I have lower back pain since last 4 years. Please advice.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned a 4 yrs history of back pain which actually is quite long in terms of duration and requires extensive evaluation in form of x rays, mri scans etc. But if you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back, mri ls spine and few blood tests for evaluation. Hi if it is only back pain and there is no leg pain then you need to do regular back strengthening exercises and take vitamin d3 tablets.

Ganglion cysts usually occur around the wrist, and often communicate with the radiocarpal or intercarpal joint.
Needle aspirations alone may lead to recurrence.
To prevent recurrence, it is important to excise the feeder channel located in the tail of the ganglion cyst, and remove the cyst wall in total.
Always send the tissue for histopathology.

Please help me out with this. I developed severe and apparently permanent adverse reactions as a result of Hepatitis B vaccine. About 4 years ago I was completely healthy and very athletic before this vaccination and have now suffered severe, debilitating autoimmune side effects from this vaccine. Would a referral to orthopedics or Rheumatology be better for my left shoulder pain, which isn't in both shoulders at once, just the left, previously I had no shoulder problems, I used to lift 100 KG but now I get left shoulder pain when I lift 50 KG and these started after vaccination. There's a measurable loss in stamina before the vaccine to how I am now, I experience breathlessness much earlier when I do cardiovascular exercise then before I had the vaccine. I have chronic fatigue and it is not improved by getting rest. I have a dry persistent cough when there's nothing to bring up. I have a constant pain while I'm seated, which is focused in one specific area, on the upper left side of my abdomen. This area hurts while I'm seated with my laptop or while I'm watching TV or while I'm seated on the bus. The pain is related to muscle movement and positional. The pain isn't there with change of position i.e. Standing up or lying down flat. Painkillers don't help. It's not my stomach because when I eat, I can feel the stomach processing food and the upper left abdominal pain both at the same time, and they are in two different locations. I suffered the residual effects of all these symptoms for more than 4 years. I'm having an Ultrasound scan soon and I've had normal blood tests all which came out fine.

Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

My wife is suffering pain in her right elbow since last 2 months. The local Doctor says- it is Tennis Elbow. Advised wear a belt in his right arm and not to do any heavy work from right hand, no medicine is prescribed. Still no recovery, please advice.

Tennis elbow is easily curable. Just make sure she avoids wringing of clothes, lifting of heavy frying pans or cooker in kitchen. N take some physiotherpy sessions. You cn visit me for further treatment.

Back pain could be due to many reasons. The cause could be even simple as spending long time in sitting position that cause hamstring tightness. In which case you need to stretch your hamstring muscles, keep moving around once in very 20 minutes, and will need a set of back strengthening exercises. To treat your problem the actual cause of your back pain should be assessed and then a programme of exercises should be devised based on your condition and cause of it. Kindly consult in private with a detailed note on the symptoms of your condition.

Rule out diabetes & vit.D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. SPRING BEDS, FOLDING BEDS OR THICK MATRESS ARE HARMFUL Use no pillow under the head. DO HOT FOMANTATION. Paracetamol 250mg OD & SOS x 5days. Caldikind plus 1tab OD x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. Do reply back for detailed treatment plan. Do not ignore .It could be beginning of a serious problem.
