Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. For emergency treatment contact your nearest hospital or family doctor. If it does not give relief in 4-5days, contact me again. Why not discuss with me in a video conference? Hi lybrate-user, first I will adviced to take pediatricion opinion about deficiency. As this is summer generally we feel tired, still give healthy, protinious and nutricious diet. Like friuts, green leafy vegetbles. Friuts juices made with water not with milk. For calcium add milk, milky products panner, home made butter.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. FROZEN SHOULDER SYNDROME
(Syn. Periarthritis Shoulder, Adhesive Capsulitis)
Frozen Shoulder Syndrome usually presents as a limitation of shoulder movements associated with pain. It usually presents around the age of 40 years equally affecting males and females. Diabetic and obese people are more prone to it. Bilateral frozen Shoulder is almost always found in Diabetics. It usually starts with minor trauma, often ignored by the patient. The patient often claims it to have insidious, spontaneous onset.
On many occasions the patient may have been confined to bed rest for some medical problem, does not do any shoulder movements, and hence gradually slips into shoulder stiffness. Cervical Spondylosis when causes pain in neck and around shoulder, may initiate and precipitate frozen shoulder.
Treatment is simple.
Diabetes must be ruled out in all the patients and treated well in those who are suffering from it.
No treatment will ever succeed unless diabetes is controlled.
X-ray should be done to rule out other associated disorders
E.g. Koch’s, metastatic deposits
Patient needs anti inflammatory drugs (NSAIDS) to control the inflammatory process going on inside.
Muscle relaxants are needed to relieve the spasm of the muscles. They facilitate the shoulder movements.
Neurotropics, antioxidants and omega three fatty acids are needed to improve the general metabolism and health of the patient. They also potentiate the analgesic effect of NASAIDS.
Sometimes manipulation of the shoulder under General Anaesthesia may have to be undertaken.
With advancement of surgical technology, arthroscopic fibrinolysis of the shoulder joint is undertaken with good results.
In the end, it is the active physiotherapy that is the sheet anchor of treatment, until the patient makes a wilful determined effort no treatment will succeed.

If you are it profession you have to change some postural changes example sitting with straight back support, arms should be rested, avoid prolong standing, forward bending, weight lifting etc. Consult physio they will give treatment to reduce pain after reducing pain you have to strengthen your back muscles. Apply ice or hot pack for 5 to 10 minutes daily 4 to 5 times.

Mere left leg me 5 din se pain ho raha hai. Main apna left leg ko mod ke baith nahi paa rahi hu. Jara sa bhi modne par pain hone lagta hai. Kripya kuch sujhav de jisse mujhe turant aur hamesha ke liye aaram mil jaye.

Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold): keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Shoulder pain k liye I usually give this exercise when the patient has a clear reduction in pain with the use of the Scapular Assistance Test as described by Jeremy Lewis (source) or in those that I think have poor scapular movement, although links to scapular position and movement causing pain and pathology is tenuous (see Chris Littlewoods piece on this here), as is our ability to spot it. However, most people I see with classical sub acromial pain tend to have weak posterior shoulder muscles and this is a great exercise to start addressing this. The prone Y lift is simply done by lying on your front, on a bed or bench, with your arm hanging over the edge, then lift the arm up and out to raise it above your head, and I like to instruct that the thumb is on top pointing upwards to ensure the shoulder is in external rotation (see image below). Again as with the isometrics the exact angle of the lift can be varied, but for best results the research says 120° of abduction produces the stongest contraction in the supraspinatus (source) and upper/lower trapezius muscles (source). I also place a big emphasis on the eccentric action of this an most all other shoulder exercises. Eccentrics are known to produce higher forces in musculotendious units than concentric actions and maintain or increase myofibril length, all essential and positive effects of rehab.

How to avoid lower backache as I am now suffering for more than a year and I could not bent forward and had to do it forcibly.

It needs to be investigated. Investigations advised - hb, tlc, dlc, esr, sugar f & pp, uric acid, vit d3 urine r/e x ray of the affected part. Go for fasting once a week. You can go for absolute fasting or rely on a liquid diet to digest ama (toxin). Fasting should be followed by hot fomentation or by taking a bath with lukewarm water or steam bath. After fasting and fomentation, mix powdered long pepper, black pepper and dried ginger in equal quantities and take 1 teaspoon of this powdered mix with lukewarm water twice a day. This will help you restore your digestive fire. Take one tablespoon of castor oil before going to bed, once or twice a week. It helps in cleansing ama (toxin) build up and balancing vata. Mix equal quantities of powdered fenugreek seeds, turmeric and ginger powder. Take one teaspoon daily in the morning and evening. For dry fomentation of the affected joints, take a cloth and fold it into four, fill it with hot and sand make a potli (bag) out of it. Use this bundle for the fomentation of different joints. This is very helpful in reducing pain and swelling.Apply oil on affected joint gently... before fomentation

Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Cyera 20mg 1 tab od1/2hr. Before meal do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.

You probably have osteoarthritis. You may start on tab. Calcium 1 gm per day with sachets of vit. D3 once a week. Also you may try muscle relaxants for your backpain.
