My grandson is three and half years. He is absolutely normal health wise. He goes to play school and seems to enjoy. He utters words like Amma appa ayyappa. But is not coming out with sentences. Vocabulary is limited. Very cheerful active playful. Some say he has mild Autism. For three months he went through some sort of speech therapy. Slight improvement was found. Eye contact is there. Some times he does tow walking. Beta tests are negative. What could we do? Any advice?

MBBS, MD - Psychiatry, DNB (Psychiatry)
Psychiatrist, Thrissur
Respected Sir. I would like to request you to please forget about giving your grandson a diagnosis, atleast for now. Autism is quickly becoming a sort of "fashionable" diagnosis among the upper middle class people. I would like to know why you believe that he has Autism and supposing he does, I am sure you have already searched on the internet and consulted many others and already learnt that Autism as of now has no definitive treatment. From what I could gather from your question, he does appear to have mild delays in certain areas of development which I think will improve if given adequate training in that area. Also it would be advisable to keep monitoring his IQ and also identify other deficits as he grows up so that they can be caught early and managed accordingly. I would advise you to be in touch with a Pediatrician, Neurologist and Psychiatrist for regular followup of your grandson.

Our voice changes at three stages in life. But if male voice does not change at puberty also it is termed at puberphonia. Many a time we come across a patient where they go under depression due to female voice in male.  Puberphonia is treated very well. Therapist should be able to assess and some time team approach required to help the patient.

My brother have a problem of voice. He is male and his voice is like female voice he is 19 year's old. What should he do for getting male voice?

MASLP

This problem of high pitched voice is called puberphonia in medical terms. Voice therapy can help him speak in low pitch. Male voice. Consult a voice therapist.

Hi !i am a stammer since when I was 5 years old @ 17 years I have started trying to over come it but it's not really working I had tried in many ways it really pushing me down in all aspects can any one help me?

MD - Psychiatry
Psychiatrist, Chennai
Stammering is typically recognised by a tense struggle to get words out. This makes it different from the normal non-fluency we all experience which includes hesitations and repetitions. Commonly it involves repeating or prolonging sounds or words, or getting stuck without any sound (silent blocking). Sometimes people put in extra sounds or words. Often people lose eye contact. Some people who stammer talk their way round difficult words so that you may not realise they stammer at all. This avoidance of words, and avoidance of speaking in some or many situations, is an important aspect of stammering. Stammering varies tremendously from person to person and is highly variable for the person who stammers who may be fluent one minute and struggling to speak the next. Get an mri brain and eeg with a psychiatrist evaluation.

Hello Mr. lybrate-user, change in voice calls for professional help. If you had deep voice that means you must be trying to lower down the throat and speak. If you were changing your voice. It can lead to some problem in voice box. First thing throat should be examine and than we can reach on the conclusion. But for now take deep breath through nose and than talk, see if there is some betterment in your voice. If not than meet speech therapist.

Dear lybrate-user, welcome to lybrate. Stammering is" speaking with sudden involuntary pauses and a tendency to repeat the initial letters of words" or" say something with difficulty, repeating the initial letters of words and with sudden involuntary pauses" stammering could be due to two reasons. First reason is vocal cord misalignment. Second is psychological performance anxiety. First of all you should consult an ent specialist and evaluate your vocal chords. If there is problem, ent specialist will be able to correct it. If there is no problem with your vocal chords, you should consult a psychologist for stammer therapy. Take care.

Am a student of Bcom 2nd year. I love to sing. Am doing riyaz from last two years. My voice is not getting the range. How can I raise my voice without stress.

MBBS
General Physician,
Do warm saline gargle regularly at least thrice daily. Have homemade freshly prepared lemon juice which is rich in vitamin c thereby increasing your body autoimmune system which prevents infection.

My son, age 4 yrs, can not speak but to gain attraction he usually cried out very loud. We consult ENT and did several tests like beta, tympanometry and audiometry. One doctor recommend cochlear implant, Another told to go for hearing aid. After various test the people who sell hearing aid are now demanding to deposit 60000 cash as security deposit for the hearing aid. Is this necessary? Can anybody help what should we do?

Ph.D
Audiologist, Bangalore
You have to pay for hearing aids. Free hearing aids are available only in some Government projects like ADIP in selected institutes. But quality of devices not assured. If hearing aids doesn't help, you may try cochlear implants. However, in spite of these amplification devices, auditory training and speech therapy decides the successful treatment.

My grandson is 20 month as of now he is not speaking nor responding doctors are predicting Autism can you suggest what to do.

M.Sc Applied Psychology, Advanced Diploma Child Guidance & Counselling
Psychologist, Delhi
Dear Lybrate user Please give proper stimulation and time to the child to grow and develop. It's too early to label a child as having autism. First get your grandson hearing test done, because if hearing is not appropriate then it could lead to language delay. Another thing is you can try giving him language exposure like labelling the door as door with the words marked over it. Recite rhymes to him, tell him bed time stories to him. Have patience, all the best.
