Hi Doctor, I had fasting blood glucose of 140 on 21st July 2017 for which I was advised to skip rice & non vegetarian at dinner for 10 days. Again I checked on 1st Aug which read as 113, along with 2 hrs post prandial at 134 & HBA1C 5.8%. Please advise if I am diabetic or not? Any actions to be taken. My doctor has advised to exercise along with diet.

Mr. lybrate-user, Thanks for the query. The blood glucose levels mentioned suggest you have a pre-diabetic state. Diabetes is diagnosed if FBG is equal to or more than 126 mg & PP 200 mg and above & HbA1c% 6.5% and above. So your present situation is pre-diabetic state. At this stage, if one follows a modified lifestyle, then diabetes can be prevented. For that you need to maintain near normal weight (65 kgs), exercise regularly for over 1 hour and take a proper balanced diet and avoid sweets, deep fried foods, junk foods, pastries, bakery items and foods made out of maida. Also avoid sweetened aerated drinks (colas), alcohol and smoking. Red meat also is not good. Thanks.

Erectile dysfunction (ED) is a condition in which man has trouble in keeping or getting an erection. It is a common male sexual dysfunction. It becomes more common as you get older but it is not a part of aging. MECHANISM OF ERECTION An erection starts in the brain. Physical and mental stimulation is required for an erection to begin. This stimulation causes nerves in the brain to send chemical messages to nerves which are present in the penis which results in the free flow of blood into the penis. This leads to high pressure in the penis which in turn traps the blood within both corpora cavernosa and then the penis expand. This causes the penis to sustain an erection. When the inflow of blood is stopped, then it opens outflow channels and then penis becomes soft. Then an erection is said to be reversed. CAUSES OF ERECTILE DYSFUNCTION Erection is a complex process which involves hormones, brain, emotions, muscles, nerves and blood vessels. Problem with any one of these can lead to erectile dysfunction. Sometimes stress and mental health can also lead to erectile dysfunction. Combination of Physical and Psychological issues can lead to erectile dysfunction. PHYSICAL CAUSES OF ED • Heart disease, high blood pressure, High cholesterol. • Parkinson's disease, obesity, alcoholism, atherosclerosis. • Use of tobacco, surgeries that affect the pelvic area or spinal cord. • Treatments for prostate cancer. PSYCHOLOGICAL ISSUES OF ED: Depression, anxiety, stress and other mental health conditions are some of the psychological issues which causes erectile dysfunction. SYMPTOMS OF ERECTILE DYSFUNCTION • Trouble in getting erection • Difficulty in keeping erection • Reduced sexual desire HOW CAN ERECTILE DYSFUNCTION BE PREVENTED? By decreasing the chances of developing several diseases like diabetes, heart disease and hypertension, chances of developing erectile dysfunction can be decreased to some extent. By eating healthy diet, erectile dysfunction can be managed. By doing Exercise daily and by avoiding smoking, risk for developing erectile dysfunction can be decreased. Erectile dysfunction (ED) or impotence is condition characterized by the inability to develop or maintain an erection of the penis during sex. A penile erection is the effect of blood entering and being retained in sponge-like bodies within the penis. The process is most often initiated as a result of sexual arousal when signals are transmitted from the brain to nerves in the penis. The most important organic causes are heart disease and diabetes, neurological problems (for example, trauma from prostatectomy surgery), hormonal insufficiencies and side effects to certain drugs Psychological impotence is where erection or penetration fails due to thoughts or feelings (psychological reasons) rather than physical impossibility; this is somewhat less frequent but can often be helped. Notably in psychological impotence, there is a strong response to placebo therapy. Erectile dysfunction can have severe psychological consequences as it can be tied to relationship difficulties and masculine self-image generally. Most men have occasionally experienced some difficulty with their penis becoming hard or staying firm, but this is not erectile dysfunction, which is a concern only if satisfactory sexual performance has been impossible on a persistent number of occasions for some time. Broadly, two forms of erectile dysfunction can affect men's sex lives - the main cause is either medical or psychosocial. Here, psychosocial means psychological effects - including the effects of social relationships - on sexual performance. Of course, medical conditions can also affect psychological wellbeing, and while the main cause of sexual impotence will be medical or psychological, there can be overlap between the two. Erectile dysfunction used to be known more widely as 'impotence' before the causes became better understood and successful treatments came into use. Impotence is a term that is still in use, however. Signs and symptoms Erectile dysfunction is characterized by the regular or repeated inability to obtain or maintain an erection. It is analysed in several ways: • Obtaining full erections at some times, such as night time erecting when asleep (when the mind and psychological issues, if any, are less present), tends to suggest that the physical structures are functionally working. Other factors leading to erectile dysfunction are diabetsic neuropathy. • Causes • Drugs (anti-depressants (SSRIs[6]) and nicotine are most common) • Neurogenic disorders[7] • Cavernosal disorders (Peyronie's disease[8]) • Psychological causes: performance anxiety, stress, and mental disorders[9] • Surgery[10] • Aging. It is four times more common in men aged in their 60s than those in their 40s.[11] • Kidney failure • Diseases such as diabetes mellitus and multiple sclerosis (MS). While these two causes have not been proven they are likely suspects as they cause issues with both the blood flow and nervous systems. • Lifestyle: smoking is a key cause of erectile dysfunction.[12][13] Smoking causes impotence because it promotes arterial narrowing.[14] Surgical intervention for a number of conditions may remove anatomical structures necessary to erection, damage nerves, or impair blood supply. Erectile dysfunction is a common complication of treatments for prostate cancer, including prostatectomy and destruction of the prostate by external beam radiation, although the prostate gland itself is not necessary to achieve erection. ED can also be associated with bicycling due to both neurological and vascular problems due to compression.[15] The increase risk appears to be about 1.7-fold.[16] Fast facts on erectile dysfunction Here are some key points about erectile dysfunction. More detail and supporting information can be given on personal or private chat .Erectile dysfunction is a persistent problem getting an erection sufficient to have sex.  Causes are usually medical but can also be psychological.  Organic causes are usually the result of an underlying medical condition affecting the blood vessels or nerves supplying the penis.  Numerous prescription drugs, recreational drugs, alcohol and smoking can all be causes, too.  Diagnosis with the help of a doctor will differentiate the likely cause by including a number of questions, blood tests for conditions such as diabetes and heart disease, and physical examination.  First-line treatment of erectile dysfunction is with drugs called PDE-5 inhibitors, including the well-known pill sildenafil (Viagra).  Mechanical devices are also available to help achieve an erection, and occasional cases that do not respond to medical treatment may be candidates for surgery, to fit a penile implant. What causes erectile dysfunction? To understand the causes of erectile dysfunction, it helps to understand how an erection happens physically. Relaxation of muscle (in parts of the penis and the walls of the arteries supplying it) allows blood to rush into the cylinder-shaped spongy tissue of the so-called erection chambers, and valves prevent the venous loss of this pressure. The resulting hardness persists until after stimulation has stopped or an ejaculation has happened.4 This blood flow process is led by nervous and hormonal triggers activated when the brain is stimulated by touch or other stimuli, and these brain signals can be suppressed by anxiety.4 A healthy lifestyle can prevent erectile dysfunction since the problem can be caused by preventable conditions such as diabetes and vascular disease. Effects against any of the following normal physiological contributions needed for an erect penis could affect normal erectile function:1  Blood flow  Nervous supply  Hormones. Vascular causes that affect blood flow, and neurological causes that affect nerve signals are the major medical causes of erectile dysfunction, often due to the underlying disease processes of atherosclerosis and diabetes.5 Smoking and diabetes often cause atherosclerosis, and this narrowing or 'clogging' of arteries in the penis is the most common vascular cause of erectile dysfunction.5 Diabetes can also lead to neuropathy - damage to the nerves. Diabetic neuropathy is a common neurological cause of erectile dysfunction, as is nerve damage resulting from various types of surgery around the pelvis. Less common neurological causes are stroke, a type of epilepsy (partial complex seizures), multiple sclerosis, non-diabetic neuropathy, and spinal cord injury.5 Reducing well-established risk factors for diabetes and atherosclerosis - including diet, physical activity and other lifestyle factors such as smoking - could also prevent erectile dysfunction.

You need to have diet plan based on your BMI & activities. Whatever we eat is metabolised by the body for the energy needed to do daily work and the excess is stored as fat. Abdomen is the first place to gain weight and last to disappear. In brief, Weight reduction can be achieved only by reducing the intake of food 1/3 of what you take now. And by appropriate exercise plans for each part like belly, thighs, legs / chest etc. to burn off fat. Some tips 1) Drink three or four cups of water whenever you feel hungry and just before eating food 2) Take boiled or raw vegetable like cucumber, carrot etc. as half of your diet. Then take 1/2 of your total diet as sprouted pulses / proteinaceous food like roti, cooked pulses and remaining with rice or other carbohydrates. Oil or ghee etc. should be just bare minimum. And do aerobic exercises, running, jogging or at least walking briskly 3-5 KMs and swimming if possible, and in a gym if feasible. The most important is to burn off the energy intake by exercises. There is no short cut or medicines without exercise & dieting to take just as mush food as needed for your activities. The food we eat is utilised for our energy needs and the remaining will be stored as fat which leads to obesity. It is better to do some tests. A DIET CHART Timing Foods To Eat Calories Early Morning Lukewarm Water with Lemon 1 glass 0 Tea without Sugar + 2 Biscuits 90 Breakfast 2 Rotis + 1/2 cup Paneer Curry 330 OR Brown Bread Upma 1 plate + Milk 1 cup 300 Mid-Morning 1 Banana/1/2 cup Melon/20 Grapes 50 Lunch Brown Rice 1 cup (195 gm) + Mixed Vegetables 1/2 cup + Salad 1 bowl + Raita 1 small bowl 345 Evening Butter Milk 1 cup 35 Dinner 2 Rotis + Vegetable Soup 1 bowl + Salad 1 bowl 370 Total 1220/1190 Cal To lose weight, you must burn more calories than you consume. One pound equals 3,500 calories. If you cut 500 a day from your diet, you'll lose a pound a week. People who lose weight slowly, about 1 to 2 pounds per week, are more successful at keeping the weight off. You'll also burn additional calories if you increase your physical activity. You weight-loss plan should do all of the following: • Emphasize fruits, vegetables, whole grains, and fat-free or low-fat dairy products. • Include lean meats, poultry, fish, beans, eggs, and nuts. • Contain foods low in saturated fats, avoid trans fats, and limit cholesterol and salt (sodium). • Avoid refined sugars and processed foods. Some tips to help you lose weight: • Keep a food diary. Writing down everything you eat is critical! Noting how you feel when you eat will help you identify emotional triggers that may cause you to overeat. Emotional eaters tend to overeat. • Eat when you're truly hungry rather than when you're tired, anxious or stressed. • Eat slowly. If you eat too fast, you eat more than you need to satisfy your hunger. Your brain needs time to catch up with your stomach. • Plan your meals ahead of time so you make healthy choices. When dining out, check the menu online and decide what you will order before you sit down. Ask for dressings on the side. Opt for foods that are baked, broiled or steamed versus fried or swimming in creamy sauces. Avoid chips and bread baskets—they add unnecessary calories. • Stay hydrated. People often mistake thirst for hunger. • Eat breakfast. If you skip this meal, you'll be starving by lunchtime. You'll have more difficulty making healthy choices throughout the day. • Find resources to help you keep track of your food choices and nutritional values, and that can offer additional support when you need it. Try CalorieKing.com and MyFitnessPal.com, which offer a searchable database of foods with nutritional values. Exercise for Belly Fat Not only females but also males desire to have flat belly. Read on and find about the best exercises for losing belly fat. Fight your belly fat with the right kind of exercise and the right diet. Belly fat can be effectively reduced with abdominal exercises. They help you to tone the abdominal area apart from enhancing the flexibility of the area. The most important point is that they resist the accumulation of fat around the belly region. The only thing that you must keep in mind is that you must regularly perform these exercises and be patient as nothing happens overnight. Exercises for Losing Belly Fat Some very simple but effective exercises to Lose Belly Fat Stretch 1 – Lie down on a floor mat. Position your legs vertically up and stay in this position for 5 to 15 seconds. This can be repeated thrice. Stretch 2 – Lie down on a floor mat. Stretch your legs straight. 1. Masturbation is a natural habit in both the genders during their life time. Continuous masturbation in men will certainly leads to unwanted curvature of penis and in women clitoris looses its sensitivity due to rough use of their fingers on clitoris. 2. Later they may not get normal pleasure or orgasm in intercourse (thrust of penis) after their marriage. 3. Masturbation once a week is fine but do not make it your habit.
