I am 62 year retired male I got 3 cm malignant tumor in the rectum just above the anal verge. It is localised according to reports of ct and colonoscopy doctors are saying that it is t3n0m0 stage and highly curable. But doctor suggested for a direct surgery i.e apr and if there is a further need of chemoradiation can be taken. So I went for a second opinion and got advised that first I have to take chemoradiation for 35 days after that a rest of 4 weeks and then go for surgery. Now I am confused about what to do adjuvant therapy or neoadjuvant therapy.in both cases a permanent colostomy is advised coz tumor is just above anal verge.

Dear lybrate-user, please DO NOT get your surgery done. FIRST is CHEMORADIATION. Please assess with MRI if sphincter can be preserved. There are ways to prevent your sphincter avoiding permanent stoma. Do not get your APR done unless you receive CHEMORADIATION FIRST. After that MRI to be repeated and reassessed Regards.

Anal fissure is a common disease, treated by general surgeons. It requires sitz baths, some local intra anal ointment application, anti inflammatory drugs, treatment of constipation etc. Sometimes surgery may be required if severe condition or if medical treatment fails.

In August I woke up with very sharp pain and I had to go Emergency Room. They did CT scan and said I've 5mm kidney stone. Followed up with urologist who said without examining me that I probably passed kidney stone. After 1 week I had another sharp pain which I ended up taking very strong pain killer.

I am feeling to urinate frequently and after doing that still i am not satisfied.Many tims i feel a lot of pain in urinating. Always there is a pain. Also i am feelin pain in left side of stomach in reguler interval.

Madam, Your complaints are suggesting of Chronic Urinary Tract Infection. You need to take immediate action. You may start Syr Uriliser / Alkasol - 2 TSp (10ml) mixed with half glass of water 3 times a day after taking food. This shall reduce your pain but no cure. You consult a good senior physician of your locality, get your urine tested for Routine and Culture with Drug sensitivity to have the right choice of antibiotic. Your physician shall say if any other investigation is required. Please don't neglect it. Untreated, incompletely treated or neglected UTI is not a good accompaniment in life. We have lot many other unavoidable problems to carry on, why the avoidable one?

Haemodialysis - 4 Things a Person Must Follow if Your Undergoing it

Kidney is the most vital organ in the body as it eliminates the waste material from our blood. When kidneys fails to carry out its work effectively the waste materials gets accumulated in the blood and end up with many serious disease condition that affects the other vital structures in our body. The function of the kidney is carried out artificially by an external device called dialyser to purify the blood and remove the waste products from it. This procedure is called as dialysis. There are two types of dialysis they are haemodialysis and peritoneal dialysis. In Haemodialysis, the waste products are removed from the blood and there by the fluid and electrolyte balance is maintained.

Things a person should follow if they are undergoing haemodialysis

  1. Love your heart: Patients undergoing dialysis are at more risk of getting cardiac problems. So it is best advised to follow few of the following measures to take care of their heart
    • Keep your blood pressure and cholesterol levels under control
    • Quit smoking
    • Reduce stress
    • Have healthy low fat and heart friendly diet
    • Get regular physical activity but avoid excessive strainers activity that may require extra effort from your heart
    • Maintain a healthy weight
  2. Diet: Dialysis not only removes the waste matters from your blood but sometimes also the essential electrolytes and nutrients. Protein, potassium sodium and phosphorus are the major things that are to be noted.
    • Encourage the patient to take high quality protein rich foods like meat, fish, milk, egg and chicken which is essential to improve your muscle strength and immunity.
    • Have a check of your sodium level and take sodium rich foods accordingly, as excess sodium may increase the fluid intake.
  3. Limit your fluid intake: Use ice cubes instead of water or use small cups of water. Make sure you measure every sip of water you take.
  4. Don't Overeat: Slow down while eating and stop eating when you feel full. don’t over load your stomach. If you wish to discuss about any specific problem, you can consult a nephrologist.

Your creatinine is very high a) drink herbal/green tea (2 glass daily), drink 6-8 glasses of water (not excess), b) restrict salt intake, reduce protein intake like meat/fish/chicken/egg, diary product (cheese/milk/yogurt/butter), white bread, white flour, sweet/tea/coffee/ beer/wine. C) avoid strenuous physical activity/exercise (go for light exercise like yoga, walking instead of strenuous exercise like running/playing basket ball/foot ball), d) sleep well (7-8) hours daily e) avoid nephro toxic drugs like ibuprufen/ace inhibitor, cyclosporine, f) control diabetes/hypertension, g) do thyroid function test to rule out hypothyroidism, h) avoid potassium containing food like orange, banana, potato, brocoli, spinach, bean, peas dry fruits. I) take low potassium containing fruits like apple, blue berry, corn, cucumber, j) avoid excess salt/salty food, excess protein like red meat/ diary product. K) avoid phosphorous rich food like cheese/fish/pumpkin/soybeans. L) take cabbage, cauli flower, carrot cucumber, omega 3 rich food, raw onion, garlic, turnip, raddish, m) consult physician for medicine if the creatine level is still high.

Tips for removing kidney stones:: 1. Avoid tea, coffee, alcohol, cold drinks, pickles, fish, poultry, meat, spinach, and chocolate. 2. Avoid sour, heavy, and dry food. 3. Increase intake of liquids like water, juice, soup, lime water lemon soda etc. 4. Be sure to drink enough water each day. Usually 1 to 2 liters of water per day is adequate. 5. Drink more if you are perspiring heavily, working outdoors, the weather is hot, etc. 6. Increase the use of barley, horse gram (a type of bean), ginger, and brown rice or parboiled rice. 7. Avoid suppressing urges to urinate and defecate. 9. Boil 10-12 basil leaves in 1 cup of water for 15 minutes. Mix 1 teaspoon of honey in it and take twice a day. Pls send ur USG report becoz medication for stones depends upon size number and location of stones and can be given after USG.

My intestine is sore. Because I can feel pain and it is little out in my anus and it's almost 3 days now that I can't poop. I don't know if it's OK that my intestine is coming out in my anus. And I don't know the cause why I feel pain. My intestine I feel that it's little out when I was in college but I never tried to see a doctor because I don't feel any abnormal or pain but now I'm kind of worried cause of the pain and I can't poop. Hello LENIE I guess you are suffering with hemorrhoids. The symptoms match with your described findings. I suggest you to take high fibre diet, avoid hot and spicy foods. Start taking sitz bath for 10-15 minutes for 3-4 times a day. Don't be afraid to get yourself examined through a surgeon. A detailed history and clinical examination will help to properly identify the diagnosis and thereby the treatment. Hope this helps. Thanks.

It is due to excess drinking of water. You should not take more than 2.2 liters of water (8-10) glass daily. Reduce water intake. If symptoms persist, consult doctor.

Dear lybrate-user, you must go for usg whole abdomen, urine complete. Coz you may have renal calculi or ureteric calculi problem. That can be diagnosed in your usg report. So that I advice you to go for usg whole abdomen & urine complete.

You may be suffering from benign enlargement of prostrate, a common condition at your age group. Symptoms due to persistence of residual urine. Advised 1. Urine test2. Blood sugar, fasting and 2 hr after food. 3. Ultrasonography prostrate area.

First you get examined by a general surgeon and proctoscopy to be done to find out what exactly it is, by the time keep your stool soft by taking plenty of fluids orally and high fibre diet, use ointment lox anally before defecation.
