Understanding postpartum depression- Women's day special

Understanding postpartum depression- Women's day special

Many women have mood swings right after childbirth. They can be happy one minute and sad the next. Even when their baby is asleep, they may have difficulty sleeping, eating and feel a little depressed. If these symptoms begin a few days after delivery and go away after 7-10 days without treatment, they are in all likelihood the 'baby blues,' a short-lasting condition that 50-80% of women feel and that usually doesn’t require medical intervention. Clinically diagnosed postpartum depression, however, is another story.

Postpartum depression affects 10-15% of women anytime from a month to a year after childbirth. Women with postpartum depression may feel restless, anxious, sad or depressed. They may have feelings of guilt, decreased energy and motivation, and a sense of worthlessness. They may also have sleep difficulties and undergo unexplained weight loss or gain. Some mothers may worry about hurting themselves or their baby. In extremely rare cases less than 1% of new mothers-women may develop something called postpartum psychosis. It usually occurs within the first few weeks after delivery. Symptoms may include refusing to eat, frantic energy, sleep disturbance, paranoia and irrational thoughts. Women with postpartum psychosis usually need to be hospitalized.

Researchers aren't sure what causes postpartum depression but think that the dramatic shifts in hormone levels during pregnancy and immediately afterward may result in chemical changes in the brain leading to the condition. Childbirth is also a major life change that can create ongoing stress and contribute to depression. The new mom's responsibility for the baby, the household and her work duties upon returning after maternity leave may affect her risk of getting postpartum depression.

The good news is that, like diabetes or heart disease, postpartum depression is an illness that can be successfully treated with medicine and therapy. Women treated with antidepressant medicines and talk therapy usually show marked improvement. Depending on the type of medication they're using, they may be able to continue breastfeeding.

Researchers are making progress in understanding how changing hormone levels and other factors affect the brain after childbirth. They hope to develop better medications for treating postpartum depression by targeting the chemical pathways they're uncovering.

If you suspect that someone you know is suffering from postpartum depression, it's important to show understanding and support during this stressful time to help the new mom avoid the stigma, shame and isolation often associated with postpartum depression.

Normal pregnancy lasts for 36 to 40 weeks. And the mode of dilivery is best decided according to the clinical situation at time of labour.

I strongly advocate breastfeeding babies till at least 2 years of age, even later if required. It is never late to relocate and re-establish breastfeeding even if your baby has had a gap for a while. I am a lactation consultant, and I can help you with re-establishing breastfeeding. Let me know if you need my help.

How to Know If You are Getting a Heart Attack?

Sr. Fellowship in Electrophysiology & Pacing, M.R.C.P.(U.K.), DM - Cardiology, MD - General Medicine, MBBS
Cardiologist, Delhi
How to Know If You are Getting a Heart Attack?

Heart attack or myocardial infarction mostly occurs when the flow of blood to the heart is blocked due to one or more reasons. The most prominent reason could be high fat or cholesterol that results in plaque build up in the coronary artery. The restricted blood flow has the potential to damage or even destroy the heart muscle. Some of the symptoms that can be helpful when it comes to the identification of a heart attack are listed below.

1. Discomfort in the Chest - Your chest may feel tight and pressured when you are about to be hit by a heart attack. Pain or discomfort in the chest is usually central typically felt behind the sternum, the central bone of chest. It may not be pain, it may be only discomfort, it may only be in upper part of tummy, very commonly confused with a gas problem. It is often accompanied by a painful sensation in the chest or arms. If a chest pain lasts more than 15-20 min it can be assumed to be a heart attack. This aching sensation may even spread to your back, neck or jaw.

2. Feeling of inconvenience - Often before the heart attack, the person may have feelings of nausea, ingestion, abdominal pain or a heartburn.

3. Breathing problems - Another prominent symptom of heart attack is shortness of breath which is accompanied by cold sweat and fatigue.

Emergency Measure:

If you think you are getting a heart attack, you should immediately take one tablet of Aspirin, 4 tablets of a drug called Clopidogrel, and one tablet of Atorvastatin 80 mg and rush to the nearest possible medical facility as time is very important in the treatment of an acute heart attack.

'Consult'.

Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. If you are bleeding from longer time please get your haemoglobin tested first. Also get her examined by a gynecologist and do ultrasound. Heavy menses can be treated with 3 months course of medicines.

Don' t worry. It can happen sometimes. Don' t be confused. Straightaway, consult doctor and find out if there is any difficulty with you or your wife. As you already had a child, the chance of success is very high.
