Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent neck pains. If you have localized neck pain with no other symptoms like radiating upper limb pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your neck while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of neck region and few blood tests for evaluation. Tips for backache: heavy objects should be pushed rather than pulled. The weight of the objects should be taken in your bent legs and arms and not in your back. During an acute attack sleep with a pillow under the knees. Choose shoes that offer good support in the arch and have low, but not flat, heels. Prop up one foot on a box or low stool when you're standing for a long time. Use a chair that provides support for your back, or tuck a little pillow behind your lower back.

Any muscle or group of muscles are susceptible to anxiety and stress caused tension and pain. Once the muscles are strained, they can spasm and'lock up, which can cause even greater pain and immobility, such as that commonly associated with chronic back pain. The long-term solution to anxiety and stress caused chronic back pain is reducing your anxiety and your body’s stress sufficiently so that your body stops exhibiting chronic back pain symptoms. Some short-term remedies that might also help alleviate chronic back pain: Anything that reduces your body’s stress can help, such as deep relaxation, rest, and getting good sleep. Regular light to moderate exercise can also help loosen tight muscles, such as those responsible for chronic back pain. Heat (heat causes tight muscles to loosen). Having a warm bath. Have a massage. Vibrator on the back (vibration can help loosen tight muscles and stop muscle spasms). Avoiding stimulants (stimulants stress the body). Muscle relaxants. Gentle/mild stretching. Slowing down a hectic schedule or lifestyle. Taking some time off so that your body can reduce its stress. And so on. Again, anything that can help reduce the body’s stress and relax muscles, can help alleviate chronic back pain.

You may be having bad posture always sit straight, avoid forward bending learn back strengthening exercises from physiotherapist and do it regularly take more milk and cured in your diet and more protein in your diet.

Osteoporosis - How It Can be Treated?

Your bone is a living tissue that constantly undergoes the cycle of breakage and replacement. Osteoporosis is a condition wherein the rate at which new bones form cannot match up to the worn out bones, thus making the bones fragile and brittle. Osteoporosis mainly affects the wrist, hip or spine and can result in a stooped posture accompanied by height shrinkage and back pain caused due to a collapsed vertebra.

Reasons Behind Osteoporosis

  1. Hormones: Lowered estrogen count in women and testosterone in men as a result of ageing or other treatments can result in this condition. Excess thyroid secretion in the body, an overactive adrenalin gland or parathyroid gland can also accelerate bone loss. Women are more vulnerable to osteoporosis during menopause.

  2. Prolonged use of injected: Prolonged use of injected or oral corticosteroid medications such as cortisone or prednisone messes with the bone-building process. Medicines used to combat cancer, gastric reflux and seizures can be responsible as well.

  3. Medical Conditions: Certain medical conditions such as Rheumatoid arthritis, cancer, liver or kidney disease increases the risks of acquiring osteoporosis.

  4. Sedentary Lifestyle: A sedentary lifestyle with not much physical activity along with an excessive consumption of alcohol and tobacco makes you susceptible to osteoporosis.

Treatments for Osteoporosis:

  1. Medication: Commonly prescribed medicines for remedying fractures include bisphosphonates such as Zoledronic acid, Ibandronate, Risedronate and Alendronate.However these medicines are not free of side-effects such as abdominal pain, nausea or heart burn. In case, your regular medicine doesn’t work, you can use Denosumab and Teriparatide as alternatives but only after consulting your physician.
  2. Hormone Therapy: Estrogen therapy for women after menopause helps sustain bone density. However such procedures can escalate the dangers of getting afflicted with cancer, heart diseases or conditions such as blood clotting.
  3. Alternative Medicine: Ipriflavone, a laboratory manufactured product, when combined with calcium can curb chances of bone loss and alleviate pain.

Do hot fomentation keep a pillow under the legs at night &/when resting. Any way take ibuprofen 200mg od & sos x 5days you will need other supportive medicines also. Do knee exercises it may have to be further investigated make sure you are not allergic to any of the medicines you are going to take. You will need other supportive medicines also. I get imbalance feeling/ not standing still/ bouncy feeling with exertion/ activity, bending over, standing and working for sometime. It does not happen when I get up in the morning but as day goes by. It causes tiredness and I need to sit down. This goes away after sleeping/ taking rest. What could be the reason. I have cervical spondylosis, (pain in the shoulder blade/ base of neck, crackling sound when moving neck) and eardrum perforation from long time. Bp normal range, pls guide.

Quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Treated with medications, still I am having difficulty in raising up and articulating my right arm. Well, you have rotator cuff tear. It happens in this age group commonly. Most doctors don't take it seriously. You need specialist care. Shoulder specialist orthopedic surgeon. Even general orthopaedic surgeon may ignore it. If its cuff tear because of trauma, you need to get mri with 3 tesla machine at the earliest. Longer you weight, worse is outcome if it's cuff tear.

Reasons can be-* cold weather *sitting in inconvenient posture constantly, without changing your position or without movement, *wearing ill fitting sandals, *lack of exercise or *internal disorder like arthritis. Point to observe- * if you have difficulty in climbing stairs, take lift while going upstairs. If there is no lift, go up slowly. * if it is due to strain of games or injury, take rest. * if it is due to obesity, reduce weight. * if it is due to constant sitting at a particular posture, change posture and engage in slow walk. * walking regularly at slow pace without unduly straining helps as it improves blood circulation around knee and eases the joint. *you can use mustard oil for massage which is good in winter season for relieving pain. Eucalyptus oil is also good homoeopathy has a good treatment for this without adverse effects. Take bryonia 200---twice daily morning and evening and give feedback after 7 days for further treatment, consult privately (prime-rs. 149/).

Goodmorning mam I have understood your question. Firstly you develop a good posture while sitting standing. Then when your sleeping make sure you put a pillow under your knees. That will reduce the stress and load on the spinal column. Secondly if your pain continues please go in for a session of ift treatment physiotherapy for your back. Please avoid extension of back. You should make sure of taking adequate rest for your back too. Do not worry your pain will come down slowly with all those precautions.
