Gestational Diabetes - Causes And Treatment

Gestational diabetes is a form of diabetes where the blood sugar levels of the body increase during pregnancy. When you are expecting, your body is more repellent to insulin so that a larger amount of glucose is manufactured for the baby’s nourishment. However, this excess blood glucose can build up within the body which causes gestational diabetes. This can lead to health problems for both the mother and the baby. At the end of the gestational period i.e. once the baby is delivered, blood sugar reduces to normal.

When are you at a risk for gestational diabetes?

  1. Gestational diabetes can also be hereditary. If a family member or a relative has diabetes, you are more likely to develop gestational diabetes.

  2. If you are overweight, you are more susceptible to developing gestational diabetes.

What causes gestational diabetes?

During pregnancy, the placenta connects your baby to the blood supply. This produces other hormones which reduce the levels of insulin. This increases the level of blood sugar in your blood. As your baby develops, your body manufactures more of such hormones which block your insulin levels leading to gestational diabetes. This usually occurs from the 20th week of your pregnancy.

Complications:

  1. Increase in birth weight – Extra glucose crosses through the placenta which instigates the baby’s pancreas to manufacture more insulin. This can make your baby very large. Sometimes, it becomes difficult for the baby to pass through the birth canal during childbirth as a result.

  2. Premature birth and respiratory diseases – High blood sugar may increase the risk of an early labor. It may make the baby due before the 37th week of pregnancy. This also makes the baby more susceptible to respiratory disorders.

  3. High blood pressure – Gestational diabetes also raises your risks for high blood pressure. This can be fatal for you as well as your baby’s health.

Treatment:

  1. Book an appointment with a dietitian and follow a healthy eating schedule. Limit the amount of carbohydrates and ensure you have a healthy and balanced meal.

  2. Do light exercises regularly during pregnancy. This helps you to control your blood sugar level. Physical activities which require low levels of energy utilization such as swimming and walking will control your blood sugar level without exerting you too much.

  3. Taking insulin shots and medicine for diabetes will also control your blood sugar levels if you have a pre-existing condition.

PCOS - 7 Signs You Are Suffering from it!

PCOS is a disorder characterized by enlarged ovaries and the formation of tiny cysts on the outer sides of the ovaries. Polycystic Ovary is a hormonal condition that causes thesurplus surplus production of androgen in women. Androgen causes development of weight gain and hair in women as well when they are present in high amount. They trigger acne and excessive growth of body hair in women accompanied by irregular or lack of ovulation (release of afemale female egg, ovum) which is reflected in the form of absent or erratic menstrual cycle. Owing to troubles in ovulation one may experience difficulty in becoming pregnant. 

Many women suffering from PCOS have a resistance to theaction action of insulin hormone. Normally Insulin drives sugar into the body cell to provide energy for it’s work. Many women with PCOS may appear unaffected by this change, but at cellular level, this resistance means it requires a larger amount of insulin production to sustain a normal blood sugar level. High levels of insulin as a result of insulin resistance, in turn drastically boosts androgen production. 

Symptoms of PCOS include: 

  1. Irregular Menstrual Cycle. Women with PCOS may miss periods or have fewer periods (fewer than eight in a year). Or, their periods may come every 21 days or more often. Some women with PCOS stop having menstrual periods
  2. Excessive hair. Excessive hair on the face, chin, or parts of the body where men usually have hair. This is called "Hirsutism"." Hirsutism affects up to 70% of women with PCOS. 
  3. Acne. Acne occurs on the face, chest, and upper back 
  4. Hair Loss. Thinning hair or hair loss on the scalp; male-pattern baldness and underneath breasts 
  5. Skin tags. Skin tags, which are small excess flaps of skin in the armpits or neck area

Treatment for PCOS:

There is apparently no procedure to cure PCOS. The aim of treatment varies as per age and the requirement of the patient. 

  1. If the patient is having Infertility: A lack of ovulation hinders the secretion of progesterone (a hormone preparing the uterus for pregnancy), thus hampering conception. In this case, the treatment is directed at maintaining a regular ovulation, by use of ovulation-stimulating medication. 
  2. In patients with irregular cycles NOT wanting pregnancy: The irregularity of menstrual cycle can be fixed by consuming birth control pills. These pills comprise of both progestin and estrogen which aid to bring down androgen production. 
  3. For patients with PCOS and weight gain, obesity: Insulin resistance seen in PCOS warrants the use of certain medications such as metformin which are prescribed to enhance sensitivity of the cells to insulin. Metformin enhances cell function and maintaining sugar levels by its increased consumption by the cells. If you wish to discuss about any specific problem, you can consult a gynaecologist.

Pregnancy - How Eating Disorders Affect It?

There are three main types of eating disorders. These are bulimia nervosa, anorexia nervosa and binge eating. Anorexia nervosa is an exaggerated fear of gaining weight all the way to the point when the patient becomes abnormally thin. Bulimia nervosa is eating too much and then vomiting it out on a regular basis. Binge eating is when an individual eats an excessive amount of food in one go. All three types of eating disorders have adverse effects on pregnancy. 

Here is how eating disorders affect pregnancies:

1. Unplanned pregnancies: Women with eating disorders are much more likely to have an unplanned pregnancy. This is true both for women who had eating disorders in the past as well as women who have eating disorders at the moment. In a study by University College London and King's College London consisting of 11000 women showed, only 28% of women reported that they had an unplanned pregnancy without suffering from any eating disorder as compared to the 41% who did have an eating disorder at some point in their life. The reason for this is that women tend to think that they do not need contraceptives due to an irregular menstrual cycles, which is one of the consequences of eating disorders during pregnancy. 

2. Happiness about their pregnancy: The same survey of 11000 women conducted by University College London and King's College London reported that 71% of women were happy about their pregnancies. However, the number would have been a lot higher if there were fewer women with eating disorders. It has been said that 10% of women were unhappy to discover they were pregnant, if they had an eating disorder in the past as compared to 4% who never had an eating disorder.

3. Consider pregnancy a 'personal sacrifice': It has also been said that women who have had or have eating disorders during or before their pregnancy tend to consider pregnancy as a 'personal sacrifice' much more often than women who have no history or eating disorders.

Dear lybrate-user, Vagina is a muscular structure and accommodates the penetration after initial few seconds, especially if there is no friction for first dew seconds. You might be experiencing the pain on insertion of the finger more due to psychological fear regarding the intercourse, rather than any physical obstruction. The presence of hymen makes the mouth of vagina smaller. But once it is broken, there should be no obstruction. If you are still worried, please see physically (locally) a Gynecologist for a checkup. Thus I believe that there is no physical problem as such with you. You should work with your fear regarding the sexual intimacy as it may interfere with your married life. In vitro fertilization is generally used in couples who have failed to conceive after at least one year of trying who also have one or more of the following fertility issues:

Blocked fallopian tubes or pelvic adhesions with distorted pelvic anatomy.

Severe male factor infertility (low sperm count or low motility).

Failed 2-4 cycles of ovarian stimulation with intrauterine insemination.

Advanced female age - over 38 years old.

Reduced ovarian reserve, which means having a low egg supply. A day 3 fsh, ovarian antral follicle counts and amh blood levels are often used as screening tests for egg quantity.

Severe endometriosis.

Management of Endometriosis

When tissues that usually grow inside the uterus, start growing outside the organ, it is called endometriosis. Painful periods, pain during sex, pain while urinating or bowel movements, extreme bleeding, fatigue, infertility, diarrhea, bloating and nausea are some of the symptoms of endometriosis.

Treating endometriosis usually depends on the following factors

  1. Age
  2. Severity of symptoms
  3. Severity of the disease
  4. Future plans for children

Depending on these the doctor might opt for any of the following treatments

  1. Medication for Pain: If the symptoms are mild, then the doctor will prescribe some pain killers like Nonsteroidal anti-inflammatory drugs. 
  2. Hormone Therapy: Hormone therapy is effective, but once you stop them, your symptoms may come back. Some of the hormone therapies include: 
    • Hormonal Contraceptives: Birth control pills, vaginal rings and patches are useful in controlling the hormones that are responsible for tissue build-up outside the uterine cavity. 
    • Medroxyprogesterone: This drug stops menstruation, which in turn stops the tissue build-up and put an end to all the symptoms. But on the other hand, gaining weight, low bone production and depression are some of the common effects of this drug. 
    • Gonadotropin-releasing hormone (Gn-RH) antagonists and agonists: Hormones that stimulate ovaries are blocked by these. They bring down oestrogen levels and hence prevent menstruation. 
    • Danazol: This drug prevents menstruation and symptoms of endometriosis by blocking the hormones that stimulate the ovaries. However, Danazol may not be a favourable option because of its side effects. The drug should be avoided during pregnancy.
  3. Conservative Surgery: If you are trying to get pregnant, but you already suffer from endometriosis, then removing endometriosis surgically is a viable solution. Your uterus and ovaries will be preserved, which might increase your chances of becoming pregnant. Even suffering from severe pain might find relief from conservative surgery. 
  4. Hysterectomy: In extreme cases, the only way to deal with the symptoms is to completely remove the uterus, ovaries and cervix. Since a hysterectomy means you cannot have children, it is kept as the last option for women in their reproductive age. Since you are trying for more than a year without any results, please consult a fertility specialist and get both of your basic blood tests done. This will help you to know if any hormonal correction needs to be done or not or you need any treatment for conception.

In majority of the cases irregular menses are accredited to hormonal imbalance, but in some cases it may be pathological and secondary to some pathology of ovaries mostly. Now if you want to regularized the menses then you must establish permanent balanced first and this can be achieved only with homoeopathy, because in modern medicine we get only a hormonal support with a very temporary relief. But with homoeopathy you can strike the hormonal balance permanently which later on also give you regular menses even if you stop the medicine.

please take rajahpravartini vati 2 tabs twice daily .take asokarista 4 tsf in half glass of water half an hour after meals.continue the regime for one month .Take a week time break and again take it for one more month.Good luck n stay in touch

I have PCOS problem due to which I am not able to conceive, but I have very regular periods. My gums had put me on clomid for 6 cycles and on gonadotrophin injections for 4 cycles and did IUI for 2 cycles. For this month I am not taking any medications as suggested by my gums. But she has told me to take Chirocyst Tablet (contains Chromium Picolinate, D-Chiro-Inositol, Myo-Inositol, N-Acetylcysteine, and Vitamin D3 as active ingredients). How is it going to help me? I am due to my period but still no signs. Am I pregnant? As I heard many stories which says after coming off clomid we can conceive naturally. I really need a professional advice please.

Chirocyst tablet is used in women with PCOD. It is an ovarian sensitising agent and improves hormonal ratios and reduces insulin resistance. This helps in achieving ovulation and pregnancy. Yes it is true that a few women can conceive naturally after coming off clomid. This is related to a healthy lifestyle, weight control and correction of hormonal imbalance and insulin resistance in PCOD. If you have missed a period please do a serum b-HCG which a blood test to see if you have conceived!

As they say, it is all in the mind. The chemical and psychological correlation between the brain and intercourse is way too high. Arousal and physical gratification are chemical reactions and with watching porn, what starts as an innocent indulgence can play a major role in these chemical pathways and alter the natural course of things.

What many people don’t realize is that by watching too much porn, the mind’s channeling is affected and therefore the levels of satisfaction are altered. Read on to know some ways that porn harmfully affects your body.

  1. Personal disconnect: By watching too much porn, a person does not need a partner to get aroused and satisfied. In most cases, the porn and the re-runs of it induce the arousal and gratification. Therefore, when there is an actual partner involved, their role is minimalized and therefore they feel neglected. The partner never feels wanted or satisfied in these relationships.
  2. Kills libido: Because the person connect is so less, there is no drive to enjoy a good sexual relationship. The partner is not required to turn the addicted person on and so there is no sex drive. There is no role for cajoling, cuddling, or anything that gets personal. It is a mechanical process, and therefore, for the partner, it can be a very harrowing experience. The definition of “normal” keeps changing and evolving for the porn watcher, which may not be easily accepted by the partner. This can lead to a lot of expectation mismatch and failing relationships.
  3. Short-lived: Because watching porn is all about satisfying the urge for yourself, it is only that in the mind. There is no consideration for a foreplay or intimacy or what the other person could be wanting. This makes it very selfish, leaving a wide gap in terms of loss of intimacy, which is very much expected here.
  4. Playing it rough: Most people who watch porn gradually move to violent forms of sex and therefore expect to follow it in their lives too. This again can lead to forced impersonal sexual experiences which leave both the partners very unhappy.
  5. Mechanical: In most porn watchers, the end result is not to enjoy the whole act but to reach a high and be done with it. This makes it mechanical, again adding to the bitterness in the relationship.
  6. Selfish behavior: As noted in all of the above instances, it always becomes too much of what one person wants. There is very less consideration for the partner, which makes your relationship unhealthy.

So, while watching porn is one’s personal decision, but watching it within a certain limit may keep you from its side effects, and not let it rule your sexual expectations.
