I am Dr Akhilesh Rathi I am practicing Orthopaedic and Orthoscopic surgeon in Dwarka.

Today I will be discussing about shoulder pain in the elderly, shoulder pain has been a very common problem and you must have heard that term frozen shoulder, heard similar kind of terms in your relatives or elderly people. So, these days very effective treatment for these kind of pain are available and because these diseases can be very debilitating. People are there who are unable to lift their arms to the routine course, so in these patients there are lot of treatment options are available if timely manage they can be done even in the non surgical way. So to start with the first and foremost treatment for most shoulder problems like the rotator cuff tendinosis and rotator cuff tear most patients can we manage with simple stretching exercises, range of motion exercises, shoulder strengthening exercises and some physiotherapy. So these patients recover well and they are able to manage their pain and regulate their routine with these exercises. Apart from these around 10 to 20% of the patients who do not improve with these exercises and are suffering from bigger tears or bigger problems, in these patients there highly advanced treatments are available nowadays. What we call as shoulder arthroscopy, arthroscopy means like you have must heard about laparoscopy where surgery is done in the abdomen through small portals. Nowadays, we can do surgery in the shoulder through small tiny incisions, 3 to 4 incisions and repair all the damage muscle in there. So even in those patients who are unable to lift their arm or move there arm or shoulder is stiff despite the physio, despite the exercises in those patients we can make it better. We can even think about having almost 100% normal kind of lifestyle. So I would like to restress is whenever you are having any shoulder pain, any stiff shoulder and it is not improving after 4 to 6 weeks then please do get it checked by an orthopaedician near you and most likely it will manage with physiotherapy and exercise and if it doesn't improve with physio, exercises I think you might need injection the shoulder or maybe orthoscopic surgery which can make it normal and which can repair the damaged muscle. Because if you delay too much, there are chances the damage muscle may retract and it may not be fixable even with the orthoscopy anymore. So the timing of your visit to your doctor is important, don't delay, don't ignore your shoulder pain, don't ignore your stiff shoulder. So because if you ignore it then you won't be able to operate and fix it arthroscopically, in those cases in which we done lake, the muscle has already retracted or is unrepairable then we need to replace the joint. So the best option is get yourself timing check, if your shoulder pain is not going in 4 or 6 weeks think about visiting your doctor and get it fixed and thank you all for listening to me patiently.

Today I will be talking about the knee pain in elderly, knee pain in elderly is very common issue and you must have seen people around you elderly people having a lot of knee issues. So the first and foremost issue, we get manage is simply by some lifestyle modification, exercises, some weight control all these things help a lot in making knee pain better. So quadriceps exercise are the key to any kind of knee pain, plus there are some sessions which we can take at with the physiotherapist which help in making the knee pain better, also a lot of supplements are available like glucosamine and many other are available which your doctor can prescribe you so that it can improve the cartilage damage and also prevent further damage. Apart from that you can also take about some injections which are available which are the hydraulic acid and other viscosupplements which are available for improving the knee pain and the cartilage quality. So most of the patients almost believe me 90 to 95% of the pateints can be managed with this simple measures. In those patient in home despite all the measures have mentioned patient is still having pain, he is still having difficulty walking, he is still having difficulty in doing his routine course we should think about other options. Usually if the age is around 66, 55, 60 we can think about knee replacement surgery and if the patient is of younger age we can think about other osteotomy surgeries like the white osteotomy. So knee replacement is a good option in these patients in home other measures like the conservative measures I have already discussed are not working. So the timing of knee replacement is very important considering the fact that we only change the joint and not the muscle. So if you are not walking for a significant period of time maybe a year or two because of the knee pain your muscles will go down and despite the knee replacement your surgery may not be a successful. So usually my indications for knee replacement, includes if you are walking distance going down you are not able to walk for 20 to 30 minutes and the stretch, your knees are bending and your xray is showing significant osteoarthritis grade 3 or grade 4 osteoarthritis, you should think about knee replacement. So to conclude I will like to say that knee problem have solution and most of the time you can manage it with simple measures like your weight control, exercises and lifestyle modification like avoiding squatting, sitting cross legs, avoiding for long standing and avoiding stairs. Also kindly note and never said not to walk, so even in severe pain maybe a day or two you can avoid walking but otherwise walking is good for your knees so don't cut it out and a timely done knee replacement can save you from lot of medical issues apart from saving your new problems.

I am 25 years old and pursuing my doctral degree. My MRI report says its an Grade 3 ACL tears and moderate knee joint effusion. I am not facing any knee instability, but the swelling on the knee persist and I feel pain occasionally. Since reconstruction surgery takes to recover nearly 6 months and above, I do not want a surgery and ask of you is there any alternative or surgery is must.

There is no conservative way to heel the ACL. U can try physio and quadriceps exercise which heals the pain and swelling to quite an extent. Yes if you do not have laxity and you do not have problem in running and stairs, then you can wait else surgery is the best choice.

Torn Cartilage - Is Surgery Required For It?

Torn or damage cartilage is not just a problem for the sportsmen. It can occur to anyone. It is generally caused due to a direct hit on the body parts. Its treatment is far more complicated than a simple bone injury. The best sign to know whether or not you have injured your cartilage is to check out your activities. If you have hurt your joint and don’t notice major symptoms, like if you are still capable of putting weight on and move the joint, you might have had a cartilage injury. It doesn’t hurt you a lot in the beginning, but might get worse later. 

Here are the initial care and self-treatment procedure: 

  1. Protection: It is the first thing that has to be kept in mind. You must protect the affected area from any further injury. Thus, providing an external support, for example, use a knee brace. 
  2. Rest: Provide complete rest to the affected joint. The rest is must during the first two or three days. Crutches may help the situation if you’ve got an injury around your knee or ankle. It is then advisable to return back to light activity over the next few days or weeks, as prescribed by the doctor. 
  3. Ice: It is best to apply an ice pack to the injured area for about 15 to 20 minutes in every two to three hours. This must be repeated in the first two to three days. 
  4. Compression: Use a bandage to provide the needed compression to the injured area to avoid any swelling and movement. This will not let things go beyond control. 
  5. Elevation: You must keep the injured area raised. Keep it supported on a pillow to reduce swelling. 

If there is an unbearable pain, take painkillers such as paracetamol or non-steroidal anti-inflammatory drugs (NSAIDs). If your condition doesn’t improve after the initial therapy, you must consult a doctor immediately. 

  1. Physiotherapy: It can be helpful in treating the affected joint. If there is any difficulty in moving the affected joint, you must opt for physiotherapy. A professional will be able to teach you exercises that will strengthen your muscles and provide support to the joint. This will help you in getting rid of the pain. It can be also beneficial at a time when you are recovering from any surgical procedure. 
  2. Surgery: If physiotherapy doesn’t work, it means you have severe cartilage damage. In that case, you might be advised to take up surgical procedures to set things in proper shape. Surgery is done either by opening the joint or by Arthroscopy. There are various types of surgery which can be done. 1st the old and successful surgery wherein damaged part of the cartilage is debrided and filled with normal cartilage from the non-weight bearing part of the joint. 2nd is ACI (Autologous Chondrocyte Implantation) is a 2 stage procedure wherein chondrocyte is grown using stem cell and implanted in defect part in 2nd stage. 3rd is multiple drilling of the area usually done in childrens.

Osteoporosis: Management and Prevention!

The bone is one of the hardest substances in the body. It is equal to a matrix which is filled with various minerals like calcium and phosphorus. With age, people lose on the minerals and what remains is the porous matrix. This is a very common condition in elderly people and is known as osteoporosis - osteo for bone, and porosis for the porous condition.

The good news is that this condition can be managed and the onset can also be prevented.

Management: This involves a multi-pronged approach, covering lifestyle changes, diet, exercise, and medications.

  1. Lifestyle changes: Quitting smoking and reducing alcohol help in improving osteoporosis.
  2. Diet: Improving vitamin D, calcium intake, and protein supplements help in preventing the onset of osteoporosis. If you do not get the required amounts of vitamins and calcium from the diet, you may be advised to take supplements.
  3. Exercise: The main issue that people face due to osteoporosis is that the bones get brittle and can fracture and lead to frequent falls. Exercising regularly by including some weight-bearing workouts can a help in strengthening the bones, thus preventing fracture.
  4. Reduce the incidences of falls: It is very essential to alter the living spaces to reduce incidence of falls. This could include improved lighting, using anti-fall rugs, regular eye checks, etc.
  5. Medications: Bisphosphonates is the most commonly used drugs for osteoporosis. In addition, hormonal supplements are also used in women who are post-menopausal.

Prevention would include understanding risk factors and working on reducing the impact of these, as below.

  • Calcium supplements are suggested for people with a family history. This helps in strengthening the bone matrix and thereby prolongs porous bone formation.
  • Women over the age of 40 should take calcium supplements. Menopause is a major risk factor, and this is particularly due to the hormonal changes (lowered levels of estrogen).
  • Reducing cigarette smoking helps prolong osteoporosis, as smoking is a major risk factor. The risk is increased in women smokers.
  • Sedentary lifestyle is another risk factor, and therefore those who are at risk should engage in an active lifestyle. A regular exercise regime is very essential to keeping the bones agile and strong.

Dietary changes to prevent osteoporosis include:

  1. Diet rich in dairy products, as it helps by providing adequate calcium. Yoghurt consumption also helps in improving estrogen levels, and prevents osteoporosis.
  2. Eggs also help in improving calcium and estrogen levels in the body.
  3. Fish and meat are rich in calcium and protein, so help in preventing osteoporosis.
  4. Bananas improves calcium absorption, provides essential oils required for bone health.
  5. Cabbage helps in improving bone metabolism and improving calcium uptake by the bones.
  6. Reduce alcohol and caffeine intake.

So, while osteoporosis is inevitable given the natural bone structure, it can definitely be prevented and managed.

Back pain if not improving with physio and exercises may need surgery. Usually its a safe surgery but any surgery has its due risks involved. In a properly selected pt for the right indications most spine surgeries provide good results.
