I am having problem with right ear diagnosed with tinnitus and meunier's disease. Having VERTIGO, vomiting sometimes, heaviness always in head etc. Please advise right medicine to take and oblige.

General Physician,
No cure exists for Meniere's disease, but a number of treatments can help reduce the severity and frequency of vertigo episodes. There are no treatments for the hearing loss that occurs with Meniere's disease. All of the treatments — some conservative, some aggressive — are to stop the spells of vertigo.Meniere's disease is a disorder that affects the inner ear. The inner ear is responsible for your balance, as well as hearing. This disorder causes vertigo (a sensation of spinning), hearing problems, and a ringing sound in the ear. Meniere's disease usually affects only one ear.Although Meniere's disease is incurable, it may go away on its own. In its worst cases, the symptoms of the disease do not go away, and can eventually become incapacitating. Treatment revolves around easing the symptoms and lowering the fluid volume in the inner ear.

First of all you have to keep your ear free from water & soap. Secondly you have to prevent any indirect flow due to common cold or allergy. These are the two most important issues which needs your attention. Use a vaseline coated cotton in the ear during bath etc and take your already prescribed anti cold or anti allergic medication accordingly. Don't lift heavy weights and don't blow your nose frequently. This should dry up your ear substantially. Then you can have surgery done. If the discharge is not infected, surgery can be done in a healthy wet ear also.

Hello, It seems that you had chronic otitis media and you were operated for tympanoplasty. But unfortunately your hearing has not improved. You can use hearing aid in right ear which will help you amplify sounds.

Hi. I went to the ENT to remove ear wax. He removed the wax from my left ear and after I got home. It started bleeding a little. So I called him up and he told me to put cotton in my ear and said that it was a small skin tear and that it will stop after I change the cotton once or twice. It's late at the moment and I need some help. It bleeds little bit. I am worried. Can you help? Please. Thanks.

Hello lybrate-user, No need to panic in such situations. Most of the time ruptured ear drums remains unsymptomatic and heal. I would suggest you get an otoscopy done. Visit an ENT specialist. Here a some tips, 1) Don't let water enter your ear. 2) If you get cold and fever, consult a doctor asap. 3) Use cotton ball in ruptured ear drum ear while taking bath. 4) Do not put oil or any drops in that ear. Thank you.

Can a Sinus Infection (Rhinosinusitis) Cause Spinal Meningitis?

MBBS, MS - ENT
ENT Specialist, Delhi

The answer is unfortunately. Yes!
Meningitis, sometimes referred as spinal meningitis, is an inflammation of the membranes surrounding the brain and spinal cord. Usually caused by a viral infection, but it can also be caused by a bacterial or fungal infection.

Among pediatric patients admitted for treatment of sinusitis, 3.2% were found to have an intracranial complication. Infection of the sphenoid sinuses, however, merits concern. These thin-walled sinuses develop late in childhood, and their deep location places them adjacent to the dura mater and other critical structures.

Sphenoid sinusitis is identified in approximately 3% of cases of acute sinusitis, typically in the context of pansinusitis. Significant development of the sphenoid sinuses does not begin until age 4 to 6 years, thus, sphenoid sinusitis is restricted.

Viral infection causes most cases of spinal meningitis. Viral meningitis is usually mild and heals without treatment. Bacterial meningitis is more severe and requires treatment with antibiotics. Streptococcus pneumoniae and neisseria meningitidis are strains of bacteria that cause pneumococcus and meningococcus meningitis respectively.
It can be life threatening condition owing to its proximity to brain and spinal cord and infective media is the fluid surrounding them.

Diagnostic tools:
• Lumbur puncture – csf examination (cell count, glucose, proteins) and culture
• Blood culture
• Chest x ray
• CT scan of head and nose – pns
• MRI brain

Symptom checker in meningitis secondary to sinusitis:
• Fever (92%)
• Headache (85%)
• Nausea, vomiting (62%)
• Altered consciousness (31%)
• Seizure (31%)
• Hemiparesis (23%)
• Visual disturbance (23%)
• Meningismus (23%)

Conclusion and quick pearls:
• Complications that are less common with antibiotics
• Orbital (cellulitis, abscess)
• Intracranial (subdural empyema, thrombosis of cavernous sinus)
• Bony osteomyelitis.
• Can result in drastic sequelae
• Drain abscess and open involved sinuses
• Ent surgical involvement – functional endoscopic sinus surgery
- Usually amenable with medical treatment
- Drain sinuses if no improvement after 48 hours
• Ophthalmology check up
• Neurosurgery intervention

A low index of suspicion is necessary for early diagnosis and treatment of sphenoid sinusitis, orbital complications and prevention of intracranial complications including meningitis.
