HELLO, If your menses do not arrive in next few days then you may opt for a withdrawal bleed with progesterones to resume menses, once pregnancy has been ruled out.

Injuries - Occupational Therapy Always Benefits You!

Disabilities can occur due to injuries, infections, or old age. Whatever the reason, the person has limited movement and suffers from pain and stiffness during this period. In addition to surgery and antibiotics, occupational therapy (OT) helps the patient immensely by providing complete rehabilitation and to help recover completely. Many who do not respond well to medications have benefited from occupational therapy and have had complete functional recovery. The occupational therapist helps patients to improve and maintain skills so they can perform their daily chores on their own.

The benefits of occupational therapy are too many and would depend on the setting in which it is used:

  1. In the elderly: With age, general movement is reduced and OT can help improve overall movement and quality of life, thereby enabling them to lead more independent lives.
  2. In mentally disabled: Occupational therapists work with people with emotional and mental limitations and teach them the necessary skills to be able to perform their functions in society. This can even enable them to take up a job and lead healthy, positive relationships.

  3. Helps the body heal naturally: The body is not being subjected to any external chemical, and only natural movements and exercises are done, which is beneficial for the person.
  4. May reduce or eliminate muscle spasms: Pain and disability can lead to reduced mobility, which can cause muscle spasm. OT can help reduce and in some cases completely reverse the muscles spasms that may have set in. Can be supplemented with thermal modalities and treatment to reduce pain: Adding heat packs and moist heat to the regimen will produce better results.
  5. Treatment is customized for the individual: Each person is different with the cause and the effect of disability. The therapist will customize the program based on the person's needs and therefore will benefit the patient.
  6. Can boost self-esteem and promote feeling of well-being: The mere thought of being independent gives a big moral boost to the affected person, thereby improving quality of life.

Causes of Soreness in Vulva and Abnormal Discharge in Adolescent Girls

Pruritus Vulvae is the most common cause of vulvar soreness and abnormal discharge in adolescent girls. The growing years are a transition phase where most body parts are undergoing changes to reach the adult stage. While some changes would be obvious externally, there are a lot of internal changes, which would not be very apparent. And for growing girls, especially the changes that they see in their private parts are worrisome, sometimes unpleasant and uncomfortable. About 50% of them would not even discuss these with someone to avoid embarrassment.

Some amount of vaginal discharge is normal and contains bacteria, which are essential for a normal genital health. It also provides lubrication and keeps the area clean. However, the amount, color, and type of discharge can vary and is a good indication of any underlying abnormal condition. Most genital tract issues manifest with a change in the vaginal discharge along with irritation or soreness.

In adolescent girls, the most common problem is pruritus vulvae, which is itching of the vulva region. It is a variant of contact dermatitis caused by irritation. In the growing years, the vulva is thin and delicate with a neutral pH. This makes it easily irritable, unlike in mature women, who also have the protection of pubic hair, thick skin, and labial fat pads. This irritation leads to the condition called pruritus vulvae.

Causes of soreness:
These are primarily non-infectious and infectious (genital).

Noninfectious causes:

  1. Poor hygiene leading to irritation, soreness and itching
  2. Contamination with urine or feces
  3. Mechanical irritation from itching, use of toilet paper, ill-fitting undergarments.
  4. Chemical irritation from soaps, detergents, shampoo, chlorinated water, etc.
  5. Foreign body such as toilet paper leading to vaginal discharge can cause vulval irritation
  6. Skin diseases like atopic dermatitis, seborrheic dermatitis, or psoriasis presents with vulvar symptoms too in the form of soreness and itching

Infectious causes:

  1. Sexually transmitted infections like trichomoniasis, gonorrhea and chlamydia.
  2. Viral infections like genital herpes and genital warts.
  3. Fungal infections such as Candida such as and tinea are rarely the cause of pruritus vulvae in prepubertal females.
  4. Worms like pinworms which move from the anus to the vagina, causing itching
  5. Bacteria like Shigella, Yersinia and E. coli can also lead to non-sexually transmitted infections and subsequent symptoms like soreness and discharge.

Prevention/Management:

  1. Awareness is the first step in prevention. The girl must be educated about the changes her body is going through and the reasons behind it.
  2. Hygiene habits, especially after urination or passing feces, should be enforced.
  3. Avoid undergarments that are tight fitting and can be irritating
  4. Avoid soaps/chemicals when cleaning the genital area
  5. Topical creams (emollients/hydrocortisone) are usually effective
  6. Infections would need a course of antibiotics

When the girl is complaining of soreness and itching, it is best to visit a doctor to get a thorough examination and manage accordingly.

HeLlo doctor. After my 1st intercourse with my husband I'm bleeding for 3 to 4 days. Before 15 days I got my menses cycle. But now again I'm bleeding. The bleeding was not so high. I'm unable to understand whether it is menses or something else. Due to this I'm afraid to participate in sex. Can I participate in sex normally or else I need to wait until bleeding stops.

See with first intercourse, hymen which is the covering membrane to the vagina is torn and that's why bleeding occurs. You can definitely have sex when you have minimal bleeding. It is definitely not menses.

Hi, I am married female my periods were irregular after marriage, but my last periods were on September 25th till today I didn't get periods and doctor advised me to take pregnancy test but it showed negative. I don't why I am not getting periods. My rubella antibody was only 0.8. Is there any chance for not getting periods because of rubella antibody?

Miscarriage is a spontaneous abortion that occurs around the first to second trimester of pregnancy; wherein the foetus is automatically expelled from the uterus due to its inability to survive because of environmental influences or chromosomal abnormalities.

Unless the miscarriage occurs due to a severe health issue, the fertility of the woman may not necessarily be compromised. Studies show that one miscarriage doesn’t predict the possibility of a future one. Once the bleeding and pain subside, it is the emotional trauma that must be addressed to begin with which poses a greater threat than the physical one.

The fertility of the woman remains unaltered and the body can be ready after the menstrual cycle returns to normal. However, it still requires some time for the woman to be physically and emotionally prepared to go through another pregnancy. The medical history and reasons behind miscarriage must be evaluated before the couple decides to try again.

Complications after miscarriage only occur if the woman is above 35 years of age or has a rather unhealthy lifestyle. The concept of fertility varies from person to person and it isn’t possible to pass a common judgement about fertility in this scenario.

The gynaecologist mostly suggests couple therapy after the trauma of the miscarriage. The psychological loss is given more importance before considering another attempt by most doctors. The fertility loss only occurs with age and unhealthy lifestyle and it is widely accepted and proven that a miscarriage has little or nothing to do with it.

Ways to Increase Fertility

If a couple is certain they are ready to conceive again, there are a few things a woman can do to help her chances of becoming pregnant-

  1. The first thing to do is to stop any unhealthy habits. These include smoking, drug use, or alcohol abuse. It’s reported that high consumption levels of caffeine are detrimental to conception so that should also be limited.
  2. Eliminating stress can also help. Eliminating stress is one of the most important factors in having a successful pregnancy. So, it makes sense that getting over-stressed can reduce fertility levels.
  3. Eat healthy. If depression was experienced due to the miscarriage, it’s possible that the woman’s diet has not been the healthiest. It’s common for women who have experienced a miscarriage to have a loss of appetite and not eat enough or to eat out of sorrow, eating too much. More attention should be paid to the diet and more healthy foods added to meals.

Talk to a Doctor

If a couple is having issues with fertility after early miscarriage, they should talk to a doctor. There are many options available. More than likely, the first thing the doctor will do is to test fertility levels of both the man and the woman. If you don't want to visit a doctor for this, there is a new at-home kit available called Fertell. This test, retailing for approximately $100, measures fertility levels for both a man and a woman.

If it is found that there is a loss of fertility after early miscarriage, fertility drugs or therapies may be prescribed for a period of time.

The doctor may report that there are no issues with fertility. At this point, it would be a good idea to use an ovulation predictor in order to time effective intercourse. This isn't the most romantic idea, but it may well be one of the most logical ways for a couple to conceive.

Bottom Line

Fertility is not normally decreased after miscarriage. A woman's fertility levels return to normal as soon as her menstrual cycle returns. Fertility may be decreased due to emotional issues as a result of the miscarriage. However, if a couple is ready, both physically and emotionally, there should be no physiological hindrances to conception.

The decision to try again is not an easy one. However, fertility won't typically be an issue.

Have more of foreplay. When she is fully aroused n well lubricated, insert your index finger gradually. Continue 4 couple of days n then try inserting 2 fingers. Give her enough time n do not force. Or else she ll withdrew herself.

Before going in for details of health, disease and cure, few points are to be understood in proper perspective as we are dealing with the living system. Every living system is comprised of one or more units - called the cell. Every cell contains the living substance. The essence of the living substance is that it differs chemically from the surrounding medium and yet maintains a DYNAMIC EQUILIBRIUM (HOMEOSTASIS) with its environment. 

Homeostasis occurs in all living cells and in the fluids and organ systems of the multicellular organisms. Referring to mammals, the 19th-century French physiologist Claude Bernard coined the expression "constancy of the internal environment is the condition of life. The American physiologist WB Cannon, who first used the word "HOMEOSTASIS" referred to " the system of checks and balances which maintain internal constancy as the wisdom of the body." 

Homeostasis In Health 

Living systems may regulate its internal constancy despite an altered environment. And such regulation is achieved by hierarchies of controls, checks & balances, alternate mechanisms, negative and positive feedbacks etc as environmental stress is applied. 

Health Is Ease Maintained
In other words, when homeostasis is maintained at its optimal best i.e. integral harmonious performance of the vital functions tend to the preservation and normal development of the organism and there are no abnormal sensations and functions, it is termed health. 

World Health Organisation Defines Health as "HEALTH is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity." 

Homeostasis In Disease 
The living system may alter a given property with reference to the environment; the property conforms to the medium and homeostasis consists in cellular adjustments, such that metabolism continues in the altered state. So disease is an altered state of homeostasis and is attended by abnormal sensations and functions in the form of signs and symptoms. 

Disease Is Ease Disturbed i.e. DIS – EASE On one extreme, even a single named disease may vary in degree of intensity, frequency and pathology according to a relative abnormality of homeostasis; while on other extreme a single individual can suffer from many diseases also. "So health and disease are interconvertible and are two faces of the same coin." 

Homeostasis In Cure 
The cure is reversal of the altered state of the homeostasis to its normal pattern. It is not mere mitigating the signs and symptoms but to reach the optimal best state of homoeostasis i.e. health. As march from health to disease is relative, the restoration of disease to health also occurs in percentages or degrees. Health Is Ease Restored!

Men have a very important role in the process of pregnancy and child birth. It's challenging for fathers to be actively involved in pregnancy although no one doubts the fact that some of them become super dads while raising their kids.

  • Research shows that Fathers are needed: Studies have proved that infants whose fathers had minimal involvement in pregnancy were more likely to be born premature and with low birth weight. Sometimes your physical and emotional presence is all that is required during pregnancy. So here let's have a look for role of dad in pregnancy and prepare quarterly roster.
  • Be prepared to welcome the little one: Planning to start a family is a water shed moment for parents and the delivery of the ''Good news" certainly brings along mixed feelings of excitement and nervousness. It's like an examination schedule that has been pinned on the notice board. Husbands have a calming effect on moms in managing morning sickness. They have the responsibility to encourage her for a balanced and nutritious diet apart from planning antenatal visits to obstetrics clinic. They also take care of pregnancy mood swings of her partner because it's the hormones that are affecting and also be conscious and sensitive while making love in pregnancy.
  • Be prepared for all circumstances: Dads need to be prepared to handle the odd symptoms of pregnancy and should find out about what they have should do. They can get references for mood related aspects. You can support the health of your partner by cooking and eating healthy meals together and exercising together. Prepare yourself for moving to a new place. Make a list of the hospitals that are not far and stay in touch always with your wife as it could be bit of emergency anytime. Plan for hospital stay and be a helping hand. Also try and engage your own parents so that they can also be a part of your training for baby care. Attending child birth classes together helps a lot to prepare you for arrival of new born.

Do get actively involved in labour and cord cutting also a father has a lot of role in successful breast feeding. It is best that you get more involved with your wife in the 9 months. The anticipation of a new one coming to your house should grip your imagination. Remember for many parents, it is the best time of their life's.

8 mm endometrial thickness is required for implantation of embryo. We have seen many patients with maximal endometrial development of only 4 to 7 mm successfully achieve pregnancy. Consult a gynecologist as they may prescribe you some hormone supplements for the same and can advise possible alternative methods of treatment.
