Can HIV Be Diagnosed Through Oral Manifestation?

HIV is a systemic disease which affects all parts of the body. The oral cavity also has some tell-tale symptoms which indicate HIV / AIDS. A careful examination and detailed history of symptoms is essential. In some cases, the oral manifestations could be the area where HIV is suspected. This can help in reducing morbidity and improves prognosis. The oral lesions that occur in HIV patients can vary and differ significantly in children and adults. While there are a variety of oral lesions in HIV-infected individuals, listed below are some common infections seen in HIV patients. These are a combination of fungal, viral and bacterial infections.

  1. Candidiasis: Candida is an opportunistic fungus that is normally present in the oral cavity and with reduced immunity of HIV, recurrent bouts of the infection begins to show up. It can be in the form of regular thrush which is whitish and cannot be scraped off (pseudomembranous candidiasis), hyperplastic candidiasis (white patches which can be scraped off) or erythematous (reddish patches). Candida can involve any part of the oral mucosa including the pharynx and the palate.
  2. Herpes Simplex: This is the most common viral infection seen in patients with HIV/AIDS. There could be primary or secondary infection of herpes virus, especially inside the mouth and the vermillion border of the lips.
  3. Herpes zoster: This virus, when already present in the body, can be reactivated with HIV/AIDS and with oral herpes. The distinction with herpes simplex is from their distribution. These are unilateral, along the distribution of the maxillary or mandibular nerve. The lesions appear both on the facial skin and the oral mucosa. While the facial ones break open and form crusts, the mucosal ones coalesce to form larger lesions.
  4. Hairy Leukoplakia: This is present in about 20% of asymptomatic HIV patients. Onset of hairy leukoplakia is an indication of rapid progression of HIV with increased CD4 counts. The typical lesion is a non-movable, hairy lesion along the side of the tongue and can spread to the top and the undersurface of the tongue. There are large amounts of Epstein-Barr virus (EBV) identified from biopsies of hairy leukoplakia.
  5. Cytomegalovirus: If the ulcers have a necrotic base with a halo surrounding it, it is CMV infection, usually seen on any oral mucosal surface.
  6. Periodontal disease: This is one of the bacterial infections that manifests itself in HIV patients. It can take two forms such as Linear Gingival Erythema (LGE) which can subsequently lead to Necrotizing Ulcerative Periodontitis (NUP). The oral hygiene is generally good with minimal plaque and there is rapid bone loss and soft tissue reddening and swelling. The, mouth, therefore is certainly a window to one’s health.

Diagnosing HIV with Western Blot Test-

It is a series of blood screenings are performed to test for HIV. The enzyme-linked immunosorbent assay (ELISA), also known as an enzyme immunoassay (EIA), is the first test that your healthcare provider will order to screen for HIV. ELISA, like the Western blot test, detects HIV antibodies in your blood. Antibodies are proteins your immune system produces in response to the presence of foreign substances, such as viruses. If you test positive for HIV on the ELISA test, your provider will order the Western blot test to confirm HIV infection.

Dear patient this is actually a nutritional suppplement and vitamin used to treat or prevent the medical conditions that can occur if you are deficient in these vital nutrients.  Thus, the effects of long-term use have not been studied, however, they are generally considered safe, as long as you are not taking extremely high doses.  Regards.

Homeopathy and obesity – Obesity means having too much body fat. It is not the same as being overweight, which means weighing too much. A person may be overweight from extra muscle, bone, or water, as well as from having too much fat.

In general, women collect fat in their hips and buttocks, giving their figures a “pear” shape. Men, on the other hand, usually collect fat around the belly, giving them more of an “apple” shape.

India is now in the grip of an obesity epidemic and the trend needs to be immediately arrested by taxing junk food, restricting food ads and making food labelling clearer, according to a study. Currently, almost 1 in 5 men and over 1 in 6 women are overweight.

Causes, incidence, and risk factors—-

Taking in more calories than you burn can lead to obesity because the body stores unused calories as fat. Obesity can be caused by:

  1. Eating more food than your body can use

  2. Drinking too much alcohol

  3. Not getting enough exercise

Today, we know that biology is a big reason why some people cannot keep the weight off. Some people who live in the same place and eat the same foods become obese, while others do not. Our bodies have a complex system to help keep our weight at a healthy level.


 

We are surrounded by things that make it easy to overeat and hard to stay active:

  1. Many people do not have time to plan and make healthy meals.

  2. More people today work desk jobs compared to more active jobs in the past.

  3. People with less free time have less time to exercise.

  4. Obesity is a major health threat. The extra weight puts added stress on every part of your body.


 

People with obesity are at risk for these health problems:

  1. Bone and joint problems -Excess weight puts extreme pressure on your joints, sometimes causing inflammation in the joints (known as arthritis). Obesity is also a factor in osteoarthritis a form of arthritis that is caused by wear and tear on the joints

  2. Gallstones – Obesity causes the liver to secrete excessive amounts of a fluid called bile, which is stored in the gallbladder. Bile is necessary for the body to digest fats. But an overproduction of bile sets the stage for the formation of gallstone.

  3. Liver problems.

  4. Heartattack from coronary heart disease, congestive heart failure, and stroke.

5. Highblood cholesterol and triglycerides (dislipidemia or high blood fats).

6. Highblood pressure (hypertension).

7. Highblood sugar (glucose) or diabetes.

8. Stopping breathing during sleep (sleep apnea). This can cause daytime fatigue or sleepiness, poor attention,    and problems at work.

9. CANCER – Women who are significantly overweight are at increased risk of risk of cancer of the endometrium (the lining of the uterus). If you are obese and your periods are irregular.

10. INFERTILITY – Obesity sometimes leads to changes in hormone levels that can result in a failure of the ovaries to release eggs regularly. This abnormality, called an ovulation, is associated with irregular or absent periods and infertility.

Sometimes, medical problems or treatments cause weight gain, including:

  1. Underactive thyroid gland (hypothyroidism)

  2. Medicines such as birth control pills, antidepressants, and antipsychotics

  3. Other things that can cause weight gain are:

  4. Quitting smoking. Most people who quit smoking gain 4 – 10 pounds in the first 6 months after quitting. Some people gain as much as 25 – 30 pounds.

For women: Menopause — women may gain 12-15 pounds during menopause and not losing the weight they gained during pregnancy.


 

The two most common ways to measure health risks from your weight are:-

  1. A BMI of 25 to 29.9 is defined as a “pre-obese.”

  2. A BMI of 30 to 34.99 is defined as “obese class I.”

  3. A BMI of 35 to 39.99 is defined as “obese class II.”

  4. A BMI of or greater than 40.00 is defined as “obese class III.”

An active lifestyle and regular exercise, along with healthy eating, is the best way to lose weight. Even modest weight loss can improve your health. You will need a lot of support from family and friends.

When dieting, your main goal should be to learn new, healthy ways of eating and make them a part of your daily routine.

Many people find it hard to change their eating habits and behaviors. You may have practiced some habits for so long that you may not even know they are unhealthy, or you do them without thinking. You need to be motivated to make lifestyle changes. Make the behavior change part of your life over the long term. Know that it takes time to make and keep a change in your lifestyle.

Work with your health care provider and dietitian to set realistic, safe daily calorie counts that help you lose weight while staying healthy. Remember that if you drop pounds slowly and steadily, you are more likely to keep them off. Your dietitian can teach you about:

You may see ads for supplements and herbal remedies that claim they will help you lose weight. Many of these claims are not true, and some of these supplements can have serious side effects. Talk to your health care provider before using them.

Several prescription weight loss drugs are available. Most people lose between 5 and 10 pounds by taking these drugs. Most people also regain the weight when they stop taking the medicine, unless they have made lasting lifestyle changes, such as exercising and cutting unhealthy foods from their diet.

Possible causes include:

In healthy teeth, a layer of enamel protects the crowns of your teeth—the part above the gum line. Under the gum line a layer called cementum protects the tooth root. Underneath both the enamel and the cementum is dentin. 

 

Dentin is less dense than enamel and cementum and contains microscopic tubules (small hollow tubes or canals). When dentin loses its protective covering of enamel or cementum these tubules allow heat and cold or acidic or sticky foods to reach the nerves and cells inside the tooth. Dentin may also be exposed when gums recede. The result can be hypersensitivity.


 

Sensitive teeth can be treated. The type of treatment will depend on what is causing the sensitivity. Your dentist may suggest one of a variety of treatments:

  • Desensitizing toothpaste. This contains compounds that help block transmission of sensation from the tooth surface to the nerve, and usually requires several applications before the sensitivity is reduced.

  • Fluoride gel. An in-office technique which strengthens tooth enamel and reduces the transmission of sensations.

  • A crown, inlay or bonding. These may be used to correct a flaw or decay that results in sensitivity.

  • Surgical gum graft. If gum tissue has been lost from the root, this will protect the root and reduce sensitivity.

  • Root canal. If sensitivity is severe and persistent and cannot be treated by other means, your dentist may recommend this treatment to eliminate the problem.

Proper oral hygiene is the key to preventing sensitive-tooth pain. Ask your dentist if you have any questions about your daily oral hygiene routine or concerns about tooth sensitivity.

However, in the indian society sexuality is often considered the domain of the young, and the idea of older women having and enjoying sex sits uncomfortably with many people. The ideas of older women’s sexuality often stem from Victorian times, where the woman was passive in her sex life, and sex was mainly for reproductive purposes Hence, the idea was that sex would stop after the menopause.

The menopause is often described as a very negative time for women, especially in medical literature. On the other hand, many feminists celebrate the menopause and subsequent years as a time of positive change, without the commitment to childrearing, and a time to find new fulfilment. The experience for each individual woman is probably somewhere in the middle. 

Hormonal changes in the menopause, such as a drop in oestrogen levels within the body, can bring about physical changes such as vaginal dryness, which can affect the sex life. But at the same time, there are many ways of adjusting to these bodily changes that can lead to new ways of lovemaking.


 

Physical complaints and sexuality:

Vulval discomfort -
When you get older, your body produces less oestrogen. This can cause vaginal dryness, which means that sex might become less enjoyable or even painful. 


 

Urinary Tract Infections (UTIs)-

When a woman’s vaginal walls become thinner because of lower levels of oestrogen in her body after menopause, penetrative sex can not only cause vaginal discomfort, it can also cause urinary irritation or urinary tract infections. This is because the urethral passage also thins. Penetration might irritate the bladder and the urinary tract through the thin walls of the vagina. This irritation can lead to infections.


 

Sexually Transmitted Infections (STI) and AIDS-
Articles and books on STIs hardly ever talk about older women specifically, but STIs as well as HIV/AIDs are on the increase in older women. This might partly be because women after the menopause do not need to use birth control, and therefore also do not use protection such as condoms anymore. If you have had the same partner for many years, and your partner has also not had any other sexual partners, the risk of getting a sexually transmitted infection is low. But if either you or your partner has unprotected sex with anyone else, the risk is increased. There are many different STIs, and some of them do not have any symptoms. However, if you notice any unusual or smelly discharge from the vagina, bleeding, blisters, itches, pains or sores, you should go to a sexual health clinic.


 

Pelvic Cramps-
Sometimes women may experience pelvic cramps during or after sex, especially women who have penetrative sex. It is more likely to happen if you do not have orgasms very often, and is generally nothing to worry about. Just like any muscle in the body, it can ache if it doesn’t get used very much and then gets a lot of exercise.


 

Psychological changes:
Sometimes, changes in the family can be difficult to adjust to when getting older, for example if your children leave home or you stop working and retire. It can help to talk with your friends and your partner about these changes and how they make you feel.
Because of the physical changes that occur as part of the aging process, it is possible that you may feel less comfortable with your body and that you lose some of your self-esteem. But again, these changes happen gradually, and many women adjust well to them.
However, if fatigue or psychological illnesses such as chronic depression or anxiety develop, they can interfere with your libido and sex life.


 

Medications and sexuality:
As women get older, they are more likely to need long-term medications. Often, these medicines have side effects that influence libido, for instant blood pressure medications, and tranquillisers. However, alternative therapies might be available, and if you think that medication impacts negatively on your sex drive, speak to your doctor and ask if there is an alternative. Antihistamines and other drugs can cause drying of the vagina, and although this might not influence your libido, it might curb your enjoyment of sex.

 

Final thoughts
Many women actually enjoy sex more in later life, maybe because they are more experienced, and know what they want. In addition, there may be more opportunity for spontaneous sex, for instance if you have children who have now left home. Overall, women in their late 40s are said to be much more likely to have fulfilling sex lives and multiple orgasms than women half their age 5.Women’s sexual response is different from men, and many women do not have ‘spontaneous desire’, meaning it may not be until starting to engage in some sort of sexual activity that they start to feel sexual desire, and many women do not have any sexual feelings or thoughts, unless engaging in sexual activity. Because women’s sexuality is so complex, it might actually be more appropriate to try and solve occurring problems with counselling and medication.

When if one frequently passes watery stool that have no lumps in them look out for other symptoms of diarrhoea. These can include vomiting, fever, and sometimes blood or mucus in your baby's stool


 

Newborns pass stool freq, so you may worry that your baby has diarrhoea when in fact one pass stool that are normal for her age.

Also, how often your baby does a stool will depend on whether she's being breast or formula-fed.


 

Here are more signs of normal motion:

  1. The stool of breastfed newborn babies is usually yellowish and on the soft or liquid side. Your baby may fill nappy up to five times daily.

  2. Sometimes a breastfed baby will pass motion during or immediately after each breastfeeding session. This happens because as her stomach fills up, the milk stimulates her entire intestinal tract, prompting a bowel movement.


 

Within a month, most breastfed babies do one or two poos a day.


Causes of diarrhoea?

The most common cause of diarrhoea is a virus called rotavirus

Rotavirus causes an infection of the gut called gastroenteritis. The infection damages the inner lining of your baby's intestine, allowing fluid to leak and food to pass through without any nutrients being absorbed. In some cases, rotavirus can lead to a serious bowel infection and dehydration.


 

A vaccine against rotavirus will be offered as part of your baby's routine immunisations when she is eight weeks and again when she is 12 weeks.


 

Diarrhoea can also be caused by:

  1. Allergies (find out more about milk allergy and intolerance and food allergies)

  2. Antibiotics

  3. Food poisoning

  4. Enzyme deficiencies, although these are rare


 

How should care during baby's diarrhoea?

  1. Make sure that your baby drinks plenty of fluids to help ease her symptoms and prevent dehydration.

  2. If your baby is taking breast or formula feeds best than

  3. You can try your baby with sips of oral rehydration solution (ors) a few times an hour.

  4. An ors helps to replace the fluids and salts your baby loses because of the diarrhoea. Keep giving water and usual breastfeeds

  5. Don’t give her fruit juices, glucose drinks and fizzy drinks. Unabsorbed sugar draws water into the intestine and can make diarrhoea worse

  6. Don’t give your baby anti-diarrhoeal medicine. It must not be given to children under 12 years old, as it could cause serious side-effects

  7. To prevent your baby passing on her diarrhoea to other children, keep her away from childcare or nursery until at least 48 hours after her last diaeroohea


 

Breastfed babies less likely to get diarrhoea?

Yes. Certain elements in breastmilk can inhibit the growth of the germs that cause diarrhoea


 

Should stop giving solid foods in diaerrohea?

  1. No. If your child is six months or older, unless she is vomiting frequently, you can still give her solids.

  2. You could try foods such as bananas, rice, apple puree, and dry toast if your baby has recently started solids.

  3. For an older toddler you could try small amounts of chicken and starchy foods, such as mashed potatoes and pasta.

  4. But don't worry doesn't want to eat. It's more important that drinks to avoid dehydration


 

When should call the doctor?

Diarrhoea can be worrying if it lasts for more than a few hours, but it will usually clear up on its own. If your child has loose, watery stools for more than a couple of days, call your doctor


 

The biggest concern with diarrhoea is fluid loss, so don't delay in showing your pedatiaric if your baby shows these signs:

  1. Dry skin or lips

  2. Listlessness

  3. Tearless crying

  4. A sunken fontanelle

  5. Discoloured hands and feet

  6. Strong yellow motion

  7. Fewer wet nappies than usual


 

Shows the following secondary symptoms:

  1. Vomiting which lasts more than 24 hours

  2. Fever that lasts longer than 24 hours

  3. Refusal to drink

  4. Blood in motion

  5. A swollen tummy


 

How to decrease baby’s discomfort?

Cuddle and comfort as much as possible, and keep her dry. Use care and tenderness when changing nappies since it's easy for a baby's bottom to become sore with diarrhoea. Use a barrier cream to prevent irritation if the diarrhoea lasts more than a day.


 

How prevent baby from getting diarrhoea again?

Proper hygiene can help reduce the chance of diarrhoea, because the germs that cause it can be easily passed from hand to mouth. So wash your hands thoroughly with soap after handling soiled nappies or using the toilet.

Dear Mr. lybrate-user, please avoid Milk, fruit juice, spicy and fatty meals for a month. Take small frequent meals rather than overfilling yourself. Motioning walk or jogging will help. Reduce associated stress if present. If there is no improvement inspite of all these consult doctor for complete evaluation and acid suppression drugs for few month.
