Good morning sir, I am suffering with continuous hair fall. Can you please suggest me?

Is your hair loss accompanied with greying of hair and have they become very thin? your hair needs a very high protein well-rounded diet. Hair and nails are the last organs to get nutrition in the body, hence only if all the organs get sufficient nutrition and proteins, only then the remaining nutrition would be used for hairs. A strand of hair is composed of mostly protein, which means your hair needs protein to grow. Hence a therapeutic diet prescribed by a registered dietitian can help a lot in hair growth. Besides diet excessive stress, current/past dandruff and some medication are conclusively known to retard hair growth and promote hair loss. - cut down on the use of hair spray, hair gels, and styling creams that contain harsh chemical ingredients, which can dry out the scalp and cause the formation of flakes. Also harsh treatments such as perms, colours, relaxers etc take a toll on the health of your hair. - avoid changing your shampoo and conditioner constantly. Stick to one good mild cleansing shampoo for six months before changing. - excessive exposure to the sun, pollution, rain water and dust without proper protection makes the hair dry, brittle and limp. Use a live in conditioner during the rains to protect it from the humidity and wash and oil it regularly to maintain a healthy scalp. I may also prescribe medications like biotin or minox. That are proven to help in hairgrowth and repair I being a both a registered dietitian (rd) and doctor have been successfully helping patients with their problems through a holistic approach using customized therapeutic diet and medications. Guidelines for vary from time to time, but generally accepted classification of hypertension ( JNC7)is

Average of two or more properly measured readings at each of two or more visits after an initial screen:

Normal                  < 120 mm Hg SBP             AND   DBP < 90 mm Hg

Prehypertension    120 -139 mm Hg SBP     OR      DBP 80 to 89 mm Hg

Stage 1 HTN            140 -159 mm Hg SBP     OR      DBP 90 to 99 mm Hg

Stage 2 HTN            >/= 160 mm of Hg SBP   OR     DBP >/= 100 mm Hg.

Prevalence of prehypertension among adults in United States is approximately 37 percent.

Study done by Yadhav S et al showed prevalence of prehypetension was 32.3 percent in India with highest  36% among 30-39 yrs age group, indicates that awareness is necessary for regular blood pressure check up.

People diagnosed with hypertension represents “tip  of an iceberg”.

Many patients get to know about their raised blood pressures at the time of diagnosis, heart attack, stroke or kidney disease( end organ failures) which would have been preventable  if treated at an early stage.

Why hypertension should be diagnosed and treated ?

Hypertension currently causes  2/3rd s of all strokes and half of all cases of ischemic heart disease.

Reduction in high blood pressure leads to large reduction in stroke, heart failure, renal failure, aortic dissection, coronary events and death.

Prehypertension: It is an entity where SBP >120 -139 mm Hg or DBP 80-89 mm Hg.

Multiple epidemiological studies demonstrated increased cardiovascular risk in patients with prehypertension.People with prehypertension have increased risk of progression to sustained hypertension, the prevalence of hypertension increases from approximately 10 percent at age of 30 yrs to as high as 90 percent after age of 65 yrs.

Framinham hypertension risk prediction score, may help identification of prehypertensive patients who are at gretest risk for pregression to hypertension. Risk calculator includes variables like age, sex, family history of hypertension, body mass index and smoking.  Most important predictors of these were higher baseline blood pressure and older age.

Screening for hypertension: optimal interval for screening is not known.

2007 USPSTF( United States Preventive Services Task Force) guidelines recommend

Screening every    two years for persons with SBP <120 mm Hg and DBP < 80 mm Hg     

                                 Yearly for persons with SBP 120-139 mm Hg or a DBP 80 -89 mm Hg

How to manage Prehypertension:

TROPHY stydy( TRial Of preventing Hypertension)  results showed that there is no role for pharmacotherapy in prehypertension except in special conditions like diabetes, chronic kidney disease etc.

Treatment:As per JNC 7 reccomendations patients with prehypertension who do not have diabetes, chronic kidney disease , end organ damage , or clinical evidence of cardiovascular disease are generally treated with non pharmacological therapies.

Major non pharmacological therapies that aid to decrease blood pressure are: 

Weight reduction:  Maintain normal body weight ( BMI 18.5 to 24.9 kg/m2)  this can reduce BP by 5 to 20 mm hg per 10kg weight loss.

Adopt DASH eating plan : (DASH- Dietary Approach To Stop Hypertension) consists of fruits, vegetables, legumes, low-fat dairy products with reduced saturated and total fat.This can reduce BP by 8 to 14 mm Hg.

Dietary sodium Restriction: Reduce dietary sodium intake to no more than 100meq/day(2.4 gm sodium or 6 gm of sodium chloride). This can reduce BP by 2 to 8 mm Hg.

Physical activity: Engage in a regular aerobic physical activity such as brisk walking (at least 30 min per day, most days of the week). This can reduce BP by 4 to 9 mm Hg.

Moderation of alcohol: Limit consumption of alcohol to no more than 2 drinks per day in most men and no more than 1 drink in women and lighter weight persons. This can reduce BP by 2 to 4 mm Hg.

Obesity or excessive weight is a common problem for both men and women irrespective of their age.   1. Antimonium Crudum: This medicine is advised to children or young people suffering from excessive and inappropriate weight.   2. Calcarea carbonica: This medicine is recommended if fat gets accumulated in the abdominal region. It helps to lose fat from the abdomen region. It is also given to a person showing the symptom of excessive perspiration.

Sir I start running from a week ago now I feel pain in my knees and my legs does any problem with me?

Hi, what seems, you may have'runner knee' the stress of running can cause irritation where the kneecap (patella) rests on the thighbone. While biomechanical issues like (flat feet, knock knee, high arch foot etc) may be to blame, or ot can be poorly conditioned quadriceps and tight hamstrings i. E. If you start running without warm up and stretching exercises. As weak quads are not able to support the patella, leading it to track out of alignment, and inflexible hamstrings can put pressure on the knee. If you want to treat and avoid another bout withp runner's knee, add good strengthening and stretching to your routine. Also check for ny biomechanical problem with good ortho or physio. Donot worry you will be fine. Run on softer surfaces, keep mileage increases less than 10 percent per week. Wear proper shoes according to your foot type. Start strenghtening exercises also.

Have pain in my back so much during mensuration period what to do to reduce it? Is it right to take medicines during this?

Hello, Is there any history of injury or hurt to the area? Any underlying medical condition? Medication: Meantime, take homoeopathic medicines - Arnica 200 and Rhus Tox. 200 - take both of them thrice daily for 1 week. Also, apply Schwabe's Topi-Arnica and Topi MP Gel on the affected area in circular motions gently thrice a day. Get back to me thereafter.

She should have a normal healthy diet with lots of fresh vegetables and fruits and fiber so that she doesn't get constipated and apart from diet she should follow an active daily routine as far as possible without getting tired.

This is very common for babies to pass green stools. If baby is passing urine normally and gaining weight do not do anything.

I had gone for TSH test where result was 3.82. Below the results it was mentioned -- Pregnancy 1st times: 0.3-4.5 2nd times: 0.5-4.6 3rd times: 0.8-5.2 What does this pregnancy times indicate, plzz help one of my frnd saw this report and is asking me whether I was pregnant in past. It is trimester. Pregnancy of 9 months is divided into 3 parts of three months each. They are called trimester. Values of tsh in pregnancy are different so they are mentioned separately. So tsh report has nothing to do with pregnancy confirmation.

I am 36 years old my sugar levels are 160-190 before food and 240 after food. Bp is normal, I am 140kgs, I do smoke and drink, so I am not taking any medicine but want to control my sugar levels by diet. Pleases suggest.

Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me with all details like your daily eating pattern and medicines you take. I assure you a proper guidance.
