I am 34 year Old male with a secondary infertility case i.e. I already have 6 years old daughter which we got via natural conception. I don't drink, don't smoke at all and IT engineer by profession with no exposure of any heat whatsoever. It's been over 10 months since we are trying for our second baby and we aren't getting result. We had a rigorous tests done for my wife and she is all fine as she was during 1st conception hence with repeated semen analysis it's discovered that I have Azoospermia God knows how come it got developed in me :(. I have absolute no surgery or something hitting my testis which could ever lead to this. Doctor has advised me to go for Testicular Biopsy some have advised IUI / IVF. I want to know do I really need IVF as I strongly believe that I must be having sperms but it's not just coming through ejaculation. Please advise.

Azoospermia means your semen doesnot contain sperms. It could be due to either lack of production or due to obstruction of the tube which delivers the sperms at sex. In patients of azoospermia, is the volume of the ejaculate (semen) is good and fructose test is positive, it could mean there is no production. If the fructose if absent it suggests ejaculatory duct obstruction. Usually hormones tests like FSH, LH, Inhibin B levels are advised and on their basis, some hormonal treatment can be offered. Normal volume, PH and fructose being present and presence of GPC (glyceryl phospho choline) means there is NO obstruction, But Normal Volume, PH and fructose positive but NO GPC means it is vas deference epididymis level obstruction. Low volume, PH and fructose negative and NO GPC means it is ejaculatory duct obstruction So het the hormone tests done, GPC levels tested, and meet a urologist or andrologist who can guide your treatment. In patients who have sperm production but tubal block, the sperms can be aspirated by TESA or MESA but these need to be sued for IVF only. If the hormone levels are low, and there is NO obstruction, there are injections which can help increase the production. In patient who have never had a child or are not affording the expensive testicular biopsy, TESA, MESA,IVF, gynecologists also can suggest a donor semen insemination as a attempt for those who want a pregnancy desperately. So decide for yourself and then meet an andrologist or urologist.

The cause of irregular periods are Imbalance in the Eostrogen & progesterone hormones. The cause of imbalance are- * stress * Thyroid disorder * Diseases of uterus * PCOD * Ovarian tumours For making regular your period these are some instructions given below: • Take proper diet full of nutrition . Eat green leafy vegetables and seasonal fruits and a glass of milk daily. Eat a fruit daily . • Do Not take stress because stress causes imbalance in hormones of female and make your menstrual cycle disturb and make your ovulation disturb .Do some meditation for maintain hormonal balance. • Do jogging and running exercise daily .If you are obese lose some weight this is also a cause of irregular periods. • Do skipping regularly for 2 to 3 minutes daily. • you can take these medicines for regular periods. it will take some effort for you to flatten your tummy. I am outlining a 21 day plan for you. If you follow this plan sincerely then you will see a noticeable difference in just 21 days. There are 5 things that you need to do: 1. Eliminate all refined sugars from your diet. This means no sugar in your tea/coffee/milk, no consumption of sugary beverages like coke, pepsi and total elimination of all snacks like biscuits, cakes, sweet dishes. I know it is difficult but remember that you just have to adhere to this for 21 days. If you wish to snack you can have unsalted peanuts, dry fruits, seeds or yogurt. 2. When you get up in the morning, put your pillow aside and lie with your back straight. Put both your legs together and lift them so your legs are at a 30 degree angle with the bed. Maintain this pose of 5 minutes. Keep breathing while your legs are in the air. 3. Eliminate dinner altogether. In place of dinner have a bowl of papaya. You can have a small bowl of yogurt with black salt if you are not able to control your urge. 4. Keep hydrated through out the day. You need to drink at least 10 glasses of water every day. Tea/coffee are not substitutes of water. Minimize consumption of milk. Add raw vegetables to each meal. 5. One 500mg tablet of Calcium D-Glucarate and one tablet of B-Complex every day. If you miss even one of the steps mentioned above during your 21 day regime then you have to start all over again from Day 1. Please bear in mind that it takes 21 consecutive days of performing all the above mentioned steps for the stomach muscles to start toning up.

Hello Doctor, It was around 3 years back, when I was 18 years old, my bra size was 80cms.(n 80 has remained cost. For three later years) Later, in the next one and a half year, the size got increased to 85 Cms. Which was still ok. But the size 85 cms, lasted only for few months (5 to 6), and now it's 90 cms. Not everyone with irregular periods will have PCOD. Also you need a scan and blood test to diagnose it. When you get married you can get evaluated if your pregnancy gets delayed. As of now exercise regularly and keep your weight under control? That is the only thing you need to do now. For back pain learn simple yoga and start doing.

But the chance of getting sexual tract infections is more like HIV, Hep B, C etc. Because the anal mucosal layer is so thin that it will get injured so easily and also because of its rich blood supply. Sexual intercoure in the first three months (till 13 weeks) can lead to miscarriage and in the last two months can lead to pre term labour. Best is to avoid it in this time zone. In the rest of time too, you need to be very gentle in insertion to avoid excessive pressue on the mouth of the uterus.

Hi i am taking Novelon 21 day pill from june. Missed second last doubled with the last, break of 6 days from 2-7 july. Menses from 5-7july. New pack from 8th july. Protected intercourse but condom used thrice on 9th july (first timw intercourse) Next fully protected on 14th july (last intercourse) Missed 21st july pill doubled 22nd july night. Light bleeding on 22nd july morning (breakthrough bleed or implantation bleed?) Pill ended on 28th july menses from 1-6 aug. Negative hpt at 4,5,6,7,8,11,12 weeks all negative hpt done with first morning urine with different brands. No pills or intercourse after 1-6aug. Periods due on 1st sept as it dint came did a usg and hcg beta on 8th sept (10 weeks from first and 9 weeks from last intercourse) Hcg was 0.11 and usg shows pcod (no mention of pregnancy) Dr. Prescribed deviry 2 tab for 5 days for withdrawal bleed. But after taking just a single dose bleeding started from 11-16th sept. It was heavy. On 21st sept morning when I woke up I can feel my pelvic bone in the front (can it be due to exercises like crunches leg rise etc with weight loss? I was 72 in july 70 in aug and now I am almost 68 kgs) and after continuous touching I made it hurt. Scared went for usg on 21st sept (12 weeks ftom first and 11 weeks from lasr intercourse) Usg only showed bulky ovaries with pcod. I asked him if my uterus was normal he said its absolutely normal. Usg was transabdominal. The hardness yet not gone. Moreover m fln gassy all the time and I cn feel sensation under my rib and back and m passing lots of gas. M worried too. Finally ve taken an hpt with first morning urine at almost 12 weeks 6 days. And its negative. Can their still be risk of pregnancy? My menses was due on 11th october and it is now delayed by 5 days. I did not have any intercourse after 14th july. M not experiencing any pms symptoms. Moreover egg white cervical mucus is seen at various point of the cycle. Will I wait for my periods to come or will I again take tab deviry? Is their any risk of pregnancy? Do I need to redo hpt or BHCG test? M I 100% safe?

No risk of pregnancy. U can either wait for periods to come or take withdrawal with tab devilry. Both options is fine. After periods come you will have to get complete check done by gynaecologist and take treatment for Pcos.
