I am about to get married, need to ask about family planing issue. Me and my wife would be using condoms for the same as we are planning baby after 2 years. I need to ask is there any side effects of using condoms for such a period? And also how to stop pregnancy if in case there is unprotected mate between us. Hello- you can do sex without condom in safe period of the pregnancy (4-10th day & 21-28th day in a 28 days cycle) however in between these days you can use condom to avoid pregnancy. Don't go for hormonal pills for contraception as it will reduce your chances of pregnancy in future when you need most.

Here are Causes, Symptoms, and Treatment of Chest Pain

Good evening friends, I am Dr. Bharat B. Kukreti. I am a CI consultant intervention Cardiology at Paras Hospitals, Gurgaon. Today, we will discuss heart problem manifestation, chest pain.

We all have experienced chest pain at some point in time. It is such a common syndrome that everybody has gone through it at some point. The important thing here is that chest pain may be associated with a wide variety of disease which may range from a very simple gastritis to a very lethal heart attack. So the problem with chest pain is that we can’t identify whether we are going through a heart attack or just another chest pain. So, this is what we will be discussing today.

First of all, How your chest pain occurs in a heart attack? A heart attack patient will typically about chest pain in a sensation of heaviness or squeezing or burning in the central chest or the left side of the chest which is radiating to either one arm or both arm or shoulders or jaw. It may be associated with a very profuse sweating and a sensation of restlessness, breathing difficulty or the sometimes patient may just faint. These all will be the symptoms of an ongoing heart attack.

Important thing is that these symptoms can be because of a heart attack but sometimes heart attack can occur even without the proper symptom which you can identify. Then, it becomes a difficult situation to identify how we know that it is a heart attack. So, once somebody is having a chest pain, you should first consult a doctor and get an E. C. G. done and to see whether the chest pain seems to be of heart region or something else. So, what can this something else be? This something else, that is a differential diagnosis of the chest pain may be apart from heart these other structures like lungs, they may be bone, they may be the muscular origin and the chest pain may be of neurological origin and sometimes just simple gastritis can give you a very severe chest pain also.

So, out of all these probabilities, the heart disease probability is the most dangerous. So that’s why once somebody has chest pain, it’s important to rule out the heart problem first so that life can be saved. So, if somebody is having chest pain, the first thing to be done is E. C. G. and it is to be seen by a qualified doctor to see whether the E. C. G. is suggestive of a heart problem or not. There is one interesting fact here. Up to 50% of the first E. C. G.s of the heart patients may be normal. So the first normal E. C. G. does not rule out a heart attack. Diagnosis for ruling out a heart attack is a systematic approach which includes Serial ECGs; Serial ECG means ECG now, ECG at half an hour, ECG at one hour, ECG at two hours, thus several ECGs and then afterward Cardiac Enzyme Test is done.

For most people, the screen of a cell phone is the first thing they see in the morning and the last thing they see before going to bed. Nobody can deny the fact that living without a cell phone today is extremely difficult but must we be so glued to them? Staring into a phone or tablet screen, before going to sleep affects your sleep as well as impacts your health. Here's how:

  • It is harder to fall asleep - We feel sleepy when the brain releases a hormone called melatonin. This release is determined by the body's circadian rhythm which is controlled by light exposure. The blue light of a cell phone screen interferes with this rhythm and tells the brain not to secrete melatonin. This makes it harder to fall asleep even after you have switched off your phone.
  • It hampers the quality of your sleep - A direct effect of sleeping late is a reduced amount of sleep. On top, that, using a cell phone before going to bed also hampers the quality of your sleep. When an office email or a text message is the last thing you see before going to sleep, your brain cannot completely shut off. This makes you tense even in your sleep.
  • It lowers your attention span - When you wake up from a restless night's sleep, your productivity suffers. Not getting enough sleep is known to hamper your decision-making skills, impairs both long term and short term memory and make you distracted. This leads to unnecessary stress and anxiety.
  • It increases the risk of weight gain - The body is more likely to feel hungry when you don't have enough sleep. This is because sleep-deprived people have a lower metabolism. Thus using a cell phone before sleeping also increases your chances of snacking on something unhealthy to get through the day and in turn you put on weight.
  • It weakens your immune system - The body's immune system produces antibodies that fight infections while we are asleep. Depriving the body of the sleep it needs weakens the immune system by not giving it enough time to build its forces. Not only does this increase the risk of falling ill it also slows down recovery from illnesses.
  • It damages your skin - Sleeping late at night and not getting enough sleep can cause dark circles, puffy eyes and fine lines across the face. The blue light of a cell phone screen also tells the body to secrete cortisol which makes the skin less elastic and smooth.

Thus, even if you look at a phone screen for just five minutes every night before going to bed, the side effects last much longer and affect your mental and physical health as well as your looks.

Yes uterine malformation may be associated with painful periods and subfertility. You will need a laparoscopy / hysteroscopy and dye test to evaluate your condition a bit further. Depending on the exact nature of the problem corrective surgery can be done at the same time.

Types of Hemorrhage

In simple language, Hemorrhage is when the blood escapes from veins or blood vessels. This escape may happen because of various reasons like, for example, injury, low platelet count, threatened abortion, piles (Hemorrhoids), cancer, surgery, liver disease, brain trauma, acute bronchitis, menstrual problem and so on.  

Types of Hemorrhage: 

As a matter of fact, Hemorrhage is grouped into several categories. Hemorrhage can occur post surgery, and such a Hemorrhage is called as postoperative Hemorrhage. Similarly, for various reasons Hemorrhage can occur from the artery (Arterial Hemorrhage), vein (Venous Hemorrhage) or capillary (Capillary Hemorrhage). If Hemorrhage happens because of an injury, it is called primary Hemorrhage.  

Other types of Hemorrhage are delayed bleeding post injury (Reactionary Hemorrhage), bleeding following childbirth (Postpartum Hemorrhage), delay in bleeding following Sepsis popularly called as septic (Secondary Hemorrhage), bleeding from urine (Haematuria), bleeding while coughing (Hemoptysis) and bleeding caused in the stomach (Hematemesis). 

Further, Hemorrhage can be broadly classified into two namely internal and external:

  1. It would be needless to mention that excessive bleeding may lead to several complications. It can result in organ failure and there are instances that excessive bleeding can be fatal. On the other hand, blood loss can be external or visible Hemorrhage like those caused during an injury. There are also instances of blood loss occurring internally in which case the patient may notice blood either in the urine or stools, vagina or while coughing or through the nostrils or even from the mouth. In such cases, the patient should be rushed to a doctor who after careful examination, and tests would be able to identify the causes of the internal bleeding. 
  2. In case of external bleeding or external Hemorrhage like those caused due to an injury, you may provide the necessary first aid. When the bleeding is noticed, do not get alarmed and also try to keep the patient calm. Hemorrhage caused due to external injury would normally stop within one or two minutes. However, if the bleeding persists even after two or three minutes, such cases would require medical attention, and therefore, you should rush the patient to the doctor at the earliest. 

Remedies

There are several remedies to tackle the problem of Hemorrhage. Some of the normal procedures followed are IV fluid, oxygen, administering medicines like vitamin B12 and so on. In complicated cases, the physician may suggest bone marrow surgical procedure, Endogenous EPO or such other procedures. If Hemorrhage is left unattended, it can result in several complications. Therefore, you should rush the patient to the physician urgently.

Last month my wife had menses only for one and a half day. This month she didn't had menses. We did urine pregnancy test and the result was negative. But still menses didn't come. After one week we again did the test and the result was null. The next day we again did a urine test and it also resulted in null. Although evrytime we took a new device for test. She doesn't want to get pregnant. What should we do to avoid pregnancy. Please help. Menses time is between 10 -15th of the month. Should we wait for next month.

Hi Mr. Lybrate-user, please get a blood test called serum bhcg to confirm that you are not pregnant. Also get pelvic ultrasound done to rule out ovarian cyst. She will need medicines to get period, don't worry mostly it will not be pregnancy.

you can try for pregnancy during ovulation period which is 8t to 18th day of period.get an ultrasound of lower abdomen and thyroid test and husband's semen analysis test done.visit to doctor for further medicinal treatment.

Basti Therapy: Best Way To Treat Vata Issues, Especially In Monsoon

In Ayurveda, vata is considered to be extremely important. It regulates the movements of your body and mind. In the body, large intestine is considered to be the main site of vata as well as in the bone tissue. Therefore, medications which are administered through the rectum affect the bone tissue. Vata maintains and regulates our daily activities, nervous system as well as our excretion. An enthusiastic and a lively mind are all manifestations of a balance of Vata. 

Subsequently, its imbalance is considered as detrimental to health. Basti therapy is a treatment that manages the pain and treats the diseases arising from this imbalance. In this, a liquid concoction of sesame oil and other herbs are inserted through the rectum, which in turn treats various ailments arising from the imbalance of vata, such as pain in the joints, in the spine and back as well as painful menstruations.
 
Before Basti therapy, Abhyangam and Sedanam are practiced. Enema can be assumed, but Basti has power to detox and nourish as well. Yoga Basti for is done for 8 days, Karma Basti for 1 day and Kala Basti for 30 days, or according to the health issues and affected system, in order  to drain doshas and nourish dhatus.

There are primarily three types of Basti therapy:

  1. Niruha
  2. Anuvasana
  3. Uttar Basti

All three of them, individually treat various ailments, some of them are:

  • Stiffness in the back and pain in the spine: In this method, pain in the spine and vertebral column are managed and treated. It involves pouring warm, herbal oil on the spinal cord, which is retained by small wells created from flour or gram paste. This method is particularly beneficial for treating back problems. This method nourishes the liquid in the joints that consequently relieves you of the pain.
  • Bell's palsy: Bell's palsy is a disease where the muscles on the face become weak or you suffer from a facial paralysis. Basti therapy has proved to be particularly beneficial in treating it. In this, a medicated oil is placed on the head for a specific period of time which helps in treating Bell's palsy as well as any numbness on the face.
  • Painful menstruation: Though menstruation is a natural process, most women suffer from extremely painful cramps during this time. Anuvasana therapy is extremely useful in lessening this pain. In this various oils and decoction enema is provided to patients, the consumption of which have resulted in significant improvements. Painful menstruation cramps are also treated by Uttar Basti where medicines are provided through the uterus. This method helps in cleaning the blockages in the uterus as well as providing nourishment to it. 

Ailments that accompany monsoon: Though monsoon provides relief from the scorching summer, but at the same time it is accompanied by a host of diseases. With the onset of monsoon and the fall in temperature, there is a tendency for acidity and the vata that was accumulated in summer manifests itself in loss of appetite, indigestion and piles. Basti therapy operates as a corrective for the vata imbalance and treats various digestion related problems. At the same time it also prevents return of the ailments. The oils and decoction used rejuvenates the body and as well as help in regulating the metabolism.

I think he is suffering from chronic otitis media. Symptoms reccur after catching cold. He may have a small perforation in his tympanic membrane but must be confirmed by otological examination. He may need antibiotic ear dps. It heals with passage of time. Precations sud be taken to save him from frequent cold. Keep his ear dry and not allow water to go into his ears while bathing. Medication is required if there is active discharge from the ear.

Vaginal tightening is only achieved by kegel exercises if done correctly. Surgery to tighten vagina is not the solution for you as it can leave a scar. You should get hymenoplasty done to have bleeding after the marriage, which is considered by many a sign of virginity. Having sex does not relax vagina that much, so you should not be much concerned about it. Only childbirth trauma relaxes the vagina which needs to be tightened.

Do you know that the future health of your baby is decided before you even conceive? Your preconception health is very important. Planning a baby is a demanding task, but if you plan it well you will have a healthier and easier pregnancy. 

These are a few tips which can do wonders

1. Relaxation at Work and Household Backup - Plan out your work schedule. You might need to change your work hours or may be change the job profile. You will also need someone to help you out with the household chores around this time.

2. Finances - Get your finances settled and insurance plan changed accordingly.

3. Lifestyle Modification - Eat healthy. Stop the intake of alcohol and stop smoking. Set aside a time for regular exercise if possible. Try to lose that extra flab and stay very active.

4. Reduce Stress - Create a support system for yourself. Engage in activities that uplift your mood.

5. Take Folic Acid - Take 400 mcg of folic acid tablet daily. It helps prevent birth defects of brain and spine in the baby.

6. Avoid Exposure to - Harmful chemicals, metals and toxic substances at workplace and home. Protect yourself from infections. Stay away from cats.

7. Up-to-Date Vaccination - Make sure your vaccination is complete. Vaccination against rubella and flu are very important. It is also important to maintain a gap of at least one month between your rubella shot and pregnancy planning.

8. Optimize your Medical Condition - If you have Hypertension, Diabetes, Asthma, Epilepsy or any other medical condition you must take complete advice from the physician. Blood pressure and sugar levels have to be under full control. The type and dose of the drug may need modifications.

9. Check Family History - Ask your parents, grandparents, brothers, sisters, aunts and cousins about any health problems in the family like genetic problems, mental retardation or malformations in babies. This can help your Gynaecologist to determine which problems to look for and how to prevent it. 

10. Visit your Doctor - Carry a record of your medical conditions, family history and a checklist of your queries. Get tested for Li Blood Group, Thalassemia, HIV, Thyroid Levels, and Blood Sugars.
