Cervical Cerclage (Cervical Stitch Operation)

MBBS, DGO, MD - Obstetrics & Gynaecology, MRCOG

Cervical Cerclage (Cervical Stitch Operation)

Cervical Cerclage (Cervical Stitch Operation)

This is also called as cervical stitch operation. The cervix is the mouth or entry point of the uterus which extends into the vagina.

It is advised in cases where a woman may have a previous second-trimester pregnancy loss or some procedure done on the cervix earlier leading to weakness of the cervix. This weakness may lead to a lax birth passage through which a premature baby can slip out accidently, causing a miscarriage or premature birth.

What is it?

It is an operation done to surgically close the cervix so that the baby does not slip out through the open passage. A small suture is passed through the cervix and tied similar to the way one would tie a filled balloon. This stitch is expected to hold the pregnancy and the baby securely inside till we remove the stitch near delivery.

When should it be done?

An emergency stitch may have to be applied anytime during pregnancy when your doctor feels that the cervix is showing signs of opening. These signs may appear on ultrasound scan or while examining your cervix.

An elective procedure is usually done between 12-14 weeks of pregnancy.

What should I expect before the surgery?

You would be admitted to the hospital and put on some intravenous medication to quieten the uterus. Antibiotics may be started on the basis of your vaginal culture reports.

Surgery is usually performed under general anesthesia and takes around 30 minutes.

What happens after the surgery?

Slight bleeding and cramps in the stomach may happen after the surgery and you would be given medications to make you comfortable.

One usually needs to stay in the hospital for one or two days after the surgery to look for signs of miscarriage or any other complication.

Once you go home you should avoid exertion, vaginal intercourse and lift heavy weights( not more than 5-8 kgs) for the rest of the pregnancy.

Please inform your doctor immediately if you have any excessive discharge from the vagina/ fever/ pain in your belly or any other discomfort.

You must follow your doctor's advice on regular checkups and other investigations during the rest of the pregnancy.

When do we remove the stitch?

The stitch remains in place for the rest of the pregnancy till around 37 weeks. It may have to be removed earlier if you start your contractions or go into preterm labor.

The stitch is usually removed without any anesthesia but if there is any problem in its removal because of its position or extra tissue growing over it, then a short anesthesia may be required to cut it.

Top 5 Recommended Exercises To Get In Shape Quickly

Top 5 Recommended Exercises To Get In Shape Quickly
Getting bulges on the wrong places is common as you grow. Waist line stretching, jiggling arms, fat on thighs and unwanted tyres on your torso keep chasing you. Get fitter, toned and chiselled. Deal with all such issues by learning these tone-up moves.

1) Push Ups - The best of all. They target all muscles, from your arms to your abs to your legs and the butt. If they feel difficult at first, go on to your knees. But once you feel strong, go ahead and do the full ones. Push-ups should ideally be done as many as you can!

2) Squats - A simple squat exercise can take care of all the fat accumulated at your bottom. For a squat, stand hip width apart go down to your knees, make sure that the knees don't come ahead of your ankle. Then, move up slowly. Repeat.

3) Crossover Lunge - Arms at shoulder height with palms down. Now, step with one foot forward, crossing the other. Twist the torso. This is a cross-over lunge. It will help your oblique, sides bulges and legs all at the same time.

4) Bench Dips - Take a car or a bench. Place your hands at hip width apart and move forward. Now, press yourself up and down. Bench Dips are excellent for toning your arms.

5) Plank - Simple but great for your core. Plank requires you to stay in the first part of a push up position. Make sure that everything, from your head to your ankle are in a straight line. Suck the stomach in. Don't let your hips sag.

Make sure that you also include a healthier, balanced meal as a part of lifestyle to help your body get fitter.

If you have any further questions on fitness, click on consult privately to ask me.

Yes, it's safe. In fact, it's unsafe not to take thyroid medication during pregnancy if you need it. Whether you have hypothyroidism (an underactive thyroid) or the much less common hyperthyroidism (an overactive thyroid), you'll probably need to take medication and be closely monitored. During pregnancy, mild hyperthyroidism, in which TSH is low but free T4 is normal, does not require treatment. More severe hyperthyroidism is treated with antithyroid medications, which act by interfering with thyroid hormone production. Radioactive iodine treatment is not an option for pregnant women because it can damage the fetal thyroid gland. Rarely, surgery to remove all or part of the thyroid gland is considered for women who cannot tolerate antithyroid medications. Antithyroid medications cross the placenta in small amounts and can decrease fetal thyroid hormone production, so the lowest possible dose should be used to avoid hypothyroidism in the baby. Antithyroid medications can cause side effects in some people, including allergic reactions such as rashes and itching a decrease in the number of white blood cells in the body, which can lower a person’s resistance to infection liver failure, in rare cases. Health care providers may prescribe PTU for the first trimester of pregnancy and switch to methimazole for the second and third trimesters. Some women are able to stop antithyroid medication therapy in the last 4 to 8 weeks of pregnancy due to the remission of hyperthyroidism that occurs during pregnancy. However, these women should continue to be monitored for recurrence of thyroid problems following delivery. Studies have shown that mothers taking antithyroid medications may safely breastfeed. However, they should take only moderate doses, less than 10−20 milligrams daily, of the antithyroid medication methimazole. Doses should be divided and taken after feedings, and the infants should be monitored for side effects.

Don't worry. Try to avoid smoking and tobacco. I usually advise couples to some after one year of marriage, unless they are very anxious. Don't feel bad. All the best. 6 month is not always sufficient time.

Got get a urine pregnancy kit test done for confirming pregnancy. You may also have an sexually acquired infection of vaginal tract if it's not pregnancy. Usually the discharge would be foul smelling in that case.

Yes excessive weight can be a major hurdle in conceiving. The hormones tend to be abnormal in people with excess weight especially in females. Please reduce weight in a healthy way first before trying to conceive.

Sometimes my breasts pains sometimes its all ok. Is there anything to worry about. Is there any chance of breast cancer.

DGO, MBBS
Sexologist,
Sometimes my breasts pains sometimes its all ok. Is there anything to worry about. Is there any chance of breast cancer.
Breast pain is a common problem if it occurs before periods, because it happens due to hormonal changes, however if the pain stays all through the month or if you can feel some lump/swelling then you may need to do ultrasound examination of breasts. Wearing a well fitting bra may help. Though chance of cancer is very low yet you have to do ultrasound to be sure.
