Melasma is one of the most common forms of hyperpigmentation .It commonly appears as brow or gray -brown patches on the face . These patches are usually symmetrical and visible on cheeks,forehead,bridges of nose,above the uper lip and chin .

CAUSES OF MELASMA

Melasma is more common in women than men.

2} Depigmenting agents

a} Hydroquinone- the most common used agent ,works to inhibit a melanin -producing enzyme . It has potent antioxidant abilities.Topical hydroquinone creams comes in 2% and 4% concentrations alone or with tretinoin.Creams containing hydroquinone is intended for short-term use. A dermatologist recommendation is very important .

b} Arbutin -natural source of hydroquinone ,is considered to be much safer . It is dervided from the leaves of bearberry,cranberry,mulberry or blueberry shrubs .It too has the ability to ihibit melanin production.

To enhance skin-lightening ,the following ingredients may also help .

  • Retinoic acid
  • Azelaic acid
  • Kojic acid
  • Glycolic acid
  • Low mid-potency steroids

Pocedures fr Melasma

Chemical peels - eg. glycolic acid ,tricholroacetic acid )work well in treatment of melasma . These exfoliate the superfical layers of the skin and increase the cell turnover,which triggers the fading of unwanted pigmentation . The number of sessions depend on the level of pigmentation and skin sensitvity.

Lasers- escpecially the Q-switched Nd:YAG laser ,have a positive effect on early melasma . These are also relatively safe o Indian skin.The laser light beam acts on the pigment(melanin) clumps within the skin and fragments them.Once fragmented ,these are easily cleared by body's immune system . It is done in a gap f 15 to 20 days for a minimum of 8 to 10 sessions.

A widely used surgical procedure, laparoscopy involves the insertion of certain surgical instruments, small tubes and video cameras in your abdomen through small incisions and cuts. This procedure is used to diagnose a number of ailments such as ovarian cysts, endometriosis, and pelvic inflammatory diseases to name a few. Even though laparoscopy is a very popular form of surgery, there are quite a few myths associated with it, which are:

1. Myth: The images taken through a laparoscope are of poor quality

This is not true. In fact, the visuals obtained through a laparoscope are clearer and much more accurate when compared to those obtained via an open surgery. The visuals of a video laparoscopy provide a detailed magnification of even those parts of the area that are inaccessible by the human eye.

2. Myth: If you've undergone multiple abdominal surgeries in the past, you can't opt for a laparoscopy

The truth is that you can go for a laparoscopy even if you've gone through multiple surgeries previously, irrespective of the location or size of the previous incisions. This is done through the use of a special instrument, called a microlaparscope that enables safe entry into the abdomen of the patient.

3. Myth: If you have large fibroids and ovarian cysts, you can't have them removed through a laparoscopy

False. Even though the incisions made by a laparoscopy are really small, they still allow the safe removal of large abdominal structures by the use of certain devices. For example, a cylindrical tool known as morcellator can be inserted through a laparoscopic incision and be used to remove large fibroids and ovarian cysts.

4. Myth: If you're overweight or underweight, you can't undergo a laparoscopy

No matter if you're obese or too thin, you can still undergo a laparoscopy as the tools used for this surgical procedure are available in different lengths and sizes, and can be adjusted as per the body type of the patient before the incision is made.

3 Do's Of Pregnancy Diet

Pregnancy is an important time in a woman's life. During this period, you need to be most conscious of what to eat, do and how to change everyday habits. Following these dos and don'ts will ensure a smooth pregnancy experience for you.

Do's:

1. Take your prenatal vitamin


If you have not yet started taking a prenatal vitamin, now is the time to start. It's particularly critical to get enough folic acid while trying to conceive importantly during the first trimester. It greatly reduces your baby's risk of developing neural tube birth defects such as spina bifida.

2. Quit smoking

Smoking increases your risk of developing several problems including miscarriages, placental and preterm birth problems. It slows the fetal growth and increases the risk of stillbirth and infant death. It's never too late to quit or cut back smoking. Every cigarette you don't light gives your baby a much better chance of being healthy.

3. Do eat seafood

Seafood is loaded with vitamins and minerals. Consume a variety of seafood so you don't have a concentration of minerals from only one type of fish. But remember to not eat more than 12 ounces of fish per 7 days.

Don'ts:

1. Don't drink alcohol


Alcohol may immensely impact your baby's development. Women who drink alcohol while pregnant could easily deliver a baby with fetal alcohol syndrome (fas). Symptoms of fas are usually low birth weight, learning disabilities, behavioral problems, and lagging patterns in terms of growth and development milestones. Limiting or avoiding alcohol is a step that you need to take.

2. Don't eat raw meat at all

Raw and undercooked meat or eggs carries the risk of food-borne diseases as well as food poisoning incidence. Make sure all eggs and meat that you eat while you are pregnant are cooked well.

3. Don't drink lot of caffeine

Caffeine easily travels through the placenta and increases your baby's heart beats. Research shows that women can safely consume a cup or two of coffee every day, but avoid downing a triple shot strong latte while you have got a bun in the oven.

Category C drugscan be given in pregnancy if the benefit of drug outweighs it's risk. As you have taken the drug in early pregnancy before the start of organ development, everything will be fine. Go for a ultrasound at 13 weeks and level 2 usg at 20 weeks.

Hi. ICSI is definitely a good option for women nearing menopause because her eggs may not be as healthy and fertile as it was when she was below 35. Also, ICSI takes care of penetrating capacity issues of sperms if any. Best wishes.

Hi. Sperms need to be deposited in the vagina around the ovulation period, that is, release of egg, which can happen anywhere from 10th to 16th day of her menstrual cycle depending on the length of the cycle. The deposited sperms will live in the uterus not more than 24-30 hours. Best wishes.

Hi lybrate-user ji, If your wife is less than 34 years old and has regular periods, try for few more months. The best time to try is to have regular intercourse (alternate days is sufficient) from approximately day 10-20. If she is still not pregnant, please let me know and I can advise some tests and medicines for both of you. If she is over 35 or has irregular periods, kindly reply as then you need to be seen sooner.

Thyroid is a bit raised. You need to start on thyroid medicine. Also your weight is on higher side. Try to reduce it to 60 kg-65 kg. PCOD is directly related to weight. Greater the weight more severe the PCOD.

Dear user sometime i-pills can cause menstrual irregularities. But if, there is no semen discharge inside vagina then not to worry about pregnancy. You can do urine pregnancy test if, you have some doubt about pregnancy for better confirmation.

I am 29 years old female, married and have two kids. I have a solid lump in breast from five years. I keep getting an ultrasound for the same every 6 months-1 year. It has not changed. Recently, a radiologist suggested to remove it surgically. Should I consult an oncologist?

Most likely its not cancer But its better to get a FNAC (Biopsy) done. Then you can continue the same. No need to get it removed.
