Dear Sir, I am 32 years male, and by profession I am a lecturer. I am suffering from lower back pain from 3 months. This ailment was due to lifting over weight unwontedly. I consulted repeated orthopedic and spinal cord specialist too, but not cured fully. The pain was first at the middle of my lower back, now it was shifted to my right side of the lower back. Taking medicines often creating some health problems to me, so I stopped taking them and started yoga. Now the condition was good, but still not able to get rid off the pain and uneasiness completely. Please suggest me a good and life saving advice on my problem. Thank you.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana -- Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day.

Knee Replacement Surgery

I am Dr Hardik Ghundiyal, practising orthopaedics surgeon since 7 years in Bombay. I am mainly into minimal Invasive Joint replacement surgeries.

Knee Replacement surgeries have been done all over the globe since last 30 years. And they have been very successful. In the past, if the patients had arthritis, we could only give them pain killers. But as if now when a patient comes with knee stiffness, pain, deformity, we can provide them with the total knee replacement. We always give a non-surgical conservative approach to the patient when the patient comes with arthritis. If that fails and they are not relieved from the pain, then we give them the option of total knee replacement. About 5,000,00 total knee replacement is being done all over the globe every year. Most of the people who have experienced knee replacement they think that they could have done it before because the pain relief is so good.

When we do a total knee replacement, we do not cut 2 inches or 3 inches of bone from the thigh, instead, we do re-surfacing of the knee. In the normal knee, there are smooth surfaces on the both sides which glide easily when the movement of the knees are done. But in the arthritic knee, the surface is pitted and get rough because of which the pain is there when they glide on each other.

In the total knee replacement surgery, we change the rougher surface into a smooth gliding prosthetic material so that they glide on each other without any pain and patient get pain free movements. The material has been improved drastically from last 15-20 years. The process however remains the same but the plastics, the poly ethylene which is used in between the metal surfaces have been improved drastically by durability as well as the strength. The research has improved the longevity and the prosthesis.

During surgery, we use a muscle sparing approach, that means we do not cut-open muscles while doing the surgery. The patient is mobile as early as possible. The patient is able to stand and walk up on the same day of the surgery. If a patient is comfortable, we make them able to walk also. Hence the hospital stay is reduced and it is minimized to 2-3 days only.

The recovery of the patients depends on how active was the patient pre-operative. Some patients need a walker, walking aid, some patients are mobile without any support. Most of the times, you can get back an activity within month time a surgery.

You will have to elaborate your problem as to a detailed discussion of your back pain which means duration of pain and mode of onset. I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias ( electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem. I am 29 year old male, suffering much pain in neck for last 2 wesk. What should I do?

Sleep on a hard bed with soft bedding on it. SPRING BEDS, FOLDING BEDS OR THICK MATRESS ARE HARMFUL Use no pillow under the head. DO HOT FOMANTATION. Paracetamol 250mg OD & SOS x 5days. Caldikind plus 1tab OD x10. Do neck, back & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take .For emergency treatment contacts your nearest hospital or family doctor. If it does not give relief in 4-5days, contact me again.. Do not ignore .It could be beginning of a serious problem.

Hands getting numb may be a feature of nerve involvement or some blood supply impairment. Patient needs to be checked first clinically and then certain investigations would help.

Extension exercises x 15 times x twice daily. Bhujang asana. Core strengthening exercises. Back stretchingstretch do the cat/cow avoid bending in front. Postural correction- sit tall, walk tall. Apply hot fomentation. Piriformis stretch 1. Lie on your back. 2. Cross your left leg over your right so that your left ankle rests on your right knee. 3. Use your hands to grab hold of your left knee and pull it gently toward the opposite shoulder. 4. Hold for 15 to 30 seconds. 5. Relax, and then repeat with the other leg. 6. Repeat this cycle 2 to 4 times.
