Pehle karib one year ago mene Dr. Se consult kiya tha tab mere vitamin d ki kami aayi thi so after treatment backache me kafi improvement ho gaya. But aajkal phir se kafi dard shuru ho chuka he. Hello, thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are many causes of pain in lower back region. It can be due to faulty posture, related to weak muscles, issues with intervertebral disc, narrowing of spinal canal, any bony instability besides many other, so accurate diagnosis is the key to proper management of the condition. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. The complaints and ymptoms mentioned by you point towards a possible intervertebral disc prolapse. It needs a detailed history and examination. For starters you can do the following, 1) take rest for few days (3-5) 2) avoid bending and lifting heavy objects 3) can use local heat application and gels 4) pain killers.

Herniated disc, bulged disc, protruded intervertebral disc (PIVD) are the different names of one condition. This is a condition where- disc in between the two vertebral bodies starts peeping out on one side- anterior or posterior. These discs are just like the bags filled with liquid material, which work like shock-absorbent. There are several hard bony vertebrae in the vertebral column and there is plenty of movements with these vertebrae. When we look on one side to other side, we look upwards, downwards or we twist and even we bend forward or backward in each and every condition, there will be a movement in these bones, these discs decide the free movements of the spine. Along with this- when we walk or run there will be shocks and jerks to all these bones and these bags decide that there wont be any kind of jerks in between the bones. So, these discs are of multiple uses.

Our spine is inverted “S” shaped. There are two main curvatures in the spine which are free to move- Cervical and Lumbar curvatures. These curvatures of the spine are maintained due to the spaces in between the two vertebrae. Whenever there is a loss or change (specially reduction) of this space between two vertebrae- pressure will be exerted on the intervertebral discs. This uneven pressure on the disc causes- protrusion/bulge/herniation of the disc- this is the condition known as SLIPPED DISC, in layman language. Why does disc slip

Generally, slipped disc condition is diagnosed by its signs and symptoms. When slipped disc starts exerting pressure on the nerves surround the disc- the nerve will be excited which gives sensation of- Pain, Burning or Numbness in the related area of the nerve. This sensation can travel long far away to the areas, which are being supplied by the nerve and can be limited to a particular location in neck or back even. Sign & Symptoms of Slipped Disc.

Ayurveda about Slipped Disc

Where modern medical (Surgical) sciences believes that this is a problem of just bones and joints of the vertebral disc besides this Ayurveda takes a “Wholistic” (Complete and wholesome) approach towards this and tells that this is a problem at three levels-

  1. Mamsa Dhatu, Asthi Dhatu, Majja Dhatu!!
  2. These are different tissue levels according to Ayurveda- Mamsa is muscles, Asthi represents bones and the Majja denotes everything which is in between and in the bones.
  3. Mamsa (the muscles) starts stiffening due to the dryness – improper diet and imbalanced lifestyle and deteriorate the curvature of the spine- which leads to the deterioration of the intervertebral discs.
  4. Asthi (the bony tissue) is affected due to the muscles.
  5. Majja (the disc and the neural tissue- spine) is compressed due to the pressure and all effects.

So, we need to treat the condition, wisely and in a wholesome manner, rather just to removal of the protruded tissues (Laminotomy) and this is the basic reason- there are risks associated to the surgery of the spine and in almost more than 45% cases patients had to go for a second surgery to stay pain free. Why Surgery for spine doesn’t succeed. Secondly, this is a mechanical problem so medicines alone can give you temporary relief rather than the cure- So better you save your money and invest it on complete cure for the problem, rather than just taking the pain killers and medicines which were indicated in epilepsy to just reduce the signs and symptoms. Pain Mechanism: What Are The Causes of Pain?

Pain is an unpleasant and distressing physical sensation caused by disease or injury, which induces hurt and anxiety in those suffering from it.
There are many different manifestations of pain, ranging from short term (acute pain) to long term (chronic pain). Other classifications include pain in the internal organs (visceral pain), injured tissue (inflammatory pain), nerves (neuropathic pain), etc.

The reason for the occurrence of pain depends on the cause and kind of pain being experienced. In most cases, pain acts as a warning sign that the body has been afflicted by a disorder of some sort, which may have arisen due to internal sickness or external wounding. Acute pains are generally cured on their own through rest or simple medication. Chronic pains, on the other hand, are more complicated in nature and the treatment requires more elaborate diagnosis.

The mechanism of pain:

Pain is an extremely personal and subjective experience and affects each individual to varying degrees. The pain signals, which can arise in any part of the body, travel through the spinal cord to the brain along thousands of specialized nerves and nerve fibres. In the brain, it is processed in the centres associated with anxiety, emotions, memory, appetite, etc. Signals and pain inputs are then returned from the brain to the spinal cord, which may heighten or diminish pain.

In some cases, pain may also be induced by damage to the brain and spinal cord, which happens after a stroke.
A constant barrage of pain signals may cause the cells at the end of nerve fibres to become over-sensitized. This is known as ‘wind-up’ and is one of the most common causes of chronic pain that occurs even though the root of the problem has been identified and treated.

The causes and effects of pain differ from case to case depending on the signals received by the brain and its interpretation. It affects the individual’s day to day activities and if persistent, can also have a harmful impact on mental health and psychosis. Pain is always handled, diagnosed and treated differently in all patients by drawing out the best possible solution to the problem. If you wish to discuss about any specific problem, you can consult a pain management specialist.

First in 99 it was diagnosed as hair line fracture, it got healed later slowly because due to school and college I was sitting on hard surface for long hours because I don't know about wedge cushions at that time, now I am sitting on wedge cushion but pain is there, what is the solution for my problem as it is becoming difficult to carry daily activities. Hiii.... Make sure you sit straight. When you sit straight, your coccyx wont touch the sitting surface. Make your spine straight and rotate your pelvis forward so that your coccyx doesnt touch the seat. Also make sure your vitamin D is normal. Get adequate sunlight exposure for atleast 20 mins a day. Also learn some core exercises by consulting a physio nearby. This should help.

Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Consult after 5 days. For further guidance and treatment.

I actually a titanium plate in my left thigh, the operation was in nov 2006. I do not have any problem though, and at the time of operation too the doctors said there is no need to take out the plate. Should I care about it?

It's better to remove plate in young people after fracture heals. It's not possible many times to remove implants after 10 years. So forget about it now. Most of the times, there are no problems.

Requires detailed investigation and examination Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomentation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

Yes acupressure can help reduce pain. Also ask her not to sit cross legged, avoid squatting position, avoid climbing stairs, and do regular exercise of knee. This help to slow process of arthritis she is having.

Dear Siddharth, namaskar, Spondylitis does not occur at this age....Sprain needs an injury to occur.....Your problem of pain in cold weather seems to be myalgia...Do hot fomentation and protect urself from cold by a muflar like cloth... Strengthen your muscles by doing Cervical spine exercises round the year OR at least when u don't have pain.... You will settle gradually... take care.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Hi we should give strignth to left leg so take following remedies 1. Lakshadi guggul 2 tab twice a day 2. Dashmularishta 4 tsp before lunch and dinner in ayurveda there is mention panchakarma treatment. Panchakarma means five procedure for detoxification of body in this five procedure for leg pain you should do basti treatment. You will get good result so consult good ayurvedic doctors.
