plus.
Joint related injuries and disabilities. In India, alone involve every one in five patients.
Now, need being the common joint involved in these joint disabilities, being the highly mobile and joint which involves in regular locomotion.

So let’s talk about the knee joint, focus on the knee joint today. Knee joint problems can be involved in any age group right from the childhood to young adult, to an elderly age group and to very elderly age group.
We get the knee disorder problems in the form of pain, there can be stiffness, there can be arthritis of the knee joint, and there can be instability of the knee joint. Pain is one of the commonest features followed by stiffness in the knee in any form of the disease, in or around the knee.

Recent advancements have mainly focused on relieving of the pain because pain is one of the most disabling symptoms which can make patient feel incapacitated, make the patient feel depressed and dependent on others and this is the purpose of our treatment to remove pain and stiffness and make the patient mobile, active and create value in his or her own life.
We are able to treat pain by the use of modern science in the form of pharmacological therapy. We use visco supplementation. We use advanced rehabilitation techniques and wherever the anatomical factors are involved, we try to correct these anatomical factors in the form of cartilage regeneration in the form of correcting ligaments, imbalance, by reconstructing the ligament, repairing the ligament, or menisci repair, so and so on.

Now, talking about the structures, there can be involvement in front of the knee, there can be structures which can be damaged at the back of the knee, there can be structures which can be damaged inside the knee, like the ACL, the PCL, the menisci all these things can be managed now by very high precision arthroscopic surgery .
But majority, most important in knee disorder management, first point is, correct history and correct diagnosis.
Once we are able to correctly diagnosis, half the battle is already won.

After diagnosing the knee disorder problems correctly, we can plan so that the patient can be relieved of his or her symptoms.
As we have mentioned already, pain mortality is to relieve pain followed by arthroscopic surgery which may help in correcting the knee imbalances to its normal and we use techniques which avoid the knee replacement surgeries like cartilage preservation, or cartilage regeneration surgery and wherever it is imperative to do knee joint replacement surgery, there are modern techniques of optimizing the results in the form of minimally invasive surgical techniques in knee replacement which we have been using regularly.
There are techniques like computer navigation surgery to achieve more precision, there are patient specific implants, there is patient’s specific equipment which also increase the precision and accuracy in order to optimize the result.

Rotator Cuff Tears: What Are My Options?

Rotator cuff is the name given to four muscles and their tendons, which keep your arms in your shoulder sockets, allowing you to lift your arms and do day-to-day stuff like combing your hair, cutting vegetables etc.

The shoulder joint is a very important joint of the body and this is why any tear that happens in the rotator cuff can be devastating.

Causes

Tears can happen in rotator cuff due to a number of factors like injury and degeneration due to age. Tears that happen due to an injury to the shoulder are called acute. Degenerative tears are tears that happen due to degeneration of the tendon over time.

Common causes of degeneration are lack of proper blood circulation, which impedes the rotator cuff’s ability to repair itself. Repetitive stress is another cause where we stress the tendon by repeating the same shoulder motions again and again while playing games like tennis, cricket etc. and doing jobs that require us to extend our arms upwards a lot. Lifting heavy objects can also lead to tears.

Symptoms

Chronic shoulder and arm pain are good reasons for you to see your doctor. If you suffer from pain in your shoulder at rest, while sleeping on the affected shoulder or pain while lifting and lowering your arm, then there is no time to wait.

The doctor will do a physical examination by moving your arm in different directions. He may follow it up with X-rays and MRI to check how big the tear is in your rotator cuff.

Treatment

Once confirmed that you indeed have a rotator cuff tear, your doctor will start the treatment. The goal of any treatment is to reduce pain and make your shoulder joint work properly again.

Nonsurgical Treatments: 

Nonsurgical treatments include rest, using of a sling to protect the shoulder and modifying your daily activities. This means you may not be able to do activities that cause shoulder pain. Your doctor can also prescribe pain and anti-inflammatory medicines. Strengthening and physical therapy bring up the rear of this type of treatment.

What you must remember is that nonsurgical treatment relieves pain and improves function in the shoulder, but does not improve shoulder strength. The size of your tear may also increase over time.

Surgery: 

Surgery, which attaches the rotator cuff to your bone, is done if you suffer from continuous pain and symptoms over a year or so. If you have a large tear, conservative treatments will not work and surgery will have to be resorted to. 

If you have been suffering from chronic shoulder or arm pain, waiting and watching is definitely not an option. You must consult an orthopedic surgeon at the earliest. Remember, if treatment is not started soon enough, your rotator cuff tear will get larger and will require only surgery.

How to Prevent Hipbone Fracture in Elders

Any mild fracture in the hip can give a tough time to people. A hipbone fracture or broken hip is mostly common in the elderly people, especially in women. Mostly, the reason is underlying thinning of bones. In addition, in any hospital hip fracture is considered to be the most common bone treatment in the orthopaedic department. 
A hipbone fracture can be intracapsular (within the joint capsule), or it can be extracapsular (outside the joint capsule). 

What causes the hipbone fracture?

As discussed before, the leading cause of hipbone fracture is thinning of bones. So if you develop osteoporosis, you end up getting a fracture in your hip, mostly when you fall. Osteoporosis generally means your bone density has decreased, and your bones have become honeycombed. This makes the bone more fragile. 

Generally, if you have a fracture in your hip, then you will feel pain around the injured portion, you will face difficulty in walking or moving. These are the most common symptoms that can be felt during a broken hip. Mostly the intensity of the fracture is determined clinically via different tests like X-ray, MRI, CT scan etc. The treatment may vary and depends upon the intensity of the injury. Now if we talk about the treatment of hipbone fracture, it's mostly surgery

Well, it will be smart enough to take prevention regarding the hipbone fracture. There are various kinds of guidelines, which can help elders to prevent a hipbone fracture. 

How to prevent hipbone fracture?

  1. First and foremost, it is advised to adopt a healthy lifestyle in young age in order to build a healthy bone, which decreases the chance of bone fractures in older age. 
  2. Always remember to maintain a healthy bone. 
  3. Calcium and vitamin D both are very important for healthy bones. Health experts advise, when you attain the age of 50 or above, you should daily intake 1,200 milligrams of calcium per day and 600 international units of vitamin D per day. 
  4. Exercise can help you to maintain peak bone density for longer periods, and it also increases the strength of your body and bones. Balance training also plays a pivotal role since it reduces the chance of falling. 
  5. Avoid consuming of tobacco and alcohols as it can reduce bone density. 
  6. Get your eyes checked every year so that you do not face any problem while seeing. This is because poor view increases the chance of falling.
  7. Keep an eye on every medicine you are taking. As dizziness or weakness can be side effects of many medicines, these can lead you to fall often. So it is advised to discuss your medication with doctors. 
  8. As you get older, the chance of high or low blood pressure doubles up. So you should be extra careful while standing up.

If you don't feel steady enough, then to avoid falling, it is safe to use a walker or walking stick in the old age.

I used to go for jogging everyday for 30 mins. Then after 10 days I found a problem in my right knee so I discontinued jogging and took pain relief medicine and now not continuing jogging now, pls suggest what to do, I want to keep if my bones are not strong and if it is then which medicine to take.

It seems you have improved but diagnosis is still elusive ,it will be better for me to examine you to diagnose and treat you properly. Till than you can use cycling as fitness regimen rather than jogging.
