I had a surgery in last year December with ovarian cyst. My question is now I have a problem of back pain whenever I do some work and I have still scars on my abdomen. Can you suggest me some medication and ointment to remove this scar.

MBBS, DGO, MD, Fellowship in Gynae Oncology
Gynaecologist, Delhi
Were you given spinal anesthesia for the surgery, if so some may have pain for few months on exertion. You can consult physiotherapist for this for some exercise. And for the scars you can use calamine lotion, and coconut oil.

I am 53 year old, have regular interest in sex but since a year or more I have erection problem and premature ejaculation. I am regularly taking losartan-h for high bp and my bp is under control and normally be 128/80. I often take 03 peg liquor (whisky) at night. Please advice, should I take tadalafil and dapoxitinne tablets or some other drug, and its dose. Thank you.

BAMS
General Physician, Delhi
Please don't take it will affect on heart very badly. As ejection problem is concern, some muscle aging is there due to age & bp, drugs which control bp, because blood flow is increased during erection & your bp problem which is managed by tablets can't support this any way. Try cap xilavit twice daily for one month & have sex with a gap of twice a week or so.

Hello doc, my problems:
1.Semen leakage 2.Premature ejaculation 3. Nightfall once in a week I want to get treatment a...
Dear Lybrate user. Semen seeping through penis after urination or while having sexual thoughts or while with an attractive girl or woman etc is common and it is not a problem at all. This is a natural body procedure whereby your body tries to evacuate the stored semen. Semen is produced within your body whenever you are sexually (even slightly) excited and is stored within your body. Now this stored semen is a waste product that your body wants to throw out. This is a natural process and you don't have any control over it. I also assure you that this is never unhealthy. You must understand premature ejaculation. Premature ejaculation is diagnosed when you have premature ejaculation within one minute or sex, continuously several times with the same partner for over six months. There is nothing called premature ejaculation while doing masturbation. Are you married? Premature ejaculation occurs when a man ejaculates sooner during sexual intercourse than he or his partner would like. Premature ejaculation is a common problem which happens due to psychological stress and anxiety in majority of people. After confirming that these are not due to diabetics, hypertension or any other organic problems, for this reason, selective serotonin reuptake inhibitors (SSRIs), such as sertraline (Zoloft), paroxetine (Paxil) or fluoxetine (Prozac, Sarafem), are used to help delay ejaculation. If SSRIs don't improve the timing of your ejaculation, your doctor may prescribe the tricyclic antidepressant clomipramine (Anafranil). I don't see any requirement of medicines for you. Two natural treatments for PE that have shown promise are red ginseng and pomegranate juice. Ginseng is thought to increase nitric oxide production, leading to improved blood flow. Pomegranate juice is a potent antioxidant and can help too. Japan oil can help if massaged externally. Psychological reasons usually get cleared with sex therapy. I suggest online sex therapy. A nocturnal emission or involuntary leakage is an involuntary ejaculation of semen that occurs during sleep or along with urine while urination. Studies show that approximately 83% of men experience a nocturnal emission or leakage at some point in their lives. There is nothing wrong with a person if he or she experiences a nocturnal orgasm or nocturnal emission or leakage, as they are normal and healthy experiences. If you have experienced a wet dream, you should not feel ashamed. There is no way to prevent nocturnal orgasms or emissions from happening. Sage tea is the best home remedy which is used to treat conditions such as nightfall. If you drink at least two cups in a day, the chances of getting wet dreams will reduce too. Take fenugreek with honey on a regular basis. Exercise regularly and practice yoga and meditation to keep calm and relaxed. I suggest online sex counseling. Take care.

I am a tobacco addict. From last few days I am trying to quit this habit but I am facing many problems like headache,...
For most people, the first week of quitting is the most difficult time. This is a time when getting over both the psychological dependency on cigarettes and physical effects of nicotine withdrawal can feel overwhelming. You might feel anxious, depressed, impatient, tired, or even hostile. During this critical time, it may be very tempting to go back to old behaviors and use smoking to deal with these feelings. Don’t worry, this stage in the process and the feelings you may have are only temporary. Although you may have cravings to smoke weeks or even months after quitting, as more time goes by, these urges will diminish in strength and frequency. Nicotine withdrawal symptoms are more short-lived, and should only last a few days (or a couple weeks) at most. When you put that into perspective with being smoke-free for the rest of your life, you know you can do it! The symptoms you experience in the first week will vary from one person to another, depending on how much nicotine your body is used to, and how much you smoked. Keep the tips below in mind to help you get through that important first week and set yourself up for success: •Remember that urges to smoke will come and go. Try waiting it out for a few minutes and occupying your mind with something else. •Remind yourself every day of why you are quitting. Your motivations for quitting can powerful reminders for when you’re in the worst parts of the quitting process. This is when you need them most to keep you on tract. •Write down your rationalizations. One way to help overcome cravings is to write down any rationalizations you may have for having a cigarette. Rationalizing is trying to find a good reason to do something you know is wrong. For example, “I’ll just have one cigarette today to get through this tough time,” or “you’ve got to die of something, might as well smoke.” If you write down rationalizations as they come to mind, it will help you to see them for what they are: traps to get you back into your smoking habit. •Drink plenty of water. Water cleanses and purifies your system while also giving you something to do with your hands. When you’re fully hydrated, your urge to eat is reduced as you’re stomach is full with liquids. This is a great tip if you’re concerned about weight gain. You can also drink herbal teas or fruit juices. •Take your mind off of a problem and come back to it later. With the symptoms of withdrawal at their height, you may feel that the world is against you. Remember that these feelings are only temporary. It may be tempting to think that it was better when you smoked because you didn’t feel this way. The truth is, these feelings will go away and once you’re past nicotine withdrawal, you are ready to feel even better than you ever did when you smoked. •Change everything! In many ways the habitual part of smoking cigarettes can be tied into your daily routines – the first cigarette with coffee in the morning, the drive to work, the smoke breaks, the after dinner cigarette. Think of ways to change everything so you can break free of the habit aspect of smoking. This is just a short-term adjustment. Once cigarettes are no longer a part of your life, you’ll go back to having different routines that you enjoy without smoking. •Don’t be afraid to ask for help. Your friends and loved ones want to support you any way they can. Your doctor can also be a great asset to you in leaving cigarettes behind, and can direct you toward other resources for help. Once you’ve gotten through the first week, you’ve passed a big milestone in the quitting process. While you may still find yourself dealing with urges and temptations to smoke, you will have the experience and success of your fist week behind you. In the weeks that follow, stay focused and keep moving toward your goal! When you quit smokeless tobacco, your withdrawal symptoms will be strongest during the first week. After two weeks of not using smokeless tobacco, your body will be rid of the addicting nicotine and the worst part will be over. Soon, you will feel better than you ever did when you dipped or chewed. But in the meantime, know the side effects you will experience when quitting so you can better deal with them: •Craving smokeless tobacco — Since nicotine is addictive, you'll have cravings for smokeless tobacco. Try waiting it out. Each urge to use will usually last only three to five minutes. Deep breathing and exercise help you feel better right away. •Feeling irritable, tense, restless or edgy— You're going through nicotine withdrawal. Try some form of exercise. Ask others to be patient. •Feeling hungry or having a desire for sweets — People do sometimes experience weight gain when they stop using tobacco. If you get hungry, drink fruit juices or eat a low calorie snack. Drink a lot of water. •Constipation/irregularity — Add fiber to your diet, like whole grain breads and cereals, fresh fruits and vegetables. If you have any further query feel free to ask. Regards, Sukanya Biswas.

Ways to Avoid Fatigue in HIV Patients

FHM FELLOWSHIP IN HIV MEDICINE, MD, MBBS
General Physician, Hyderabad
Ways to Avoid Fatigue in HIV Patients

HIV or the human immunodeficiency virus is a virus which causes HIV infection and leads to acquired immunodeficiency syndrome or AIDS. HIV affects a person in various ways over time. HIV patients are commonly characterized by tiredness and intense fatigue. There are many causes, which lead to fatigue and there are ways to reduce fatigue as well.

  • HIV virus as a cause of fatigue: HIV is a long-term infection in nature, and our body provides strong immunity to respond the virus. A lot of energy is capitalized for constantly fighting against the HIV. Fatigue slowly develops as a result of this. Some HIV patients have a high viral load and are likely to develop more fatigue. Energy is regained after starting HIV treatment.
  • HIV treatment as a cause of fatigue: Anti-HIV drugs help to improve energy levels in patients, but they lead to fatigue as well. These medicines include a lot of side effects such as tiredness and excess fatigue. These side effects are more common in the early phase of treatment.

In order to avoid this fatigue, you may change your mode of treatment or consult your doctor for a change in your medical prescription. Medicines such as cotrimoxazole, dapsone, pyrimethamine, toxoplasmosis and ganciclovir lead to fatigue. Folic acid can be an alternative. Blood transfusion can also be conducted to treat fatigue due to medicines, which produces a quick burst of red blood cells. Injecting a hormone named erythropoietin also stimulates red blood cell production.

  • Low vitamin and mineral levels: Low levels of vitamins and minerals in the body cause fatigue. You can avoid fatigue of this kind by choosing a balanced diet by consulting a dietician. Changing your diet and taking dietary supplements helps in reducing fatigue. Fatigue may also be caused because of anaemia.
  • Sleep disorders and stress: Fatigue is bound to develop in HIV patients who suffer from sleeplessness. Insomnia, the most common sleeping disorder, occurs among HIV patients. It happens due to stress, anxiety and depression. Improving upon your sleep ensures correction of the fatigue to a great extent. You can take up counselling or therapies to reduce any kind of sleeping disorder due to stress. You should avoid consuming excess of coffee, alcohol and several recreational drugs. You must also change your lifestyle and daily routine, aiming to be more productive.

Fatigue and tiredness are common among all HIV patients, which occur because of various reasons. There are remedies to get over the fatigue which should be implemented correctly.
