Orofacial Pain - Sources and Symptoms

An unpleasant experience which not only disturbs the usual routine of a person but also the psychological peace and causes extreme discomfort is called pain. When this pain involves the region above the neck, below the orbitomeatal line and in front of the ear is called as orofacial pain.

Multiple causes for orofacial pain may exist and the symptoms may include such diverse findings as headaches, neck pain, ear pain, dental pain, facial burning or stabbing sensations, and jaw joint pain.

The complaints may either develop gradually or have a rapid onset and can originate from musculoskeletal, neurovascular or neuropathic causes.

Sources of orofacial pain may include:

  1. Trigeminal Neuropathy – ant damage or degeneration to the trigeminal nerves that supplies mainly to the facial structures causes the orofacial pain. This is commonly referred as trigeminal neuralgia.

  2. Neurovascular Disorder – migraine is the most common neurovascular disorder that causes the radiating pain to the mouth eyes and other facial muscles.

  3. Temporomandibular Disorder – the major cause of orofacial pain is the temporo mandibular joint disorder which causes pain in the lower jaws. Orofacial pain may also be caused by any temporo mandibular joint surgeries and due to failure of joint replacement surgery of the jaw.

  4. Burning Mouth Syndrome – this condition is caused due to nerve damage, lack of adequate saliva production, fungal infection, certain medications and also noted for patients with diabetes. Orofacial pain is also noted in burning mouth syndrome.

  5. Cervicalgia – pain due to spinal cord injury, damage to spinal nerve, muscle and ligaments may radiate to face and cause orofacial pain.

  6. Trauma – accident is the most common cause of temporomandibular disorder and other spinal disorder that causes orofacial pain.

  7. Sleep disorders – sleep disorders include grinding or clenching of the teeth during sleep may also cause orofacial pain.

Symptoms

Though orofacial pain is caused by several factors, its symptoms includes dental pain, ear pain, neck pain, headache, pain in the jaw joint and stabbing sensation over the face. Some patients may also experience burning sensation over the face. Other symptoms include dizziness, insomnia, tinnitus (abnormal ringing or roaring sound in ears), poor muscle coordination, abnormal itching over the face and neck and tingling sensation. If you wish to discuss about any specific problem, you can consult a Dentist.

Hi. Orthodontic treatment would be best treatment for him as natural and normal tooth structure at their contacts will join each other. Their are ratios between upper and lower teeth which has to be matched and clip or orthodontic treatment matches it. Otherwise you can also go for aesthetic composite build up or crown placements. They are also good but braces will be best option.

U need to have local application gel in mouth and if its on your gums as well, you need tray to keep medicine in its contact. Along with this, you may require systemic (oral) drug as well. And as it is autoimmune condition, it may not heal fast / completely but you can have definite relief from burning or other problem.

It may be possible that your previous filling is dislodged and there is decay in that tooth. Go to your dentist he will take a xray and may suggest a root canal treatment of that particular tooth.

Healthy gums equals to a healthy mouth; exercising for healthy gums is the way to go! Gently bite down so as to make sure that your teeth make a clinking sound when they meet. Repeat this 30 to 40 times continously. This exercise will stimulate the flow of blood and will keep your gums healthy.

Hello doctor , my self sudeep , I had made a teeth bridge just appro one month ago , today when I was having my lunch in afternoon I could not see the bridge , I have a doubt if I have swallowed it. It is a ceramic teeth, will there be any issue if its in stomach , I do not feel anything right now , I mean I do not feel any problem now, please let me know, I was having paratha and water also , do not know if I have swallowed it.

First of all make it clear that the bridge is in place or not. If you are in doubt get examined by dentist. You can also see if bridge is for missing teeth than you will have space left out if by any chance bridge swallowed. Anyhow if you doubt its swallowed it will get passed in stool. But if you fell any symptom like pain get radiographic examination done.

We need more investigations with clinical examination to decide upon treatment. You may need deep cleaning along with surgical curettage or gum strengthening procedure. Rinse your mouth thoroughly with a mouth wash after every meals. Use pepsodent tooth paste & gum paint for 2 to 3 weeks.

Lybrate-user orthodontic treatment is not at all painful except for some discomfort from time to time when there may be adjustments to the wires. You are able to eat practically everything but with some caution so as not to break the braces, which if it happens ends up increasing your treatment time.

I used the toothpaste and sugar test though they do not have guarantee but I used them there was no changes in colour in the toothpaste and the sugar dissolved in the urine. No any pregnancy symptom except missed period and little cramps with white mucous discharge. Am I pregnant?

You should use pregnancy test kit to test pregnancy. Toothpaste and sugar tests are not medically advised.

A very nice question indeed. Yes, consumption of a lot of citrus food and drinks can lead to erosion of you enamel surface. Enamel is the outer protective white colored layer of your teeth. And consuming lot of citrus juices erode the superficial layer of enamel exposing the underlying dentin (part of tooth having sensation) leading to major tooth problem of tooth sensitivities. Although this erosion process is slow but in addition to you daily wear of enamel due to ageing process will definitly speed up your side effects. So I would advice you to go for weight with proper exercise and a balanced diet and avoid taking such short cuts. Get some muscle movements.

First go for deep scaling (professional tooth clening) and then I advice you gum paint n mouthwash that keeps your gum and teeth healthy.

Gutkha chewing habit may lead to a potentially malignant condition known as OSMF. You have to quit the habit ASAP. Try nicotex or nicorette chewing gum. If mouth opening is reduced you have to personaly visit an expert MDS dentist in oral medicine.

•Eat a healthy, balanced diet and avoid eating strongly flavoured or spicy food. •Cut down on sugary food and drink, as it can increase the amount of bacteria in your mouth. •Reduce your alcohol consumption. •Stop smoking. •Cut down on coffee. •Drink plenty of water to help prevent your mouth becoming dry. •Chew sugar-free gum after eating, to stimulate the flow of saliva. This will help clean away any remaining food particles. •Don't use toothpaste to clean your dentures, as it can scratch the surface and cause stains. •Clean your dentures thoroughly using soap and lukewarm water, denture cream or a denture-cleaning tablet. •Use a separate toothbrush to clean your dentures. •Make sure you visit your dentist for regular check-ups. Having regular dental check-ups will ensure that any plaque is removed from your teeth, particularly in areas that are difficult to reach. •Your dentist can recommend the best way to clean your teeth and gums, and point out areas you might be missing. They can also identify any signs of gum disease and ensure early treatment. •Gastritis, lung & sinus infection may also be the cause of bad smell in the mouth. 90% of bad breath is due to a dirty tongue. •If your bad breath is caused by a gastrointestinal problem, such as an H. Pylori infection or gastro-oesophageal reflux disease (GORD), you may be referred to a gastroenterologist. •Persistent bad breath or a bad taste in the mouth may be a warning sign of gum (periodontal) disease. Gum disease is caused by the buildup of plaque on teeth. •You may need deep cleaning & fill tooth cavities with artificial enamel filling. Rinse your mouth thoroughly with a mouth wash until then. Advance procedures can be done with laser too.
