Arthritis and Knee Pain - Know Its Physical Therapy Treatment!

Arthritis is inflammation of one or more of your joints. Pain, swelling, and stiffness are the primary symptoms of arthritis. Any joint in the body may be affected by the disease, but it is particularly common in the knee.

Knee arthritis can make it hard to do many everyday activities, such as walking or climbing stairs. It is a major cause of lost work time and a serious disability for many people. There are 2 types of arthritis the knee joint in the human body can get afflicted with. They are:

  • Osteoarthritis: The form of arthritis which, with increasing pain, slowly wears down the joint cartilages is called osteoarthritis. This form of arthritis usually affects people after the age 40. The symptoms of osteoarthritis include:

    • severe pain in the knee joints

    • pain after walking up the stairs and it subsiding once you are on rest

    • severe pain after the movement of joints for a long time

    • pain that becomes worse in rainy days

    • joints becoming stiff after waking up in the morning but they improve in the latter part of the day

    • pain which also occurs in the thighs and the genital regions coupled with joints swelling and joints getting stiff after rest.

  • Rheumatoid Arthritis: Rheumatoid arthritis is a chronic form of arthritis caused because of the knee joint inflammations. This form of arthritis can occur at any age. Being an auto-immune disease, its symptoms include, but are not limited to:

    • severe pain in the morning

    • mild fever accompanying the pain

    • joints suddenly becoming swollen, red and warm causing, immense pain

    • sudden stiffness of the joints

    • pain that increases in cold weather

    • mild fever, extreme tiredness and weakening of the muscles

Doctors are still doubtful about what exactly causes the disease; but the deformation of the immune system might cause the damage of the joints, causing people who are already suffering from obesity, smokers, and women, in general, more prone to this disease.

When the knee pain is diagnosed as a form of arthritis, the following treatments are suggested:

  1. If you are overweight or obese, losing some of those extra pounds can go a long way in reducing the pain.

  2. Muscle-stretching exercises are effective in keeping the knee joints flexible.

  3. Acupuncture and devices such as knee braces and knee caps can relieve the pain.

  4. Prescribed dosage of anti-inflammatory drugs such as Tylenol, Motrin, and Advil or injections of hyaluronic acid might relieve your pain.

  5. If regular treatments do not work, you might opt for knee-replacement surgery and osteotomy (the process of cutting a bone with the help of surgery) which might better the alignment of the knee by transforming the bone shapes.

Physiotherapy For Knee Arthritis-

Physiotherapy treatment is aimed at improving the symptoms of the disease (i.e. knee pain, swelling, stiffness), and you should begin to notice a positive difference within one or a few physiotherapy sessions.

The main goals of physiotherapy for your knee arthritis are:

  1. Reduce your knee pain and inflammation.
  2. Normalise your knee joint range of motion.
  3. Strengthen your knee: esp quadriceps (esp VMO) and hamstrings.
  4. Strengthen your lower limb: calves, hip and pelvis muscles.
  5. Improve your patellofemoral (knee cap) alignment and function.
  6. Normalise your muscle lengths.
  7. Improve your proprioception, agility and balance.

If you are experiencing joint stiffness in your hip along with pain and tenderness, you could be suffering from hip arthritis or osteoarthritis. Osteoarthritis accounts for being the most common form of arthritis. It is referred to as degenerative joint disease and old-age arthritis and usually, occurs because of age-related issues. The condition occurs when injury and inflammation of a joint lead to the breakdown of the cartilage tissue, causing pain, swelling, and deformity.

How hip arthritis affects the hip joint?
People who are affected by osteoarthritis of the hip may have difficulty in walking properly. Diagnosis in the early stage is difficult as pain may occur in various locations such as the thigh, groin, knee or buttocks.

The underlying causes
The factors responsible for osteoarthritis include increasing age, joint injury and excess body weight. Some other factors may also be responsible. They are as follows:

  1. When the joints have formed improperly.
  2. In case of inherited defects in the cartilage.
  3. When a person puts excess stress on the hip joint because of being overweight or due to certain activities and motions.

How it can be detected?
The different symptoms of hip arthritis or osteoarthritis are as follows:

  1. Joint stiffness, especially after waking up in the morning.
  2. Joint stiffness after sitting for a long period.
  3. Swelling, pain, and tenderness in the hip joint.
  4. A crunching feeling or sound of bones rubbing against one another.
  5. The inability to move the hip for carrying out daily activities.

Treatment
The aim of osteoarthritis treatment is to improve the patient’s ability to move about so that his regular life is less affected. The function of the hip and pain management are other targets of treatment for this condition. The different treatment plans are as follows:

  1. Proper joint care and rest
  2. Using a cane while walking for taking weight off your hip
  3. Losing excessive weight
  4. Non-drug pain relief measures for controlling pain
  5. Regular exercise
  6. Several medicines, including NSAIDS and other prescribed pain medication
  7. Surgery
  8. Alternative therapies

Hip replacement surgery
This is a surgical procedure used for osteoarthritis treatment when all other treatment plans fail. The damaged ball of the hip joint is replaced with a metal ball in this surgical procedure. The hip socket is resurfaced by using a plastic liner and metal shell. Apart from the mentioned treatment procedures, hip resurfacing is another surgical option for osteoarthritis treatment, which helps in giving relief when a hip replacement surgery is delayed. In this procedure, the affected hip joint surfaces are surgically removed and are replaced with metal.

Core strengthening exercises- straight leg raised with toes turned outward, repeat 10 times, twice a day. Back stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day.

Don't do throwing movement for few days. After that consult nearer physiotherapist doctor. N do some exercises for that.

Hi I am 39 years old female, having lower back pain since last 4 days, I took vitamin D3 injection and felt 50 % better. I still had pain (more in right side). The pain used to also ripple effect in my calves. I took an X-ray which shows decrease in L5 space. Can you tell me what are the treatment options available. Do I need to take compulsory bed rest. What exercises are there which will help improve my condition.

Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. This will relieve your problem. Consult after 10 days.

I am 40 years male. I wake up 6 am in morning goes for walk of 1.30 hour after that yoga for 30 mins and pranayam for 1 hour after that also I have back pain and my my body is producing lots of cough. Please suggest me what will I do to get rid of these problems.

Kindly show me photograph of the affected part. Rule out diabetes & vit.D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Kindly take BioD3 Max 1 tab dailyx10 Paracetamol 250mg OD & SOS x5days Do back(spine)/shoulder/knee exercises Make sure you are not allergic to any of the medicines you are going to take Do not ignore it.It could be beginning of a serious problem. First do not stop work out due to pain. Do it with low intensity but regular. Take rest properly. If no progress take analgesic. Hot fermentation is good.

Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang asana -- lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily.
