Hello! I am Dr. Bheem Nanda. I am a consultant cosmetic and plastic surgeon at Sir Ganga Ram Hospital, New Delhi.

Nowadays as we can see, there is too much awareness regarding the transgender group. There is a very big LGBT group and now the Supreme court is also considering Article 377. Nowadays society is also accepting these people. Now we can provide sex-change surgery for these people.

There is a condition called Gender Dysphoria. Gender Dysphoria means mentally what the person thinks is of different sex than his physical body or chromosomal body basically. Like if there is an absolutely normal female but mentally she thinks she is a boy. This battle between the mental sex and the genital sex is called gender dysphoria. The diagnosis depends on the psychiatrist. He evaluates the patient whether he or she is a case of gender dysphoria or not then. Once he establishes the diagnosis, he then gives the clearance for hormone therapy. Then these people undergo hormone therapy. They take the hormone and then changes occur in in their body. Once they are comfortable enough , only then the psychiatrist gives clearance for the gender affirmation surgery.

In Gender affirmation surgery , if it is a female, we can go ahead and make her a male and if it is a male and then we can do surgery to make him a female. Then there is a battle between the mental sex and the genital sex. When we consider female to male transformation , then the stage-1 surgery is removal of the breast. In stage-2 surgery,  we remove the ovaries, vagina and uterus. In stage-3 surgery ,we do penis reconstruction. After that, depending on the what the patient desires, we can go ahead with the scrotum reconstruction, testicular implants, hair transplant for beard or mustache etc.

We have to make an implant in penis for erection and all those things. We can do many procedures to make a female into a male. If we consider a Gender dysphoria in case of male to female , then in those patients in Stage-1 surgery we would remove the penis and make the vagina. In the same sitting , Labia majora can also be made. In Stage-2 we do the breast implants. In stage 3 , we can do face contouring like rhinoplasty to make the nose thin, jaw contouring, facial profile correction etc. We can even do voice correction also and laser hair removal for whole body or for mustache.

Liposuction - When Will You Be Ready?

Sometimes, even a strict diet and strenuous exercise doesn't give you the body shape you dream of. In such cases, liposuction can be the answer to the problem of excess fat you are facing. Liposuction is a surgical procedure of removing stubborn fat bulges from the body. Age is not a major consideration with liposuction as long as your skin is firm and elastic. Else, your skin may begin to sag in places where the fat has been removed. It is important to note that liposuction does not remove cellulite but only removes fat. Thus, for people with cellulite, liposuction may not give them the results they desire. 

If you're thinking of undergoing liposuction, do not expect the procedure to give you a supermodel's figure. Be realistic in your expectations. Here are a few other things to keep in mind :

- Liposuction does not show instant results

Recovery from a liposuction surgery is quick and you may be back to your normal routine within a few weeks. However, the treated area may still look swollen. This can take two to three months to subside. Hence work backwards and time your surgery with regard to major life events.

- The results are long lasting

Undergoing liposuction surgery does not mean that you will not be overweight again, but it means that fat will not collect in the areas that have been treated. If you do gain weight, the fat will be redistributed to other areas. 

- Smoking and alcohol must be avoided

Smoking and alcohol both inhibit healing. It also increases your risk of complications during the surgery. Stop drinking and smoking at least three weeks prior to your surgery, and avoid it until your body has healed completely. 

- Can be beneficial for many parts of the body

Liposuction can be performed on many areas of the body. The thighs, abdomen, breasts and buttocks are common areas where this surgery is performed. Other areas are the back, face and neck. 

Like any other surgery, liposuction has its risks. While some risks are very rare, others can be quite common. Here are three minor risks that you should be aware of:

- Excessive liposuction - Removing too much fat can leave your skin looking unnatural and uneven. 

- Swelling and bruising -  When liposuction is performed on the lower part of the body, swelling and bruising are common side effects. This usually disappears within 2-3 days. Choosing open drainage vs. placing sutures on the incision sites can reduce the chances of this happening.

Preparations For Facial Surgery!

Facial surgery can be opted by people for a number of reasons, and these include being born with facial impairments like cleft lip, birthmark or other birth defects, the effects of aging, sun damage or other facial traumas that are noticeable on the face as well as a sense of general dissatisfaction with your facial features. Depending on these factors, facial plastic surgery can be divided into two categories - reconstructive and cosmetic.

Reconstructive plastic surgery is performed for correcting conditions that may be present from birth, including birthmarks on the face, cleft lip and palate, protruding ears and a crooked smile. Conditions resulting from accidents, traumas or burns are also corrected by this type of surgery.

Cosmetic facial plastic surgery is performed to enhance visual appearance of facial structures and features which include facelifts, eye lifts, rhinoplasty, cheek and chin implants and liposuction.

If you are planning to get a facial surgery done, there are certain things you need to do to prepare yourself for it. These are :

1. Schedule a surgery when you are healthy

If you are planning for either of the surgeries, try to schedule it during a period of relatively good health. Your immune system will have the best chance towards a speedy recovery. Scheduling the surgery at the year-end is ideal when you can take a few days off with paid vacation time.

2. Get enough essential nutrients

Your diet, before and after the surgery should be rich in vitamin A, vitamin C, zinc, copper and selenium. They help the wound to heal quickly and in building your immunity, thereby reducing the chances of an infection. You can recover from more comprehensive procedures like tummy tuck, facelift or liposuction by including the requisite nutrients in your diet.

3. Protein intake

Calories and protein are the two most healing elements. Extra protein will help to build new tissue and blood vessels, repair injured tissue and increase production of cells that heal the wound. Choose high quality protein sources like fish, poultry, beans, legumes and meat.

4. Stay hydrated

Consuming 6-8 glasses on the day prior to the surgery is very important to help cleanse and hydrate the body. Since fluid intake is reduced on the day of surgery, water intake is especially important prior to the surgery. To avoid complications during surgery, ensure that you do not drink anything after midnight the night prior to your surgery, unless advised by your surgeon.

Over the past decade, many women have opted for Breast Augmentation Surgery. It is a surgical process to improve the aesthetics of one's breast. Apart from giving a much needed lift to the bust size, the augmented breasts can do your confidence a world of good. Breast Augmentation Surgery, if gone wrong, can create havoc. The outcome of the surgery can be influenced by a number of factors.

Thus, before going under the knife, some important and vital points need to be taken care of :

  • It is advisable to avail the services of an experienced surgeon, who has a higher success rate. A lot of confusions and apprehensions may arise during course of the surgery. An experienced surgeon will ease out the tension to a great extend and guide you better.

  • Breast Augmentation is no child's play: Do not embark on breast augmentation surgery if you are below 18 years of age. It can result in serious health complications. Surgery can wait, but health once lost, will be gone forever.
  • The type of implants to be used: Saline and Silicone implants are commonly used in breast augmentation. Both the implants come with their own sets of merits and demerits.


Being softer and smoother, Silicone implants give the augmented breasts a very natural feel. The chances of scalloping (the skin around the breasts appear wavy) are negligible in case of silicone implants. However, the silicone implants come pre filled. Thus, a bigger incision is needed to get it inserted. Though approved by the FDA, leakage of the silicone implants into the body have been reported. The leakage can affect the health seriously. Interference of the silicone implants with the mammogram can also pose a serious threat.

  • The Saline implants, on the other hand are not pre filled. Thus, a small incision is all it takes to get the implants inserted. Once inserted, the implants are filled with saline solution. The saline implants can also leak into the body, without any serious implications. The biggest limitation of saline implants lie in the fact that it gives the breasts a very hard feel. Scalloping cannot be ruled out either.
  • Do not go for breast augmentation surgery if you are planning to start a family in 1-2 years of time. A weaning or lactating lady should also avoid the surgery.

 

  • The exact location of the implant is an important factor to ponder upon.  One can go for an under the muscle or over the muscle implants.

 

  • One can opt for a round implant or a tear shaped implant. The biggest USP of tear shaped implant lies in the fact that it gives a very natural feel to the augmented breasts. The round implants, on the other hand, are much safer to use.


Go for the surgery only when you are physically, mentally and emotionally fit and healthy.

Scar Removal From Face - What Should You Know?

Scars may mar the beauty and proportion of your face and its features. There are many reasons why we may suffer from scars starting from accidents to other causes like birth defects and other ailments. When we are injured or the skin is ruptured, the bruise or the wound usually heals itself by turning into scar tissue.

While much of this may disappear for smaller wounds, there are others that remain obvious and become scars. Many of these scars may be in a prominent location like the face, which can lead to loss of confidence and self-esteem. While make-up and creams can diminish the jarring effect of such scars, there is also a permanent way to deal with them cosmetic surgery. Here is everything you need to know about the same.

  • Point of surgical techniques: While total scar removal will depend on the severity of the scar and its location, the point of most surgical techniques for scar removal involve making the skin as smooth as possible so that the scar becomes almost invisible.
  • Skin resurfacing methods: Dermabrasion and laser resurfacing are some of the most popular methods employed for this process. While dermabrasion basically helps in scraping away the top surface of the skin in order to help new skin grow back, laser resurfacing does pretty much the same with the help of laser beams that will act on the problem areas. Dermabrasion usually helps when the scar is slightly raised and embossed on the skin, giving the appearance of a second skin with its defect. The laser light of the laser resurfacing helps by smoothen the rough patch of skin that forms the scar. This is done by running the beams on the rough or elevated patches until they are removed for the new skin to emerge eventually.
  • Kelotoid scars: Most kelotoid scars are treated with the help of medicated injections that can be administered by a cosmetic surgeon. Hypertropic scars are also treated in this way.
  • Surgical incisions: In case the above methods do not help in removing particularly stubborn or old scars, the cosmetic surgeon can also suggest using a surgical incision in order to join the skin together with very fine stitches. This process can help in making the scar less prominent to a great extent.
  • Side effects: In all these methods, you would do well to discuss the possible side effects with your cosmetic surgeon. Some amount of bruising and redness of the skin is to be expected for a few days. Also, a little bit of swelling is normal. But if these side effects do not abate in a hurry and get progressively worse, then it would be a good idea to report the changes to the doctor for further treatment.

Facelift Surgery - Know The Complications!

The skin has underlying protein layers, mainly having collagen and keratin. With age, they lose elasticity and the skin begins to sag. This is where facelift gained popularity with various techniques including Botox, chemical fillers, laser beam treatment, and chemical peels. Read on to know how these minimally invasive, economic options work in literally lifting up the face:

  1. Laser treatment: A beam of laser (usually CO2 laser) is used to make small dots on the skin with normal skin left untouched in between. This causes tightening of the tissue underneath, producing a firmer appearance which can take off years from the appearance.
  2. Botox: This accidentally discovered neurotoxin is used to treat wrinkles and sagging skin. The sagging areas of the face (usually forehead and nasal folds) are injected with Botox. The wrinkles are gone and the fine lines are lost, and the result can last for up to 8 months.
  3. Fillers: Chemicals which are naturally present in the skin like hyaluronic acid and collagen extracts are injected into the skin to fill up the underlying skin and produce a fuller, younger appearance. The lower cheeks, corners of the eyes and mouth, and area under the eyes are commonly treated by this method.
  4. Chemical peeling: The top layer is often peeled off using chemicals so that fresh, younger-looking skin is generated. This also helps in reducing pigmentation, reducing forehead lines, de-tanning, and producing a glowing, radiant, younger-appearing skin. It also can be used in other skin conditions including rosacea.

While each of these is widely popular, they come with a set of potential risks and complications.

  1. Anaesthetic reaction: Most of the above procedures require local anaesthesia and there could be a reaction to this chemical.
  2. Hematoma: There could be small hematomas in the underneath layers of the skin, which will take some time but dissolve on its own
  3. Numbness/tingling: The face has a rich nerve supply, and if any of these are touched, there could be numbness or tingling or even loss of sensation in some cases. While some may improve with time, there also could be permanent damage
  4. Scarring: In chemical peels, there could be minor incisions and therefore scarring can happen.
  5. Delayed healing: Depending on the person’s immunity, healing could be delayed, and as it is the face, social and/or professional life can be affected.
  6. Infection: As with any surgery, chances of infection arise and can be managed with antibiotics.
  7. Bleeding: As most of these are non- or minimally-invasive, chances of severe bleeding are less but still persist.
  8. Bruising: The face, in the immediate postop period, can have a bruised appearance, which will improve over 4 to 5 days.

Undereye hollows are the opposite of having puffy eyes. The procedure dates to the Roman and Greek eras but with the modernization of technology the procedure has evolved, and cosmetic surgeons now use laser blepharoplasty for the procedure which is much safer and non-invasive in nature. The procedure usually takes between one to three hours to complete, and any internal swelling that occurs will resolve itself within a couple of weeks.
