Hi I'm 36 years male suffering from ankylosing spondylitis since year 2007. Now I'm taking biological drugs i.e. *(Adalimumab) Exemptia* Injection for last 6 months (Took Exemptia 40 mg 6 injections and now taking Exemptia 20 mg (already completed 4 doses of 20 mg). I got married in May 2017 and now we want are planning for a baby. So is it right that we can conceive now? As I have a doubt that as I'm taking tnf blocker i.e Adalimumab so may affect the unborn fetus? So should we conceive now or wait?& also does Adalimumab damages the sperm?

No significant difference of sperm quality between healthy controls and anti-TNF treated patients with SpA, sperm abnormalities were also found in 102 healthy controls Sperm abnormalities are a common finding in healthy men, they are more pronounced in patients with active SpA. The sperm quality of patients with SpA with inactive disease receiving long-term TNF would like to advice you to under went IuI procedure to conceive.

It seems to be due to Calcium and vitamin D3 defeciency apply Nanofast local application twice a day for 3-5 days 2. Hot fomentation twice a day for 3 days do gentle massage of leg in upward direction avoid squatting avoid sitting for long time get Vitamin D and serum Calcium test done and review with report.

I am having pain in my knees for the last 2 months I have tried everything to get relief from this but nothing happen...
Knee joint is very complex joint made by 3 bones - femur, tibia, fibula and knee cap - patella. Several ligaments, cartilage and menisci are covered all around. So pain in knee joint may be due to several reasons. Normally after 50 plus age this disorder is very common because of arthritis. But in your case it seems there may be any internal injury like cartilage etc for which you are not aware about or pain may be due to deficiency of vit d or uric acid. So in general I would advise you to please go for proper investigation so that the reason can be diagnosed. In the mean time to get relief from pain and stiffness please follow the physio measures: precautions- 1. Avoid prolong standing. Standing on one leg or running for time being if its painful. 2. Take care while bending your knees, it should not directly strain your joint. 3. Avoid heavy weight lifting. 4. Wear shoes with good arch support, do not wear high -healed shoes. 5. Use the correct positions during activities so that you do not strain your muscles. 6. Try to change your sitting position every 15 min so that it should not give strain on your knees. 7. Avoid sitting on floor with both knees bending. 8. Take diet rich in calcium like milk, dairy products, banana etc which will enhance your calcium and gives you strength. Treatment - 1. Make use of hot packs and ice packs alternately. Apply with the ration of 3: 2 which means 3 min ice then 2 min heat. Ice has very good results in relieving pain than heat so combination of both gives you complete relax. As ice reduces pain and heat stimulates blood flow around the affected area. 2. After this using your thumb massage your knee area gently. Relax for 5 minutes. 3. Do some knee (quads) isometrics atleast 10-15 times with the hold of 5 -10 seconds. Also you can do some knee strengthening in the mean time like - hams sets, knee bending and straitening, heel raising, bridging, squatting, lunge etc. Lot of exercises are there. The above treatment will give pain relief and discomfort. If possible please contact any physio who will study your daily routine, symptoms, degree of pain etc and accordingly will give you the best treatment plan. Along with it give you our pain relieving modalities like ift, us vacuum therapy for 10-12 days with mobilisation exercises. Good luck and get well soon.

Thats a good question and it is a rare complaint. You need to do good activity either in a gym or under the guidance of a physiotherapist. Postural correction exercises are important to correct the disturbed posture. Swimming activities can also be done to improve the muscular flexibility however the leg length from hip or pelvis to lateral malleolus is less it is important to compare both the leg measurements and we have to correct the foot wear by raising the heel inches by 1-3 inches which will be helpful or else we can also add heel cushion which is available ready made in the surgical shops and it can be worn inside the shoes. This will definitely help you to improve the walking pattern ie. The gait pattern, later let us know after trying all these we will help you. avoid forward bending. do back exercise{ back stretching, while lying on back}, take balance diet. take milk and good quality protein. eggs, paneer, pulses. maintain your posture when ever working on computer. it will help you.

Rheumatoid arthritis is a chronic systemic inflammatory disorder that may affect many tissues and organs, but principally attacks the joints producing an inflammatory synovitis that often progresses to destruction of the articular cartilage and ankylosis of the joints.
