My haemoglobin, mcv, mch, haematocrit levels are low and rdw level is high. Do I have iron deficiency anemia. Also my eosinophils level is 1550/cumm. Do I also have hes.

MBBS, MD - Internal Medicine
General Physician, Delhi
Dear , thanks for the detailed description. We would love to have the exact figures of your hb, mcv, mch, mchc, and your rdw. However from the data given we can safely assume that you have microscopic and hypochlorite anemia. Which has 3 principle causes 1. Iron deficiency. 2. Thalassemia 3. Sideronblastic anemia. Please do a reticulocyte count and a peripheral blood smear also. Hes is a rare diagnosis. Most probably your high absolute eosinophil counts are due to intestinal worms.

Chronic Suppurative Otitis Media(CSOM)- Know The Causes And Symptoms

MBBS, DLO
ENT Specialist, Faridabad
Chronic Suppurative Otitis Media(CSOM)- Know The Causes And Symptoms

A group of inflammatory diseases in the middle ear is termed as otitis media. When this problem persists more than a period of three months, it is known as chronic suppurative otitis media which can be described as a chronic inflammation of the mastoid cavity and middle ear. It is characterized by discharge from the ear. This happens because the tympanic membrane becomes perforated.

The phenomenon can be categorized into two, safe and unsafe CSOM and they are determined on the basis of whether or not cholesteatoma, which is an abnormal growth of skin in the middle ear, is present. The safe CSOM can be further subdivided into inactive and active depending on whether an infection is present or not. And the unsafe CSOM involves cholesteatoma which is non-malignant but can cause a destructive lesion on the base of the skull.

What are the causes of CSOM?
CSOM or chronic suppurative otitis media is mostly seen in people with chronic mastoiditis as a result of bacterial infection. It may stem from erosion of middle ear walls and the mastoid cavity which, in turn, can cause exposure of the jugular bulb, facial nerve, lateral sinus and temporal lobe dura.

What are the signs and symptoms of chronic suppurative otitis media?
Chronic suppurative otitis media presents itself with leakage from the affected ear. It also includes the possible history of traumatic perforation along with insertion of grommets and otorrhea which may or may not include otalgia or fever. Other symptoms of the disease include vertigo, otalgia, and fever. Your doctor would be able to rule out intracranial or intratemporal complication which is essential for the treatment of the disease. Some people also suffer from hearing loss in the affected ear, and you should ask your doctor regarding the impact of the problem on work and daily living.

On the other hand, the signs of the disease may include oedematousness of the external auditory canal which is not generally tender. Granulation tissue is also seen in the median canal or the ear space in the middle ear.

When there is postauricular swelling or facial paralysis or vertigo or mastoiditis, it is imperative to arrange for urgent assessment or evaluation with a team of ENT experts. The patient must keep the affected ear clean and dry as far as practicable. The instances of life-threatening complications from the disease have been reduced to a great extent with the introduction of sulfonamides and penicillin. According to the medical principles, the primary aim of managing the disease includes eradicating the infection and closure of tympanic perforation. With the help of proper treatment, your doctor would be able to reduce the intensity of hearing loss and a constant threat of microbial infection in the middle portion of the ear. If you wish to discuss about any specific problem, you can consult an Ent Specialist.

Hi! I have an important question. Suppose a girl had sex with a guy without condom and after 7 or 10 days she had sex with another man without man. In both case, the girl did not have any pill to ignore pregnancy. In this case what are the problems those may happen to the girl if she wants the pregnancy? Whose child it will be is not her problem. Her problem is if she face any illness through it.

If the person is suffering from any disease then there is chance of infection. Unprotected sex always has risk of sexually transmitted disease. Consult to discuss in details and solution for the same.

Normal Delivery Vs C-Section - Which One Should You Prefer?

DGO, MBBS, Advanced Infertility
Gynaecologist, Delhi
Normal Delivery Vs C-Section - Which One Should You Prefer?

Normal delivery or vaginal delivery is when childbirth takes place without the use of any vacuum extraction or forceps. It is basically when a woman goes into labour without the use of any other external drugs which induce labour. C-Section or rather caesarean section is basically when a baby is delivered surgically via an incision in the mother’s abdomen. This is usually performed when a normal delivery can put the child’s or the mother’s health at risk. It is also performed when you are expecting twins or rather multiples.

Why Normal Delivery is preferred over C-Section Delivery?

  1. One of the major benefits of having a normal delivery over a C-Section is that the former leads to a shorter hospital stay as there is no major surgery involved. If there is no surgery involved then obviously the risks associated with it, such as, permanent scarring, severe bleeding, septic infection, are completely eliminated.
  2. Reaction to anaesthesia is another thing that one must consider and it is the most crucial difference between a normal and a C-Section delivery. As C-Section is basically a surgery, the mother is administered an anaesthetic to numb the pain. Sometimes, even spinal anaesthesia is used and the side effects of it include: back pain, prolonged numbness or weakness, shortness of breath and in extreme cases, even maternal morbidity (death during pregnancy).
  3. After C-Section delivery, there can be a severe loss of blood leading to hysterectomy (permanent removal of the uterus).
  4. The best reason to avoid a non-planned C-Section is because, once a C-Section has been done then there is a potential risk of having all your future deliveries done via C-Section.
  5. A research has shown that women who deliver via C-Section are less likely to start early breastfeeding as the recovery time from a surgery is pretty high. If you wish to discuss about any specific problem, you can consult a gynaecologist and ask a free question.

You r suffering from pcod thats why delay in menses and weight gain. Primary objective of your treatment is weight reduction and maintain normal BMI of 18-25. This will regularize the monthly cycles and also help you get pregnant. You can take Metformin to reduce weight. You don’t need to take any medicine till 3 months for the menses to come. Only after 3 months of missed menses you should take medicines like progesterone Tablet (Deviry 10 mg twice a day for 5 days) for getting menses. For reducing your weight you should have brisk walk or jogging for 4-5 km daily, high protein diet (fish, chicken, mutton without fat, pulses, mushrooms), low carbohydrate diet (avoid rice, chapatti, bread, poha, upma, idli, dosa, potato, sugar etc and can use bhakri instead) and low fat i. E. Less oil and preferably use rice bran oil.
