I am a 74 year man with diabetes since 25 years and it is under control by medicines and insulin injections. If I make long travels by car, I want to urgently use toilets on the way for passing stools. If I could not see a toilet I usually passes in my underwear and pants while in the car itself. It has become a regular habit now resulting in difficult situations. Kindly advise whether it is due to irritable bowel syndrome and what is the remedial measure I can follow before or in a journey. When the urge comes I cannot control it.

There are many potential causes of fecal incontinence, including: diarrhea (possibly due to an infection or ibs) impacted stool (due to severe constipation, often seen in people of your age group) inflammatory bowel disease (crohn's disease or ulcerative colitis) nerve damage (due to diabetes or spinal cord injury) more than one cause for fecal incontinence is a strong possibility. Alot of cases of bowel incontinence, occur without a clear cause. First things first is to figure out the cause, if at all possible. This is a process, no single simple test will be able to figure it out. You will need to see a gastroenterologist and discuss your entire medical history. During a physical examination, a the gi may check the strength of the anal sphincter muscle using a gloved finger inserted into the rectum. Some of the tests that may be needed for figuring this out are: stool testing endoscopy anorectal manometry. A pressure monitor is inserted into the anus and rectum. This allows measurement of the strength of the sphincter muscles. Endosonography. An ultrasound probe is inserted into the anus. This produces images that can help identify problems in the anal and rectal walls. Nerve tests. These tests measure the responsiveness of the nerves controlling the sphincter muscles. They can detect nerve damage that can cause bowel incontinence. Let me try and give you an overview of what kind of treatment options you have. Recommended treatments vary according to the cause of course. Often, more than one treatment method may be required to control symptoms. Diet changes may be helpful: eat 20 to 30 grams of fiber per day. This can make stool more bulky and easier to control. Avoid caffeine. This may help prevent diarrhea. Drink several glasses of water each day. This can prevent constipation. Medications. Try these medicines to reduce the number of bowel movements and the urge to move the bowels: imodium lomotil hyoscyamine methylcellulose can help make liquid stool more solid and easier to control. For people with a specific cause of diarrhea, such as inflammatory bowel syndrome, other medications may also help. Exercises. Begin a program of regularly contracting the muscles used to control urinary flow (kegel exercises). This builds strength in the pelvic muscles and may help reduce bowel incontinence. Bowel training. Schedule bowel movements at the same times each day. This can help prevent accidents in between. Surgery may be recommended for people whose bowel incontinence is not helped by noninvasive treatments. There are many different surgeries that can be done. But I don; t want to get into this unless we know its really necessary. I strongly suggest you look into instituting the dietary changes mentioned above, after that medication may be needed. But above all, you need to be in touch with a good gi. I hope this helps. Feel free to follow-up and I will do my best to talk you through all this.

I haven't been able to sleep since 2 days and I am having a severe headache, tried multiple home remedies, but didn't work, what to do? Please help me!

Lack of sleep may be the cause of your headache, and headache may be the cause of your insomnia. This is a vicious circle. Avoid sleeping in the afternoon. Do not take any caffeine after 7 pm. Have a light meal for dinner at least 2 hours before retiring. Have a warm bath before going to bed. Drink warm milk at bed time. Listen to soft soothing music before retiring. Meditate for 15 mins before sleeping. If need be try shirodhara (ayurvedic therapeutic massage) along with ayurvedic medications.

I am not feeling well from last one month feeling so weak and I take a healthy dite and consult to many doctors but they are not capable to give any suggestions can you help me?

Dear, if you tired very fast and feeling weakness. Then you will definitely recover from weakness by following these very useful tips. * you should take proper balanced diet first. That means you have to take vegetables, milk, chapattis or rice, butter / cream, cereals (daals) or cheese every day. Very important thing, do not miss or skip your meals at any cost and take meals on time. * proper 6-8 hours sleep is very important. If your sleep is less than 6 hours, then you feel tired very early. So complete your sleep. * do breathing execises daily in morning & evening. Just sit down in peaceful place, close your eyes, take deep and long breaths in and out. Do it atleast for 10-15 minutes daily. You can also do pranayams like anulom vilom for better results. *take a walk, in first walk normally for 10-15 minutes for 15 days. You can do it on treadmill also. * you can take ashvagandha churn of baidyanath 1 teaspoon with milk daily for 2 months atleast to get your strength back. * check out your haemoglobin level in 3 months, if it is low you get tired early. So get treatment of low haemoglobin (anemia). If you have any disease like asthma /diabetes / hypertension, then get complete treatment of them. * avoid all fast foods and habit of smoking or drinking. You surely get great strength and recover from weakness by following these important things.

Hi My period is always on time never it has com late till now. This time I have took ipill couple of days before my period (around 3-5days before) after 11 hrs of sex Will it delay my period? Does these ipill works effectively?

It can delay periods upto a week. If more than I would advise a urine pregnancy test. Yes usually i-pills are effective.

Hello doctor my uterus was removed and just it took before 3 weeks but after that there was a very severe pain, so when we started scanning we found that there are 3 milligrams of stones in kidney, so what we can do and what r the foods I can have to remove it? Can you suggest me please?

Hi, First and foremost thing is, drink 3-4 litres of water per daysothat it helps to flush out small stones along with urine. Use banana plant stem as a vegetable 3-4 times a week which helps to breakup and dissolve the kidney stones. Drink tender coconut and lemon juice everyday. Avoid tomatoes, spinach, cabbage, brinjat, cucumber, groundnuts, cahew, etc. food which are rich source of calcium salts. Take cap Renali 1 cap 3 times a daycontinuously for 3 months.

Dear lybrate-user, gentle massage over the affected part with lukewarm herbal pain relieving oil for 20 minutes followed by hot fomentation. It will relax the muscles and the veins of your affected part improving the blood supply to the organ and giving strength to it. Please avoid over-use of the part affected i. E. Lifting weight - pushing or pulling heavy objects. Give proper rest to the part - let it heal with the time. Also drink hot glass of milk while going to bed at night - this will supplement natural calcium to your aching muscles. This will greatly help.

Sir I am 27 years unmarried boy, I have sugar in urine not in blood, my main problems are -urine frequently, early morning back pain, erection these are the problems I am facing from last 4 years. How can I get away form sleeping pills? I consume 0.5 mg sleeping pill every day and still have no sound and satisfying sleep. Please help me in getting away from the sleeping pills and have at least sound sleep.

Dear, These are some instructions & methods for good sleep: • Frequent oil massage of head with oil relax your mind and enhances sleep. You can also do foot massage daily it destress you. • Do not take stress. Do meditation and long breathing pranayams to relax the mind. • Do not take caffeine, tea & alcohol in excess and these are stimulant,(activate) your brain result in lack of sleep. • Avoid drinking alcohol & smoking. • Avoid sleep during day time as it makes it harder to fall asleep at night. •Try to sleep early at night. Do not think during sleep. Do exercise and long walk daily. • Avoid to take sleeping pills because these are habit forming drugs and these drugs disturb your normal sleep pattern. • Take a bowl of curd daily in afternoon and one glass of milk at night daily.

Hello, weight reduction is mainly due to improper digestion. Try these ayurvedic tips- a) improve your digestion with help of ayurvedic medicines b) practice pawana muktasana daily which will improve the digestion and increase the weight side by side c) avoid late night sessions and go to bed before 10 pm d) wake up before 6: 30 am e) avoid junk and spicy diet f) increase sweet and dairy products in your meals.

Hello. Thanks for the query. Simple answer yes; you can win over anxiety & panic attacks without medications also! but the caveat is its a long & tedious process of special counselling called as behavior therapy or cognitive behavior therapy (cbt). Its similar to reaching a place by bus versus reaching a place with a bullock-cart! so, if you wish to change medications (escit plus) only because of reduced sex drive, low energy etc. There are better medications available with less side effects too. Think over it & revert back if you need counselling advice or medications. Good luck.

Stress level is very less. Diet is also well maintained and healthy. Face acidity problem from last year. No genetic problem of baldness. Will be happy if you provide solution. It is normal to lose about 30-80 strands of hair in a day. The common reasons behind hair fall are lack of essential nutrients in the body, improper and unhealthy dietary habits, hormonal imbalance, certain medications, dandruff, alopecia and other scalp disorders, stress and tension etc. Natural oils such as coconut, olive, jojoba, almond etc. Can be used to massage the scalp for countering hair fall. Gently massage the scalp prior to head bath twice a week. Also check hemoglobin for anaemia and thyroid status with serum tsh. Hair fall may be attributed to different reasons like using artificial colors, hair styling, chemicals etc. It may be due to some internal weakness also. Please go for herbal neem shampoo. Use shikakai and amla as conditioner (soak amla and shikakai in water overnight and boil a little in morning). Apply it on hairs. Keep for half an hour and then wash. It will condition the hairs.
