Coccydynia (Tailbone Pain)- Physiotherapy Treatment Approach

We all have lost our tail with evolution…but did you ever think that the vestigial (redundant) part of that tail which has been left over in us in the form of tailbone can mess up with our daily activities and torture us to the level of not allowing us to sit!

If you are not able to sit or if you are experiencing pain when sitting right at the tail bone region, you may be having ‘Tailbone Pain or Coccydynia’.

What is Coccydynia?

The coccyx, also referred to as the ‘tailbone’ is the last segment of one’s vertebral column. Coccydynia is referred to a disabling pain that occurs in/around the coccyx. This type of pain usually starts when one sits all of a sudden or rises up from the seat after having been seated over a long period of time. Also known as coccygodynia, this condition can mar one’s quality of life. The pain, often described as ‘stabbing’ or ‘piercing’, can radiate to the buttocks, the lumbar spine, and rarely, to one’s thighs.

Anatomy

The coccyx is the final part of one’s vertebral column. The vertebral units are fused together. The frontal portion of the coccyx is the fusion site of the muscles and ligaments which control the functions of the pelvic floor. The coccyx also supports the anus’s position. The gluteus maximus is attached to the posterior part of the coccyx. Ligament or muscle damage, or muscle weakness can cause the coccyx to assume an abnormal position, thus causing pain.

Common Causes:

  1. Sitting for extended periods of time, especially on hard surfaces

  2. Effect of a direct trauma, such as a fall

  3. Giving birth

  4. Bone fracture, tumors or an infection can also cause this condition

How do you know whether it’s Coccydynia?

  1. You will experience pain while sitting, especially on hard surfaces

  2. Localized pain in or around the tailbone that worsens with touch or any pressure applied on it

  3. The pain will become severe when you stand from a seating position after considerable amount of time

  4. The pain might also start during sexual intercourse

  5. The pain increases in women during their menstrual cycles

Diagnosis

  1. Local Coccydynia

    • Traumatic

    • Idiopathic

  2. Radiated or referred Coccydynia
  3. Psychogenic Coccydynia

  4. Chronic proctalgia

Physiotherapy Approach For Treating Coccydynia

Patients diagnosed with Coccydynia are advised to avoid factors which might provoke the pain. The initial line of treatment will include making certain adjustments such as applying gel cushion when one is sitting for extended periods of time. This helps reduce localized pressure and improve his posture. Other modes include:

  1. Mobilizations: This can help realign the posture of the coccyx. Initially, there might be tenderness; hence, it is advised to start with rotational mobilization. To start with, either side should be mobilized first.

  2. Manipulation: This can be done intra-rectal and when the patient is lying in a lateral position. The coccyx is repeatedly extended and flexed with the help of the index finger. However, care must be taken so as to not hurt the rectal mucosa.

  3. Massage: Massaging the coccygeus muscles has also been proven to relieve pain. Biofeedback can also be integrated with it in order to avoid incidences of muscle pull on the coccyx. If you wish to discuss about any specific problem, you can consult a physiotherapist.

Castor oil: • helps with hair regrowth and hair loss, • makes the hair richer and darker, • prevents split ends, • treats dandruff and dry scalp, • moisturizes the hair and makes it shine, • works in an antifungal and antibacterial way and can prevent problems such as ringworm, and folliculitis. How to apply castor oil It’s best to use castor oil as a hair mask. This is particularly helpful if you’re trying to regrow hair or give it more moist and shine. 1. Castor oil has a thick consistency, so you can mix it with coconut oil, olive oil or jojoba oil in a half and half ratio. This will make the mask easier to distribute throughout your hair. 2. Use your fingers or a brush and start applying it onto your scalp with circular motions. This gentle massage increases the blood circulation, rejuvenates the hair, and stimulates hair growth. 3. Part hair into sections and apply the oil (mixture) to each section until all your hair is covered properly and evenly. 4. Wrap your hair in a warm towel (you can also use a shower cap) and leave it for two to eight hours. You can also leave it over night. This will give the oil enough time to penetrate the follicles and hydrate your scalp and hair thoroughly. 5. Wash your hair as usual using shampoo and conditioner. Make sure you rinse all the oil out of your hair. How often should you use castor oil? This depends on your objective and type of hair. If your aim is to treat hair loss, you will need to do it at least twice weekly, but even better if you can do it three or four times per week for several weeks. For split ends and increased shine, you can use the oil as a conditioner once weekly. Be careful not to use too much of it (a pea-sized amount should do) or you can end up with an over-oily scalp. For best results use unrefined castor oil, as it contains more nutrients and healing properties.

It could be due to cervical spondylosis this is quite a common condition rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Any way take ibuprofen 200mg od & sos x 5days do neck & back (spine) exercises contact me again if need be. Use of cervical collar will give you relief. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take you will need other supportive medicines also. I will suggest her to wash your hair daily with lux shampoo and apply hair oil daily and take early morning sunlight for vitaminD and Few diet tips - Not to eat fried food items and Can eat All green vegetables, moong , chana , Dal , rice etc And to apply ghee over chappati and don't apply oil over chappati And to drink milk mixed with turmeric powder twice a day And If possible eat eggs on alternate days And If possible in breakfast eat atleast one bajra ka rotla And Eat seasonal fruit

The hand is one of the most commonly used parts of our body and is susceptible to damage, wear and tear as well as injury. Pain in the hand can be attributed to numerous causes, including damage and other conditions that can affect the joints, cartilage, nerves and other parts of the hand. Causes:

De Quervain's tendinitis: This is also known as de Quervain's tendinosis. It causes pain on the thumb side of the wrist. The pain may grow slowly or suddenly. It can also spread to every part of the thumb and up the lower arm. On the off chance that you have de Quervain's tendinitis, you may experience pain while performing the following actions:

  • Clenching The fist
  • Handling on or holding objects
  • Turning the wrist

Treatment for this includes the following:

  • Wearing a prop to rest the thumb and wrist
  • Calming drugs
  • Cortisone infusions

Carpal tunnel disorder: This is common amongst the well-known nerve issues. It can cause pain in the following parts of the hand:

  • Palm and a few fingers
  • Wrist
  • Lower arm

The treatment methods includes the following:

  • Resting the hand and wrist
  • Painkillers
  • Wrist support
  • Steroid infusions
  • Active recuperation
  • Breaks

Fracture: It's a crack or break in a bone that can cause immense pain in the hand. Other than this, the hand may also experience the following:

  • Stiffness
  • Swelling
  • Loss of proper motion and movement

The kind of treatment depends on the kind of fracture. Braces are usually used for small fractures. Sticks, wires, or plates might be used to treat severe fractures. Likewise, surgery may also be recommended to set the broken bone properly.

  • Center joint of one or more fingers

The treatment depends on the seriousness of the injury and the following may be used:

  • Painkillers
  • Props for the fingers or wrist
  • Heat
  • Non-intrusive treatment

Trigger finger: Also known as stenosing tenosynovitis, this defect causes the fingers or the thumb to bend. It can be difficult, particularly when bowing or fixing the influenced finger or thumb.

Hello, There are different types of LASER Therapies available in the market for hair removal. It beams highly concentrated light into hair follicles. Pigment in the follicles absorb the light. That destroys the hair. Lasers are useful for removing unwanted hair from the face, leg, arm, underarm, bikini line, and other areas. Lasers can selectively target dark, coarse hairs while leaving the surrounding skin undamaged. Small areas such as the upper lip can be treated in less than a minute, and large areas, such as the back or legs, may take up to an hour. Most patients have permanent hair loss after an average of three to seven sessions. And later in few cases, maintenance sittings are also needed in future. If you are planning on undergoing laser hair removal, you should limit plucking, waxing, and electrolysis for six weeks before treatment. That's because the laser targets the hairs' roots, which are temporarily removed by waxing or plucking. You should also avoid sun exposure for six weeks before and after treatment. Sun exposure makes laser hair removal less effective and makes complications after treatment more likely. The rate varies from place to place and area to be treated (e.g. Rs. 500/ sitting for upper lips).

Yesterday I jumped over 5 ft I injured I did MRI and the report is as below (1) complete anterior cruciate ligament tear at its femoral attachment with loss of normal alignment (2) medial meniscus tear with mild extrusion Spain of medial collateral ligament and moderate joint effusion (3) marrow contusion in posterior aspect of lateral plateau I am very worried what is the solution please tell me if the operation is needed or not.

Hi! Seems that you have injured yourself a lot. Ligament injury is graded into 3 degrees. 1st is complete tear of the ligament, which solution is Surgery. 2nd and 3rd grade injuries are partial tears which can be managed with immobilisation of the involved joint, no weight bearing, rest, Cold packs and definetely PHYSIOTHERAPY. No other alternative other than exercises. Meet an orthopaedician to the earliest.

How to stop dandruff and hair fall specially in winter. I also have a gray hair so give me some tips for hair.
Start taking black til (seasam) with a glass of lukewarm milk early morning. Avoid pickles, spices, oily, greesy food items. Start avle ka murabba. Also have a high protien rich diet e. G. Pulses, dry fruits, black gram, ground nut etc. Also oil your hair with good herbal oil and wash with herbal shamppoo - avoid shampoos with high acid contents. As far as ayurveda's therapy is concerned - start with shirodhara with herbal oil and nasyam therapy for atleast 15 days. This will greatly help. Do gentle massage with herbal hair oil daily night for 15 minutes. Although some herbal medicines might also help you but after proper history taking. Please update with your valuable feedback.

Sir my age is 23. Height 6 ft. Weight 76 kg. Sir I had habit of masturbating 5 times daily since 5 years during phone sex by rubbing penis against bed for hours. I was enjoying my masturbating habit. I had started it at the age of 17. But sir during mid half of 22 .8 month ago .one day I felt severe pain in the left half head back side .a shooting pain. From.there it goes to whole head. I was in a severe pain those days at single point on half head. I went to an neurologist .he ordered CT scan of head. It was shown some sinus in my head. Doctor says it is normal. At that time my bp was 150/100. That time I left habit of masturbation but used to have night fall daily basis. After 2 month when I was doing yoga .one day I had frequent urination upto 30 times and weight loss on third day. Sugar was tested it was normal. So I had urinary tract infection. I had taken medicine for it but it was reoccurring. And also I started feeling bones and muscle pain in whole body. And numbness and tingling im whole body along with light abdominal pain. So I had my uric acid test .it was normal. But my urgency to pass urine was more frequent and dark colored reddish urine. again I had urine culture it was normal. But infection was with 4-5 pus cell. So I had ultrasound of abdomen and prostate .it came mild prostatomegaly 22. 6 cc and right renal cortical cyst. I had MRI plain of abdomen. It also came with right renal cortical cyst. But my problems of numbness and tingling in whole body increasing day by day. I am having excessive hair growth on whole body even on eyebrows and head but retarded growth on moustache. I am having more bone and muscle pain including mouth along with inflammation in body. Chest pain. Chest and abdomen enlargement too. Much.abdomen is looking too bad. Since 8 month my appetite is reduced. When I was well I had 7 chappati at a time. Now from 8 month I am having 2 chappati at time. But still I am having chest and abdomen enlargement. Along with hair growth on whole body .with new hair growth. Sir I had my total blood testosterone it came 2.21 ng/ml. 2 month ago and cortisol 234ng/ml. FSH was 6.28 miu/ml. So in mid I took glginseng and shilajeet and daily intake of 1 kg mangoes. After 2 month I again had my test testosterone came 3.73ng/ml. Cortisol 153ng/ml. Prolactin 11.28 ng/ml. Tsh is 1.34. .but sir still I am having problems. My bp is 150/100 and sugar is lowering 66 after eating. Sir I am also noticing that my testicle size is reducing. They are now 2 cm very small. Sir my abdomen pain when I sit and when I drink and eat something. And left half head shooting pain. It pains a lot. Sir I think some hormonal problem is in my body. Sir I think I had some type of adrenal tumour or pituitary tumour. I can not distinguish because e I am not a doctor but you can help me better. Sir can you tell me where is the problem?

Hi. Don't worry you are not suffering from adrenal tumour and all. Take thuja 1m 4pills once only do not repeat it from next day start taking following treatment sabal ser q 10 drops 3 times 5phos 4pills 3 times staphy 30 4pills 3 times for 8 days and revert back for further treatment. Do not continue same medicines more than 8 days.

Hi. Hyperhidrosis is a condition characterized by abnormally increased sweating, in excess of that required for regulation of body temperature. It can be associated with a significant quality of life burden from a psychological, emotional, and social perspective. Hyperhidrosis can either be generalized, or localized to specific parts of the body. Hands, feet, armpits, and the groin area are among the most active regions of perspiration due to the high number of sweat glands in these areas. When excessive sweating is localized (e.g. Palms, soles, face, underarms, scalp) it is referred to as primary hyperhidrosis or focal hyperhidrosis. Excessive sweating involving the whole body is termed generalized hyperhidrosis or secondary hyperhidrosis. It is usually the result of some other, underlying condition. The cause of primary hyperhidrosis is unknown, although some physicians claim it is caused by overactivity of the sympathetic nervous system. Anxiety or excitement can exacerbate the condition for many sufferers. A common complaint of patients is they get nervous because they sweat, then sweat more because they are nervous. Other factors can play a role, including certain foods and drinks, nicotine, coffee-intake, caffeine, and smells. Overweight people sweat more profusely than normal-weight individuals because fat acts as an insulator that raises core temperature. Some things are in your control when it comes to sweating. Not all excessive sweating is weight-related. But even if your sweating has another cause, getting to a healthy weight is a good idea. Other causes are: A hot and humid environment and exercise are the most obvious causes of sweating. Additional factors that contribute to excessive sweating include alcohol, caffeine, medications and spicy foods. Conditions such as fever, menopause, overactive thyroid, low blood sugar and cancer can lead to excessive sweating. Emotional triggers such as stress, anxiety, anger, fear and embarrassment can contribute to this condition. Diets that include spicy food also might have lots of calories and fat, contributing to both extra pounds and excessive sweating. Being overweight often leads to low self-esteem and lack of confidence in social situations, which can lead to higher stress levels and more sweating. Losing weight can help correct the emotional affects and higher sweat levels. Don't assume your excessive sweating is attributable solely to excess weight. Sweating can be a sign of medical problem, so get yourself thoroughly examined by a doctor with required investigations to be done. Medication: Take homoeopathic medicine- Silicea 30 and Bacopa Monierri 1x - both thrice daily for 4 weeks. For white hair: We can't change the gray hair turn black, but of course, prevent further greying! The causes of premature graying of hair are several. Here we have listed out a few common causes. Heredity is a major reason for hair greying Excessive use of tobacco and smoking Chronic emotional stress or psychological worries Lack of nutrients such as vitamins, iron, copper and iodine in the diet Medical conditions such as anemia, hypothyroidism, tuberous sclerosis or neurofibromatosis Home Remedies: 1. Add the curry leaves to the oil and boil it for 6 to 8 minutes, or till the leaves turn black. Remove from flame and let it cool. Strain the oil in a container and massage your scalp and hair with this oil 3 to 4 times per week before going to bed. Wash off with a mild herbal shampoo in the morning. 2. Turn the fenugreek seeds to a smooth powder and keep aside. Heat the oil and add the amla pieces to it, let it boil for 5 to 6 minutes, add the fenugreek powder to the oil, let it boil for another minute and remove from flame. Let it cool and strain the oil, apply on scalp and hair before going to bed. Wash off with a mild herbal shampoo in the morning. 3. Mix onion and lemon juice in a glass bowl and apply on scalp and hair evenly. Leave it on for 30 minutes and wash off with a mild herbal shampoo. Repeat it 4 to 5 times a week for 2 weeks. Medication: Take homoeopathic medicine – Schwabe's Acid Phos. 200/ once daily and Weisbaden 200/ twice daily – all of them for 1 month. And a multivitamin - A to Z/ 1 cap. Daily once for 3 months (as deficiency of zinc, copper and selenium can also cause premature greying). Use Schwabe's Arnica Shampoo and Hair Growth Oil. Revert.

You should take Acupressure therapy and take Biochemic Calc flour 6 x first week &second week Sillicia 12x 4 tab each thrice a day with warm water and take it 5 days than consult private online. And take alternate week.

Hi Apply pranacharya restopain oil or prasarini oil on your affected part then give hot fomentation. Take maha rasnadi kwath 2-2 tsf twice a day. And agni tundi vati and maha yograj guggul 1-1 tab twice a day.
