Perineum and Pelvic Floor Repair - A Complete Guide!

The perineum is the area between the vagina and the anus. This fragile area plays a significant role in the female reproductive system. This is often intentionally or accidentally torn open to allow for the normal delivery of the baby. This needs to be repaired meticulously to ensure that the pelvic floor is safeguarded from further damage.

The pelvic floor contains multiple muscles and supporting tissues which help in keeping the vagina, rectum, uterus and supporting structures in their places. When these muscles become weak due to childbirth, ageing, perineal injury etc, the uterus, bladder and rectum loosen up and prolapse down.The most common problem that is encountered is urine and fecal incontinence, where the woman has difficulty holding urine and feces. Though not often discussed openly given the private nature of the topic, many women suffer from a weak pelvic floor. Read on to know some of the common causes, symptoms, and management measures.

Causes:

  1. Chronic cough: The next time you cough for prolonged periods, notice how much pressure it puts on the pelvic area. This goes unnoticed, but chronic cough can weaken the pelvic floor muscles. It is not common for women to lose some urine during these long-drawn coughing spells.
  2. Chronic constipation: The pressure exerted during constipation weakens the pelvic floor, often leading to incontinence.
  3. Pregnancy and delivery: Most common cause of perineal injury, where the perineum is torn (accidentally or intentionally) to allow for easy passage of the baby. If it is not repaired meticulously, the pelvic floor is weakened.

Symptoms:

Urinary incontinence is the most common symptom that women present to doctors with. However, there are minor symptoms which a woman would experience prior to reaching this stage. Most often, these are ignored, and only brought to doctor’s notice when symptoms become quite severe. If left unattended, there could be more severe repercussions too.

  1. A heavy feeling in the pelvic area, commonly referring to as a bearing down sensation, a feeling that the internal organs are going to pop out through the vagina
  2. A small lump that can be felt and/or seen at the vaginal opening
  3. Feeling of incomplete emptying of the bladder and/or bowel
  4. Constipation
  5. Pain or bleeding from the vagina or the rectum
  6. Recurrent pelvic and/or urinary tract infections

Management:

  1. Kegel exercises are often used to strengthen the pelvic floor. This is very effective if the problem is identified quite early.
  2. In more progressive cases, surgical repair is the most effective option. This not only corrects the prolapses but also helps boost confidence of the person, by correcting the incontinence problems and improving sexual experiences. Smart Things to do When You Are Facing an Issue in Life

We all have issues in life which we want to resolve and feel happy and light again. What matters here is knowing how to skillfully deal with the problem rather than building up fears and anxiety around it.

Here are the two basic but extremely effective tactics to use to solve the issue before we let it take a larger form.

1. Write the issue down- we tend to worry about issue nonstop, even before sleeping. We want to get out of the situation but we don't know how. We even tend to worry about it a bit too much and out of helplessness, not knowing what to do, go into these vicious circles where it gets a little tough to come out of. We often tend to push painful emotions away, because we don't want to be in that situation, it hurts! we want to push it away as hard as possible and as soon as possible.

But just wait! take a step back and get to know what exactly you are worrying about. What is going on in the first place? you might first start from a very superficial level. Ex- I just broke up with my boyfriend. But try to do some free flow writing about the issue. Here are a few questions to help you start looking at the issue at a deeper level.

    What is going on
    How are you feeling about it
    What hurt you the most
    Why do you think all this happening? where do you think it all started from?
    Which need of yours was being satisfied that has been cut off now? example: a girl who has recently experienced a break up says: what truly scares me about my break up is that I am scared to be independent; I need someone constantly supporting me.
    How much were you truly responsible for what has happened and how much were others and other environmental factors contributing to the issue.


These are just few questions to get you started to explore the issue. Write whatever you feel like and whatever comes to your mind. Free flow writing about the issue will automatically lead you into different and interesting areas which you might not have even explored before. Try not being resistant to the idea of writing and approach it with a curious attitude.

2. Brainstorm- now that you have a better understanding about the issue, where it is probably coming from, what you should probably look at to resolve it, and also now that you feel less burdened, since it is all out on those papers, it is time to brainstorm the solution!

Remember here that every pain or suffering is temporary. There is always a way to make it better.

Pick up on the cues your mind has given you about how to resolve the issue or at least which areas to focus on, while writing down your issue. Write down the probable options you have to resolve your issue. For example- a worked up business man realizes that he is experiencing a burn out since he has stopped gyming. He realized this while addressing the question, where did it all start from. Now he realizes that gyming was extremely crucial to de-stress and resolves to make time for it again.

Reflecting on what worked for you earlier, or what helped you feel better earlier, would give you great insights into what you should probably do now.

The girl who broke up realizes that her main need was to have some constant support. Upon brainstorming if there were other ways this need can be fulfilled, she has realized that she has great friends and a lovely sister whom she can talk to, she also sees it as an opportunity to grow herself into a strong independent women.

3. Utilizing your resources to the fullest- when you brainstorm alternatives of what you should probably do, you will see that they lead you to very interesting resources. It is practically impossible to solve an issue if we don't have enough information and the right resources. Few of the easily available but unbelievably effective resources which have a large impact when implemented are listed below, in case they haven't struck you till now. These are such simple resources we often tend to overlook or underestimate.


    Parents- just spending some time around them might make a huge difference to the way you are feeling. They are almost readily available for us, but we tend to just ignore it and choose to get locked away in our rooms and go on a ride again with our negative emotions. Instead just step towards them!
    Friends- we are scared of looking vulnerable if we share our problems with our friends. We also tend to disguise the problem or give bits and pieces of it, because we feel telling what exactly is going on might make us look really vulnerable. All these could just be our assumptions. If these friends are close, exposing the vulnerable you, could even get you closer, and you can take a sigh of relief to finally put it out of you.
    Hobbies- often people say, if my situation is better only then I can go and do what I like! it doesn't quite work that way. Hobbies help you de-stress by channeling your negative energy and replenishing you with positive energy, they empower you to face the issue. This is what is going to save you big time! simple things like watering the plants, sketching what is going on in your head or something creative, just coloring, etc. Prove to be great ways to help yourself feel better.
    Social communities- it is great to be out there with like-minded people, or with people who might be experiencing the same thing as you. Just push yourself with that one first step and you just might be sorted!
    Don't be afraid to seek out for help- everyone faces issues, everyone does! but a proactive person is the one who will be able to pull him/herself out of it and feel good and confident again. Prolonging things might make the pain turn into suffering. Seek help from a psychologist/ counselor when you need it. It is a great space to explore yourself and be mentally fit.

These are just few of the resources mentioned here. Now when you grab your pen and paper I am sure you will come up with a great list! figure out which resources work for you and keep your resource box ready to rescue you!

If you can follow some basic things then you will get better results in a short period of time. 1) drink only 100-150 ml of lukewarm water during meals 2) don’t drink water before meal for one hour and one hour after meal 3) avoid oily, spicy, junk, fermented and stale foods 4) avoid late night sleeping habits 5) avoid coffee and tea…. A) take 30ml of BHRUNGARAJASAVAM and 30 ml of water and 1 TSF of jaggery after meals. B) apply equal quantity mixture of fresh lemon juice and garlic juice on the roots of hairs…. Leave it for 30-45 minutes. After that wash hairs with ROOTZ SHAMPOO (rajah ayurveda) three times in a week do all things for 30 days. These things are much more effective. THANKS. Men are always concerned about the size of their penis, and in extreme cases, this concern is also a cause of anxiety disorders. When it comes to the connection between sex and penis size, many men believe that bigger is better. But this is not the case. A large penis does not provide men with any edge, in terms of sexual pleasure. Other factors like erectile function and length of sexual intercourse play a more important role in sexual pleasure.
The very limited proof that is available on the relationship between pleasure during sex and penis size comes from research conducted on vaginal orgasm. Shampoo has the function of cleaning the scalp. Oil helps to condition the scalp. Hair fall can happen due to many reasons. It can be postpartum (after 3 months of delivery). Any acute illness like viral infections, malaria, typhoid can also cause it. It can happen in long term disorders like diabetes, high bp, arthritis. Medicines taken for all these disorders can also trigger the hair fall. It is very important to follow good diet and drink 3 lit of water/day. Oil the scalp and wash it 3/week.

Hi. It's normal to lose up to 100 hair per day, and in most people, those hairs grow back. But many men -- and some women -- lose hair as they grow older. You can also lose your hair if you have certain diseases, such as thyroid problems, diabetes, or lupus. If you take certain medicines or have chemotherapy for cancer, you may also lose your hair. Other causes are stress, a low protein diet, a family history, use of bore water, or nutritional deficiencies. Need to rule out any causative factor or underlying medical condition. Management: Take diet rich in protein like non-veg, eggs (it's loaded with essential nutrients such as proteins, vitamin b12, iron, zinc and omega 6 fatty acids in large amounts and biotin), different pulses & legumes, sprouts, nuts (sp. Walnuts and almonds); iron-rich food like green vegetables, dates, carrots, prunes, green peas; also oats, shrimp, take flaxseeds, sunflower seeds, sesame seeds, green tea, soyabean, fruits, salads, etc. Take low-fat dairy products (they also contain whey and casein, two high-quality protein sources) take proper 6-9hrs of sound sleep. Give up the habit of smoking and excessive alcohol intake. Avoid junk food and any kind of beverages (tea, coffee, aerated drinks, cold drinks, etc.) avoid canned/ tinned food or food that contains preservatives (eat fresh ones). Avoid use of hard water to wash the head rather use boiled water for head wash. Avoid use of harsh hair colour, hair treatment like rebonding, straightening, perming, etc. Avoid vigorous exercises or any supplements provided by the gym! cover your head with scarf or cap while stepping outside. Use rose wood/ any wooden hair comb to brush your hair. Regularly massage head and hair with coconut oil, almond oil (depending upon your scalp type) 1 to 3 times a week. Regular exercising is also one of the important point, but should be done under some good, qualified yoga teacher or instructor for proper guidance; it also releases the stress/ tension in the mind and the body and keeps you fit and young! Medication: Take homoeopathic medicines - Schwabe's Wiesbaden 200 & Alfalfa tonic twice a day for 1 month and revert. Use Schwabe's Arnica shampoo and Hair Growth oil regularly as per the condition of your scalp (minimum 3 times a week). After shampooing, use Bakson's Arnica hair conditioner as well after every head wash. Yes, coconut oil contains good percentage of vitamin E which is needed for healthy skin and hair.

You have to take proper treatment. Only paracetamol wont help. You can take sanjeevani. Vishamjwarantak. Godanti and giloy satwa with honey 4 times a day.

Apply maha bhringraj oil on your scalp and massage then give hot water fomentation..1-1 drop of cow ghee in both nostrils at night time before sleep....take pranacharya allergin capsule 1-1 twice a day...take lots of proteins like soyabean...egg white.....etc...avoid tea...coffee...spicy food...fermented food...junk food....apply only ayurvedic shampoo on your scalp....do pranayama early morning...

Common Reasons Behind Postpartum Menstrual Pain!

Having a child is one of the best things that can happen to a woman. While giving birth to a child is a very happy scenario, something called ‘postpartum menstrual pain’ can act as a damper. Postpartum menstrual pain occurs during the first period post-delivery. To understand it in a better way, here is a detailed overview that should help you out:

Period after childbirth
Periods usually return after 7-8 weeks of the delivery, provided there is no breast feeding. If you breast feed, then the timing of the period can vary. In some cases, a woman misses out on her periods for the entire duration of breast feeding; whereas in others, the period returns irrespective of whether she is breast feeding or not.

Once the periods start post-delivery, tampons are recommended against. Women who are breastfeeding don’t usually have their periods due to hormonal changes in the body. Hormones that induce periods are suppressed by the hormones which are responsible for the production of breast milk. The entire process of ovulation gets suppressed, thus leading to the absence of menstruation.

Difference in periods
Once your periods begin after the delivery, it will be different from the ones you have had before childbirth. As the body adjusts to the menstruation process post childbirth, one generally experiences the following symptoms:

  1. Stronger cramps
  2. Heavy menstrual flow
  3. Formation of small blood clots
  4. Pain during periods

Postpartum menstrual pain happens because of the following reasons:

  1. Pain in the vagina: The childbirth process may lead to swelling and stinging in the vagina. Painful periods will persist till the swelling subsides.
  2. Contractions of the uterus: Post childbirth uterus contractions are common as it comes back to its regular size; this may be a source of the pain.
  3. Breast feeding: Breast feeding can also trigger menstrual pain as it results in the release of ‘oxytocin’, which causes contractions in the uterus.

This problem of painful periods doesn't last longer. If you wish to discuss any specific problem, you can consult a gynaecologist.

Hello, She must get a n ultrasound done to rule out endometriosis, fibroids or adenomyosis which can cause pain during menses. To relieve she may take antispasmodics or NSAIDS.
