Mr metformin can be taken up to 2500 mg per day. But 100 mg is also a high dose only. If it is not causing any gastro intestinal side effects then no problem. It is really good that you have started on insulin because looking at amount of drugs that you are taking, it is necessary for you to shift to insulin. By doing self monitoring of blood sugar daily you can increase the insulin dosage every alternate day till you reach target fasting level. It is good to reduce or take away pioglitazone.

But I have also acidity problem. Especially, after taking a meal some sour substance come to my mouth from the stomach. But after a while it is vanished. When I usually consult with my doctor, he used to give a medicine, namely, Pandosun D. But as well as I want to stop on taking this medicine, after a few days the previous symptoms appear. Mr. Lybrate-user, hyperacidity and regurgitation problems are likely to be with you. You have to observe certain precautions like: do not keep stomach empty for a long time. Eat small but frequent meals,(every 2 to 3 hours), but total quantity should not exceed your planned daily intake. Plus please keep your blood sugar levels in check - fasting around 100 mg and pp 150 to 160 mg and hba1c% <7. All these things together will help in controlling excess acidity. Avoid spicy foods, sweets and deep fried fods, pastries and junk foods. Reduce the intake of tea or coffee. If you take alcohol that should be stopped and avoid smoking. After meal (lunch or dinner) sit erect for at least 1 hour or more. Exercise regularly for at least 1 hour a day, that helps reduce acidity.

HOW YOGA BOOSTS FERTILITY

MD - Obstetrtics & Gynaecology
Gynaecologist, Delhi
HOW YOGA BOOSTS FERTILITY
When we think of yoga we think of many added benefits like flexibility of the body, better energy level and a trim body. Apart from all such benefits yoga plays a vital role in boosting up the fertility level and helping your body to conceive in a better way. We cannot lay much emphasis on the fact that yoga helps in strengthening your body and mind, also, creating an overall balance between all your bodily functions including the reproductive system. Tight-packed schedules, monotonous office hours, pollution level and excessive workload often result in increasing stress level. Research shows how stress significantly hampers your body’s fertility. Depression, anxiety, stress and guilt also lower your chance of conceiving. Yoga comes to your rescue here. Regular practicing of yoga calms down your mind, soothes your soul and makes you happy from inside. It doesn’t assure conception but yes, it increases your fertility rate and prepares your mind and body for conception.
The question arises here is, what type of yoga should be practiced? Fertility yoga is becoming very popular due to its long-term benefits. Fertility yoga helps to nurture, support and strengthen the endocrine and the reproductive system. The endocrine system is essential for proper hormonal balance. Yoga helps in eliminating the blockages in the blood that flows to the uterus. This again helps in increasing the blood flow to the lower part of the body and supports the body in detoxifying. Some of the yoga poses boost the immune system and speed up the body’s healing mechanism to prepare it for conception. Yoga for fertility helps in eliminating the harmful side effects of fertility drugs as well. With a reduction in stress levels, the body’s ability to cope with the fertility treatments increases your chances of getting pregnant.

Fertility yoga is much like your daily yoga routine. There are many yoga centers that provide fertility yoga sessions. Specific poses are performed in that session for aiding a woman’s effort to get pregnant. Fertility Yoga can assist in boosting a woman’s biological clock and hormonal balance, including menstrual cycle regularity. With regular practice, fertility Yoga enables individuals to connect to their bodies and their partners in a better way. Fertility yoga comprises of a specific series of stretches that have positive effects on reproductive health. Include fertility yoga in your daily routine and lay down a great foundation for a healthy pregnancy.

Hi I ve query abt thyroid & perimenopause symptoms. I am 45 years old Last year around this time only I got my thyroid test done for d first time & tsh came to 26. My doc suggested 50 power thyroxine Got retest after 6 months. Tsh came down to 8 Got another test after nxt 3 months & this time tsh came down to 4.5 Was supposed to get retest again after nxt 4 months. So this time wen I got my test done. I had taken a break of 10 days before going for d test. So my current test showed rise in tsh. Frm 4.5 (4 months back. Instead of going down it is showing rise. It is showing 14 tsh Doc. All I want to kno is. Is this rise in tsh due to d gap of 10 days that I took? My second prblm is perimenopause symptoms: *hot flushes *frequent urge to urinate *craving *headache. Etc.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Hello, Madam, If one has hypothyroidism, then thyroxine in prescribed dose has to be taken regularly without break. If it is stopped, then TSH (Thyroid stimulating hormone) will increase because pituitary detects that there is low level of thyroxine and hence starts producing more of TSH. So you should not stop taking thyroxine on your own. When on treatment, to achieve good symptomatic control TSH should be between 3 to 4 mU/L. Otherwise most symptoms reappear. As regards hot flashes, these usually continue for some months around menopause. There is no specific treatment for that. Frequent urge to urinate is not a part of the symptoms of menopause. Please get your blood sugar checked. Thanks.

Sadique, Losing weight with thyroid disorder and taking contraceptive pills is real challenge. The dietary advise, which does not take into account these factors will surely not work for you. Eating a low calorie food or low fat food will surely be not very effective for you. What you need to do is consult someone who can help you holitically - proper nutrition, right kind of exercise (just few min a day), stress management and sleep improvement. Making changes in all 4 aspect, will help bring the underlying hormonal disturbances under proper control and you will be able to find more energy, and more excitement into your life... weight loss will be just the byproduct! best wishes.

I am 37 years male. I am suffering from uric acid problem and high BO. How can I reduce it?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, high uric acid in the blood may be present in many persons, but it may not cause any difficulty. But in some it leads to joint pains and gouty arthritis. In such situations it is necessary to reduce the uric acid. It is better to reduce overall fat intake as fat reduces uric acid excretion. Liberal intake of water helps excretion of uric acid. Restrict or preferably eliminate alcohol intake. Avoid certain sea foods - sardines, shell fish, scallops, herring, trout, haddock, tuna, anchovies, salmon, mussels, mushrooms makeral. Whole pulses like rajmah, chana, chole and masur. Others are good. Avoid red meat, organ and glandular meat, avoid excess salt intake, avoid highly processed foods, salted nuts, papad and pickels. Avoid bakery products. Sweetened drinks. Eat fresh vegetables, berries, orange, cabbage, bananas, tomatoes, potato and other vitamin c containing foods. Low fat milk products are good and complex carbohydrates (whole grain flour). For controlling bp also restrict salt intake. If reading is high start on antihypertensive drug.

Mr Manickam, Looking at the details provided your BMI is 28 kg/M2, which means you are over weight, your ideal body weight should be 60 kg, so there is 18 kg extra weight. This is a major contributor for diabetes. You have not mentioned about blood sugar levels, HbA1c% and whether you do any exercise. It is important in the treatment of diabetes apart from taking drugs exercise and restricted diet giving exact calories matching the needs based on Ideal Body Weight, blood sugar level and extent of physical activity. Plus one should avoid sweets, deep fried foods, junk foods, pastries, alcohol, red meat and smoking. If you want any specific guidelines please come in a private conversation with details which I have asked, where I will be able to givve specific advice.

Mr. lybrate-user, Looking at your height and weight, it is clear that you have grade 3 obesity with a BMI of 40.83 kgs/M2. This itself is the cause for knee pain. If hypothyroidism is well in control then TSH will be around 3 to 4 mU/L. Since there is no mention about either TSH value or dosage of thyroxine, it is difficult to comment. If you make a serious effort to reduce weight, most of your problems will be in check including knee pain. Thanks.
