Cholesterol level should be measured at least once every five years in everyone over age 20. The screening test that is usually performed is a blood test called a lipid profile. Experts recommend that men ages 35 and older and women ages 45 and older be more frequently screened for lipid disorders. The lipoprotein profile includes: Total cholesetrol LDL (low-density lipoprotein CHOLESETROL also called "bad"CHOLESTETROL HDL (high-density lipoprotein cholesterol, also called "good" cholesterol) TRIGLYCERIDES (fats carried in the BLOOD from the food we eat. Excess calories, alcohol, or sugar in the body are converted into TRIGLYCERIDES and stored in fat cells throughout the body. High triglycerides and cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Change life style .do meditation, yoga, Go walk daily, do any one physically activity daily i.e.play any game., Avoid fast foods, oily foods, junk foods, alcohol, smoking, fatty foods, non-veg.,avoid stress, take plenty of water in day time. And 1 glass of water before bed at night. use flax oil, omega-3 fatty meal or cap., take .green veg., whole grain. GARLIC,. Cod liver oil. Take anti cholesterol medicines . Cap. First begin with diet change and exercise. Reduce sugar, fried, fatty food and ghee from diet, change your eating pattern. Take a detailed diet consult. For exercise start gradually with brisk walking, yoga or jogging and target to increase it to 40 mins daily. Quit smoking. Reduce alcohol. Reapeat lipid profile after 6 weeks of starting this, if cholesterol is still deranged, consult for medications. Also given your age, get a Stress TMT done to have a baseline cardiac status.

The difference between two arms should not be more than 10 mm, Below 10 mm is considered as normal physiological. More than 20 is abnormal and suggests some abnormality/obstroction in vascular system. Between 10 and 20 should be assessed clinically.

High Cholesterol Level - Can It Cause Carotid Disease?

MCh, MBBS, MRCS (UK)
Neurosurgeon, Chennai
High Cholesterol Level - Can It Cause Carotid Disease?

Carotid artery disease occurs when an oily, waxy substance called plaque builds up inside the carotid arteries. These are arteries that supply oxygenated blood to your brain, face, scalp, and neck.
Carotid artery disease is very serious as it can cause a brain stroke. It, in fact, starts with damage to the inner layer of the carotid artery. To stem this damage your body starts a healing process and this leads to a build up of plaque at the point where the artery is damaged.

When this plaque ruptures, platelets are rushed in and these create an obstruction to the flow of blood to the brain. When the blood supply to the brain is cut off, which happens in carotid disease, the brain cells begin to die causing a stroke. This neural death impairs the parts of the body that these brain cells control. So, you can get paralyzed, become speech or vision impaired, have long –term effects like paralysis or even die due to a stroke.

Causes of carotid disease

  1. Smoking
  2. High cholesterol levels
  3. High blood pressure
  4. High levels of sugar in the blood due to diabetes

What’s even more worrisome is that carotid artery disease may not cause any signs or symptoms until the carotid arteries are severely narrowed or blocked. And what usually happens is that a stroke becomes the first sign of the disease.

How scary is that?
So, the next question that comes to the mind is - Can this condition be cured? Let’s learn more about management and prevention of the same-

Prevention
Lifestyle changes are key. Since hypertension, uncontrolled diabetes, sedentary lifestyles and imbibing too much fat are all risk factors, taking care of them minimizes the risk of carotid disease and strokes. Heart healthy lifestyle changes that include moderate exercise are a great way to prevent carotid disease.

Treatments
Treatments for carotid artery disease include:

Medicines: Sometimes lifestyle changes do not stem the rot. You need medicinal help. Doctors usually prescribe statin medications to control your cholesterol levels. These lower the bad cholesterol that can narrow your carotid arteries and cause brain strokes and damage. Statins are usually prescribed for people who have:

  • Heart disease
  • Those who’ve had a stroke
  • Diabetes
  • High LDL or bad cholesterol levels

Medicines like Aspirin and Clopidogrel to prevent blood clots are the mainstay treatment if you suffer from carotid disease. They prevent platelets from clumping together inside your carotid arteries and forming blood clots. Apart from these, your doctor may also medicate you for conditions that can damage your carotid arteries and cause strokes. These medicines are for lowering your BP, blood sugar levels, preventing the formation of blood clots anywhere in your body including inside your heart, and preventing and reducing inflammation.

Medical Procedures
These are used to open narrowed or blocked carotid arteries.
The two common procedures are:
Carotid endarterectomy: It’s mainly for people whose carotid arteries are blocked 50 percent or more. In this, the inner lining of the carotid artery that is blocking blood flow is removed.
Carotid artery angioplasty and stenting: Here, doctors use a procedure called angioplasty to widen the blocked carotid arteries and restore blood flow to the brain. A stent is inserted into your carotid artery to broaden the lumen.

She can eat healthy food without starving. No fats or sugars , lots of salads minus salt , fresh fruits , a glass of toned milk & curd , an egg (if she can take ) Remember dieting after a certain age is difficult because one cannot exercise as much as a young lady , secondly digestion gets weak with time , bones hollow due to less calcium. Something advised for young may not work and be practical for her , but oats as you have mentioned she is taking are good for all age groups. She may not loose weight so quickly and it is not wise to wait for heart valve surgery ,it should be done at the earliest. simultaneosly make dieting a habit for her.

Hello lybrate user, the pain that you are having in the upper part of the abdomen and the chest area-could be due to acute gastritis which basically means that you have got hyperacidity. If the symptoms are also associated with constipation, then the probability of acute gastritis even becomes stronger. You don't need any tests but you can make a few lifestyle changes. You should not take spicy or oily foods which is kind of important because you don't have a gallbladder. You should try to take fruits at least once in a day. And most importantly you should not lie down immediately after taking food. There is also help available in the form of medicines. Simpy try anti acidity medicines if nothing works.

One blockage can be cleared by baloon therapy(angiography) Merits are that there is no need to do heart surgery + it serves as a warning for rest of life to take care of heart by eating well , exercise and taking stress in your stride with no negative feelings. Good Luck. Bypass surgery should be done if there are Diffuse Blockages all over the heart / blockages are more than 2 , because getting fitted with more stent is not beneficial in the long run compared to one time heart surgery , Aftercare after 2 months is same in both.
