Gastroesophageal Reflux Disease - What Should You Know

MBBS Bachelor of Medicine and Bachelor of Surgery, MS - General Surgery
General Surgeon, Hyderabad
Gastroesophageal Reflux Disease - What Should You Know

Gastroesophageal reflux disease (GERD) is also called as acid reflux. It is a long-term digestive disorder that affects the lower esophageal sphincter, the ring of muscle between the esophagus and stomach which causes the stomach contents to come back up into the esophagus resulting in either symptoms or complications. Gastroesophageal reflux disease is mild acid reflux that occurs at least twice a week or moderate-to-severe acid reflux which occurs at least once a week.

Epidemiology

In western populations, GERD affects approximately 10% to 20% of the population and 0.4% newly develop the condition. In developed nations, the prevalence rate of GERD is associated with age; with adults aged 60 to 70 being the most commonly affected.

Signs and Symptoms

Esophageal symptoms include:

  • Heartburn
  • Regurgitation of food or sour liquid
  • Dysphagia
  • Sensation of a lump in the throat

Extraesophageal symptoms include:

Risk Factors

  1. Obesity
  2. Pregnancy
  3. Hiatus hernia
  4. Scleroderma and systemic sclerosis
  5. The use of medicines such as prednisolone
  6. Delayed stomach emptying Zollinger-Ellison syndrome which can show an increase in gastric acidity because of gastrin production
  7. A high blood calcium level which can increase gastrin production, leading to increased acidity
  8. Visceroptosis or Glénard syndrome, in which the stomach has sunk in the abdomen upsetting the motility and acid secretion of the stomach.

Factors that can increase acid reflux involve:

  • Smoking habit
  • Eating heavy meals or eating late at night
  • Eating fatty or fried foods
  • Drinking certain beverages, such as alcohol or coffee
  • Medications, such as aspirin
  • Tight clothes
  • Increased intra-abdominal pressure

Differential Diagnosis

  1. Heart disease causing chest pain
  2. Laryngopharyngeal reflux OR extraesophageal reflux disease
  3. Infection
  4. Peptic ulcer
  5. Esophageal spasm

Complications

  1. Narrowing of the esophagus (esophageal stricture): Stomach acid causes the formation of a scar tissue which narrows the food pathway.
  2. An open sore in the esophagus (esophageal ulcer): Acid causes an open sore which bleeds, causes pain, and makes swallowing difficult.
  3. Precancerous changes in the esophagus (Barrett's esophagus): Damage from acid can cause changes in the tissue lining the lower esophagus
  4. Anemia

How to arrive at the diagnosis?

  1. Upper gastrointestinal endoscopy/esophagogastroduodenoscopy
  2. Esophageal manometry
  3. Ambulatory 24-hour pH monitoring

Treatment

The goals of treatment are controlling symptoms, healing esophagitis, and prevention of recurrent esophagitis or other complications.

Lifestyle modifications include the following:

  1. Losing weight (if overweight)
  2. Eating small and frequent meals instead of large meals
  3. Waiting 3 hours after a meal to lie down
  4. Elevating the head end of the bed by 8 inches
  5. Avoiding bending or stooping positions

Medications

Surgery

  • Transthoracic and transabdominal fundoplication
  • Placement of a device to augment the lower esophageal sphincter

Botox Treatments: Make yourself look more refined!

Botox Treatments: Make yourself look more refined!

While we cannot reverse the aging process, we can slow it down and control the visible signs of aging. Botox is one such treatment that can keep us looking younger. It's mainly used for the facial lines and wrinkles that age, stress, worry, and environmental factors may produce, even on a younger person’s face.

Botox is a drug made from the bacterium called Clostridium Botulinum. Botox is one of the most common cosmetic surgery procedure. This unique treatment is also used along with facial plastic surgery procedures such as a facelift or eyelid lift to maximize its cosmetic benefitBotox treatments can lift the brow which will make your face look less tired, giving you a more youthful appearance.

It can also be used to treat misaligned eyes, severe sweating of the underarm region, cervical dystonia, migraines and uncontrollable blinking. It can also be used to treat an overactive bladder. In terms of cosmetic enhancements, Botox can reduce Dynamic fine lines and wrinkles, prevent new wrinkles from developing. Botox is commonly injected into the forehead, glabella or space between eyebrows, outer corners of the eyes, chin and neck.

Botox is used only in small concentrated dosage by experts, as it can have severe side effects if used incorrectly. The Botox procedure is as simple as getting an injection. It is administered by injecting a readymade Botox solution into the muscles. This paralyses the muscles by blocking nerve signals to the area and preventing the release of acetylcholine, which is responsible for the muscle movement. Usually the results of a Botox injection can be seen within 24-72 hours. In rare cases, it may take up to 5 days for the results to become visible.

Botox is performed as an outpatient procedure and is considered a safe cosmetic procedure. Pregnant women are advised not to undergo Botox treatment until they stop breast feeding after delivery. However, as with any other procedures, it does have a few risks associated with it. These include: Slight pain at the injection site.

Other possible rarest side effects are:

  1. Headache

  2. Nausea

  3. Temporary weakening of the eye and neck muscles

  4. Bleeding

  5. Blurred vision

  6. Tiredness

  7. Dry mouth

  8. Rashes and inflammation

  9. Wheezing

The effects of Botox may be influenced by other medications that you are taking and hence before undergoing the procedure, let your doctor know the medicine you are on. Avoid alcohol and strenuous activities for a few days after the injection.

The results of a Botox injection are not permanent and hence they may need to be repeated. Doctors advise maintaining an interval of at least 3 months between Botox injections. However, every time a Botox injection is repeated, the time taken for the lines and wrinkles to reappear reduces.

Botox can be combined with fillers injections for better facial aesthetics.

Hello, Apply Homoeopathic medicine – Thiosinaminum Q few drops mixed in a glycerin solution on scars that are visible (as it is less greasy, so is more easily absorbed into the skin). Also take Causticum 200 orally twice daily for 4 weeks. For glowing face: Management: Include healthy, nutritious food, doing daily yoga or some kind of exercises. All these automatically reflects glow on your face. Yes, the colour which is inherited from your parents/ ancestors, cannot be changed, but you can try to get glow on your face! Daily Regime: Apart from these, you do follow CTMP, i.e. Cleansing, toning, moisturising and protection (apply liberally sunscreen with SPF-30 – a thick layer of Bakson's Suncare cream may be, 20-30 minutes before going out daily and repeat it's application after every 3-4 hrs.) after washing your face daily; use the products as per your skin type. This will change the skin texture and skin tone. In case you do have tanning, apply honey with a dash of lime on your face. Eat right and healthy that too on time, relieve your stress/ tensions, sleep well, stop smoking, as this restricts blood flow to your face and leaves you with a greyish complexion. Lead a healthy lifestyle! Apart from this, avoid anger, anxiety, fear which increases stress hormone in the body and subsequently makes your complexion darker! (Hormonal imbalances also plays a major role). If the dull and dark skin tone is due to any underlying ailment or diseases, need to look into the matter. Diet: -Increase your Vitamin C intake (lemon, orange, gooseberry, etc.Fruits/juices, strawberries, sweet peppers, tomatoes, can notaloupe, and green peas). - Eat more Vitamin A (nonfat milk, oysters, and egg yolks, carrots, watermelon, papaya, tomatoes, or other dark colored produce) . If you feel your skin is weak or not tight enough, that usually suggests a lack of Vitamin A. Medicine: Use homoeopathic cream - Medicynth's Whitch Hazel or Bakson’s Fairness cream daily at night.

The vitamin C present in lemon juice can also help remove dark circles under the eyes, thanks to its skin-lightening properties. Use a cotton ball to apply fresh lemon juice around your eyes. Leave it on for about 10 minutes and rinse it off. Do this once daily for a few weeks.
