My son is a adhd kid. He has some speech delay also. What is the best way to treat him. Are there any special schools for him.

C.S.C, D.C.H, M.B.B.S
General Physician,
Stimulants such as methylphenidate and amphetamines are the most common type of medication used for treating adhd. Although it may seem counterintuitive to treat hyperactivity with a stimulant, these medications actually activate brain circuits that support attention and focused behavior, thus reducing hyperactivity. In addition, a few non-stimulant medications, such as atomoxetine, guanfacine, and clonidine, are also available. For many children, adhd medications reduce hyperactivity and impulsivity and improve their ability to focus, work, and learn. Medications also may improve physical coordination. However, a one-size-fits-all approach does not apply for all children with adhd. What works for one child might not work for another. One child might have side effects with a certain medication, while another child may not. Sometimes several different medications or dosages must be tried before finding one that works for a particular child. Any child taking medications must be monitored closely and carefully by caregivers and doctors. Stimulant medications come in different forms, such as a pill, capsule, liquid, or skin patch. Some medications also come in short-acting, long-acting, or extended release varieties. In each of these varieties, the active ingredient is the same, but it is released differently in the body. Long-acting or extended release forms often allow a child to take the medication just once a day before school, so he or she doesn't have to make a daily trip to the school nurse for another dose. Parents and doctors should decide together which medication is best for the child and whether the child needs medication only for school hours or for evenings and weekends, too. For more information about stimulants and other medications used for treating mental disorders, see the nimh mental health medications webpage. The food and drug administration (fda) website has the latest information on medication approvals, warnings, and patient information guides. What are the side effects of stimulant medications? the most commonly reported side effects are decreased appetite, sleep problems, anxiety, and irritability. Some children also report mild stomachaches or headaches. Most side effects are minor and disappear over time or if the dosage level is lowered. Decreased appetite. Be sure your child eats healthy meals. If this side effect does not go away, talk to your child's doctor. Also talk to the doctor if you have concerns about your child's growth or weight gain while he or she is taking this medication. Sleep problems. If a child cannot fall asleep, the doctor may prescribe a lower dose of the medication or a shorter-acting form. The doctor might also suggest giving the medication earlier in the day, or stopping the afternoon or evening dose. Adding a prescription for a low dose of a blood pressure medication called clonidine sometimes helps with sleep problems. A consistent sleep routine that includes relaxing elements like warm milk, soft music, or quiet activities in dim light, may also help. Less common side effects. A few children develop sudden, repetitive movements or sounds called tics. Changing the medication dosage may make tics go away. Some children also may have a personality change, such as appearing" flat" or without emotion. Talk with your child's doctor if you see any of these side effects. You can ask for special schools in your area.

My brother is having a stammering problem from his childhood. Now,he is 25 years old and is a doctor. But that problem is not gone. We have shown him to the ENT doctors and they have concluded that there is no defect in his throat. That problem has taken a mental illness and he is under total depression and not ready to do further studies and always remains in angry mood. We have also shown him to the speech therapists but it is of no use for his problem. How can his problem be tackled efficiently and effectively?

Stammering could have neurological, medical or psychological causal factors. Consult a neurologist first and get all investigations if it is a neurological problem. Sometimes, due to anxiety also people have trouble like this and stammering is the result. For psychological reason, consult a clinical psychologist who may use behavior therapy principles in its treatment management. Lastly, I would also like to tell you sometimes due to such problems, people develop depression and stress as they have difficulty in normal conversation and they feel frustrated. So dealing with frustration is also important point to be kept in mind in their treatment option.

Have breath problem and low voice due to vagus nerve tumor surgery. What is the treatment options.

PDDM, MHA, MBBS
General Physician, Nashik
Breathe deeply and exhale on a hisssssssing sound. Repeat 10 times. Proper breathing is the foundation for a healthy voice AND control over nervous energy that can make the voice quiver. Say “Mm-mmm (as in yummy) Mmm-hmm (like yes) ” Repeat 5 times. This develops mask resonance, which creates a clean and vibrant sound by creating a clean approximation of the cords and a resonance that will sound great and project easily. Say “Mm-mmm. Mmm-hmm.” up and down your vocal range, from low to middle to high and back again, 10 times.

Dear Lybrate user. In many of the cases Stammering has two important reasons. Vocal chord misalignment or other problems are the primary reason. For this vocal chord complaint, you should consult an ENT specialist and get it corrected. Second reason is purely psychological. Stammer therapy can help you in psychological reasons. Take care.

Just observe her hearing ability, mental ability and social interaction. If all is normal then hold on for few months. Just do some speech and language stimulation with books and toys.

Vocal Cord Lesions - Causes + Treatment

When the vocal cords suffer from the accumulation of non-cancerous and abnormal growth, these are known as vocal cord lesions. These lesions usually cause a variety of voice and speech related problems. These are also known as singer’s nodules and nodes, which may be caused by pressure, stress and stretching of the vocal cords. Women between the age of 20 to 50 are more prone to this condition. Here is everything you need to know about the types, causes and treatment of these lesions.

Types: There are three kinds of vocal cord lesions, that is nodules, polyps and cysts. A vocal cord polyps may be caused on one side of the vocal cords and may also lead to many disturbances in one’s voice. Another type of vocal cord lesions includes a mass of tissue that may manifest in the sac or membranes around the vocal cord. This cyst can be on the surface of the vocal cord or in the ligaments of the vocal cord. This may require surgical removal as well as voice therapy thereafter. Nodules are the third type of lesion that may form in the vocal folds. These are basically calluses that may disappear once the pressure and overuse of the vocal cords diminish.

Causes: These lesions are usually benign or non-cancerous growths that may be caused due to a variety of reasons. Screaming and overuse of vocal cords are few of the most common reasons for the eruption of such lesions. The heavy use of one’s voice can cause quite a bit of trauma to the vocal cords and may lead to problems including lesions and disturbances in one’s voice. Conditions like reflux and allergic reactions may also cause vocal cord lesions as a side effect.

Symptoms: From hoarseness to pain, there are a number of symptoms caused by this condition. A low pitch and difficulty while speaking are also some of the common symptoms of this condition. Patients suffering from the growth of such lesions will also find themselves making an effort in order to speak normally and to be heard. Constant throat clearing may be required as well.

Treatment: Besides giving your voice a rest and gargling warm saline water with salt, one can also have the condition checked by the doctor so as to ascertain whether or not it is a serious lesion that may require surgical removal and voice therapy. The doctor will usually prescribe medicines in order to deal with any underlying conditions that may have caused the lesions, including allergic reactions, reflux, sinusitis and other such ailments. One will also have to  stop smoking so that there is less chemical pressure on the vocal cord. Finally, one will have to go through voice therapy for more serious lesions. If you wish to discuss about any specific problem, you can consult a doctor.

Hi! My dad is 63 years old. So this problem started last August, when we noticed that he is having some speech issue. It was like as if he is eating something and talking, when actually his mouth was completely empty. Then the frequency of this slurring increased in some weeks/months (can not recall). When we met our doctor, he advised to get an MRI done. On getting the MRI he concluded that it is because of diabetes and blood pressure that some muscles of the tongue are weak. Since november, my dad is controlling diabetes and blood pressure and has also lost 10 kgs (he was overweight, with strict diet and exercise he has lost 10 kgs and now weighs 80 kgs) So, the slurring problem increased and now its not slurring but lisping. We also noticed that his tongue is not able to touch the roof of the mouth. And there is always some burning sensation on some part of the tongue. We met a few renowned doctors but nothing was concluded. We met a neurologist, who recommended the following tests. MRI, diffused and contrast RNT THYROID ECHO ULTRASOUND OF THE NECK. All the above tests were normal. On meeting our neurologist the last time, he has given the following conclusions. He said - His tongue muscles are weak, And blood supply to the blood is not appropriate. And has given us some medicines. Although on his prescription he has written the following. D/d Multi infarct state e bulbar palsy Bulbar palsy onset AHC He has also written follow up to see for evolution/ evaluation of AHC disease. As you can see what he said and what he wrote was completely different. My question is I did my research on Ahc and bulbar palsy, yes it is very scary but some symptoms are not matching with the symptoms of Ahc and bulbar palsy. He also said - there is no test to conclude this disease, if nothing happens after having the given medicines. We will conclude that he is suffering from Ahc and bulbar palsy. Symptoms that differ His walking is perfect - daily walks for more than 7 kms His muscles are strong No problem with swallowing Please help me out in this.

Dear lybrate user, you can try consulting a dentist who hs done masters in oral medicine and radiology or in oral pathology. With aging tongue muscles tend to become weak n also mouth gets dry making them difficult to speak properly or swallow. Moistness of mouth also plays a role in speech. Wen a person loses mutiple teeth, the space available for tongue increases n tongue seems to noticably increase in size in older patients. This again causes difficulty in speech. With aging, weak muscle n nerves coupled with slight increased side tongue n absence of muliplt teeth or complete absence of teeth may cause incomplete control over tongue. Taste buds greatly reduce in number with aging causing bald tongue. Nutritional deficiency esp iron deficiency can cause bald tongue n inflamation easily.

If she can speak English language without any problem of pronunciation and only problem is with few hindi syllable then she probably do not require speech therapy, she requires attention and correct repetition of those words and monosyllables. Motivate child to speak slowly and reinforce her when she speaks clearly. As a parent you try to speak with her as slowly as possible. Please ask her teacher also to do the same in class.
