It is better to get urine culture and sensitivity test and take antibiotics accordingly to prevent recurrent infections. Also please check your blood sugar level.

What is fibroadenoma? Is this any serious health issue? What should be done to git rid of this?

General Physician, Delhi
A fibroadenoma is a non-cancerous tumor in the breast that is commonly found in women under the age of 30. The exact cause of fibroadenomas is not known. Hormones such as estrogen may play a part in the growth and development of the tumors. Taking oral contraceptives before the age of 20 has been associated with a higher risk of developing fibroadenomas.

Dependent Personality Disorder

Dependent personality disorder (dpd) is one of a cluster of disorders defined by symptoms of anxiety and fear. The specific, identifying symptoms include:

  • being emotionally dependent on others; feeling they can’t take care of themselves
  • investing a lot of time and effort in trying to please significant people
  • displaying clingy, passive and needy behavior
  • avoiding disagreements for fear of losing approval and support
  • experiencing separation anxiety and intense fear of abandonment
  • finding it hard to be alone
  • putting the needs of others before their own
  • tolerating mistreatment and abuse for fear of disapproval and abandonment
  • being crushed, and feeling helpless, when relationships end – and forming new relationships as soon as possible
  • being unable to make even the simplest decision without the input and reassurance of others
  • rarely taking the initiative
  • avoiding personal responsibility
  • avoiding responsible jobs and careers that require independent, autonomous functioning
  • being over-sensitivity to criticism
  • feeling negative and pessimistic; expecting to disappoint and fail
  • having low self esteem and lacking confidence, including a belief that they are unable to care for themselves.
  • The cause of disorder is still unclear, and probably includes both a genetic and environmental component. Some researchers have speculated that it could be linked to an authoritarian or overprotective parenting style – which acts as a trigger for a genetic predisposition.

Treatment is usually initially sought for some other problem or concern – such as feeling overwhelmed – so that they can’t cope with life. Also, sufferers will often have a mood disorder so they seek help for depression or anxiety at first.

The normal treatment for this particular disorder is counselling or psychotherapy. However, the emphasis is short term therapy so the person doesn’t form a dependency – and then look to the counsellor to take care of them. Prognosis with support is generally good.

How to prevent lifestyle diseases?

General Physician, Nashik
How to prevent lifestyle diseases?
How to prevent lifestyle diseases?

In the olden days, we had food scarcity and too much physical work. So obesity and lifestyle diseases were less among people. In those times people used to eat heavily when more food was available and eat less when no food was available.

Then came the agricultural revolution era and a smart public distribution system. Food become abundant. It became impossible to change our centuries' old habit and we started eating more.

Then came industrial, it developments and opening up of markets. It created the following things:

Less physical work. Too much mental strain. Odd working hours. Availability of cheap tasty junk foods.

All these started leading to lifestyle diseases like obesity, hypertension and diabetes.

Then came the medical era (the current period), when the incidences of the above said diseases started growing out of proportions. Nowadays we have started realizing that

We are over eating. We are eating junk calories, food with high fat and salt content. We are leading a sedentary lifestyle.

We have started seeing our parents, relatives, friends getting these diseases, and we have come to know that there is no escape, since they too lead a similar lifestyle. This is the awareness era. Now availability of resources like internet and health magazines are changing people's mind to lead a healthy and fit life.

How to lead a healthy lifestyle and avoid lifestyle diseases?

Stop smoking, reduce alcohol consumption to once a week (two drinks only, it is better if avoid alcohol. Control your existing illnesses like diabetes and hypertension by dedicated dieting and regular consultation with your physician.
Physical activity: 40 minutes a day, minimum 5 days a week. Stop junk foods completely, reduce sugar, salt and oil consumption.
Sleep well (5-8 hours). Everybody has a different biological clock. A refreshing night's sleep varies from person to person. For some 5 hours is enough, for some they need 8 hours. Find out which person you are and start sleeping for the correct time.

Menses can be missed either because of hormonal changes or even stress or illness or even if you have put on some weight in recent times then also menses can be missed. All these problems are temporary and menses will come on its own. You can wait for another 15 days before you can take tablet deviry to get menses. In the meantime, please do urine pregnancy test if her last menses were just spotting.

Spine Tuberculosis - Catch It Early To Avoid Serious Disability!

MBBS, MS - Orthopaedics, DNB - Orthopedics

Spine Tuberculosis - Catch It Early To Avoid Serious Disability!

Tuberculosis is widely known as a disease that affects the lungs. Tuberculosis is an infection caused by a bacteria called Mycobacterium tuberculosis. Although most commonly the lungs and bowel are affected, it can affect any organ in the body including the bones and joints .

How and who:

Tubercular bacteria usually enter the body through the airway into lungs or through the gut and spread out to different areas of the body through the blood stream, spine being the most common area. They can stay dormant in the body for many years and start multiplying when the immunity goes down due to any reason. So those with reduced immunity like young kids, old age group, diabetics, those on steroid medication or other immune-suppressive medication are at a high risk of developing tuberculosis of various organs. Patients affected with HIV are also at high risk of developing tubercular infection. But it is not uncommon to see a healthy young person affected with tuberculosis of spine.

Disease manifestations:

Spine is one of the very common structures affected by tuberculosis, leading to significant disability and high risk of paralysis if undetected in the early stages. As the bones in the spine get destroyed by the disease, the spine bends abnormally at the affected level and a deformity develops. There is pus formation at the affected area which can cause compression on the spinal cord and nerves in the spinal canal and lead to paralysis of limbs and loss of bowel and bladder control. Tuberculosis of spine can affect all age groups from infancy to old age.

Spine tuberculosis initially causes pain in the back/neck, which gets worse on movements and while changing posture. Night pains that wake the patient up from sleep is another characteristic feature. Fever, weight loss and loss of appetite are usually associated. Weakness of limb muscles can lead to unstable gait and tendency to fall and the patient usually seeks a support to hold on to while walking.

Diagnosis and treatment:

With MRI being available widely, spinal tuberculosis is being detected at early stages now as compared to X-rays. Most of them can be cured well by anti-tubercular medication (ATT). The medication needs to be taken for a long duration (minimum of 6 months - upto 18 months). Drug resistance is a serious problem needing second line medicines and is more difficult to treat. There are special tests like Gene Xpert available now to detect multiple drug resistant tuberculosis.

In some patients with deformity in spine or those with spinal cord compression causing paralysis, surgery to stabilise the spine and decompress the spinal cord/nerves is required. Surgical treatment in right time can prevent spinal deformity/paralysis and help in faster and better recovery of muscle power in those who have developed paralysis.

So, any persistent back pain needs an evaluation by a Spine physician.
