My husband's age is 29 and he had taken sugar test and FBS came out to be 113 and PBS 116. He had taken FBS earlier two times as well within a gap of 10 days and first it was 106 then 113 and now again 113. Can you suggest what is the reason of this?

It is normal to see such mild variation in blood sugar. Get his hba1c done and ask him to do dietary control/exercise. If you want to consult me, ask a private chat/call/video call through lybrate.

Female Infertility - 2 Treatment Procedures

Royal College of Obstetricians and Gynaecologists (MRCOG)
Gynaecologist, Alwar
Female Infertility - 2 Treatment Procedures

Infertility is a condition wherein a woman does not get pregnant in spite of having unprotected sexual intercourse over a period of a year or even more. An abnormal menstrual cycle that is either too short or too long, irregular or even scanty can be an indication of a lack of ovulation, which in turn, is another factor behind female infertility.

Causes-

  1. Ovulation disorders are characterized by either a lack of ovulation or irregular and infrequent ovulation. These are a major cause of infertility. This may be due to defects in the regulation of the reproductive hormones by the pituitary gland or the hypothalamus (brain center responsible for producing some of the most essential hormones required by the body). Malfunctioning of the ovary is another cause in itself. Polycystic Ovarian Syndrome, premature failure of the ovaries (a condition wherein a woman’s ovaries fail to function properly even before she is 40 years old), hypothalamic dysfunction (a dysfunction of the hypothalamus) and excessive production of prolactin (a hormone that stimulates milk production among women after childbirth) by the pituitary gland are some of the factors responsible for the occurrence of such a disorder.

  2. Damaged fallopian tubes do not allow the sperms to fuse properly with the egg. They might also prevent the fertilized egg from entering into the uterus. This condition may be caused due to pelvic inflammatory diseases (a group of infections of the reproductive organs in women), an infection in the fallopian tubes or the uterus caused due to various Sexually Transmitted Diseases, any abdominal surgery or surgery of the pelvic region and pelvic tuberculosis.

  3. Endometriosis, wherein the tissue that usually grows in the uterus, starts growing outside it, is another factor responsible for infertility.

  4. Several cervical or uterine disorders, such as tumors, inflammation within the uterus, uterine abnormalities, a cervical narrowing or the inability of the cervix to produce mucous for the sperm to travel to the uterus, are likely to impact fertility by obstructing implantation or enhancing the chances of miscarriage.

Treatments-

  1. Reproductive assistance by Intrauterine insemination (a process that involves putting sperms inside a woman’s uterus in order to help her conceive) and Assisted reproductive technology consisting of IVF, which is the safest and the best.

  2. Fertility drugs can be used to regulate and induce ovulation. But they carry with them certain risks and therefore you should consult your physician prior to consumption. Few examples of these drugs are Clomiphene Citrate, Gonadotropins, Metformin, Letrozole and Bromocriptine.

  3. Surgical procedures can be recommended to correct reproductive abnormalities and restore fertility. A laparoscopic surgery or a tubal ligation reversal surgery (a surgery that unites one’s fallopian tubes again in order to enable the woman to have a baby) can be advised by the doctor.

Hello maam, I am 46 years man recently discovered diabetic and on medication from 3 months. Inspite of taking medicines and cutting off sugar completely, my sugar levels stay around 150 Fasting and 200 PP. We are vegetarians and I have reduced rice intake to 40 grams that too only in the afternoon. Morning breakfast is limited. Night two chapatis only. I have increased intake of vegetables, and taking regularly coconut oil. I am also taking aloevera juice and amla karela juice at regular intervals. Please advise what should I do to bring down the numbers further down Thanks. Nagaraj.

Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, to bring diabetes in control fasting sugar should be around 100 mg, pp 150 to 160 mg /100 ml and hba1c% <6.8. Please let me know what is the treatment you are on with dosage. Along with drugs, diet control and exercise are equally necessary. Using amla or karela juice, benefits are not really known. Most important is a strict diet control, meaning diet should be such that it should provide exact calories based on ideal body weight and extent of daily exercise and blood sugar level. That will help in controlling sugar levels better.

To start diabetes and blood pressure needs life style modification. Avoid sugar and fatty diet. Take small frequent meal 3 times a day containing missi roti (half wheat +half besan) lot of salad before each meal. Your father can take all fruits except mango, banana, grapes, chikoo and leechi and no fruit juices. Fruits must be taken inbetween meal not just after meal. Please srtict to this formula.

lybrate-user, Thanks for the query. I have read all the details given. She does seem to have multiple problems. For swelling in the body there many reasons like uncontrolled hypothyroidism (with high TSH), high BP, kidney related issues, so without knowing the exact details it is difficult to give any opinion. My suggestion is please send following details of her: TSH, T4 & T3 levels, BP readings, blood glucose levels (both fasting and after food) and CBC (complete blood cell count) and lipid profile. Plus her weight, height whether she does any exercise (walking) and current medications with dosages. It is better if you come in a private conversation then I will be able to guide specifically and even prescribe the medications. Even to advise you I need to know all the details. Thanks.

Late night food with more sugar content may give rise to fasting sugar. Take dinner before 8 o'clock and check next morning. Time to give for full digestion.

I am a hypothyroid patient since long, I was on 75 mcg medicine. Now my latest report shows my Ft3 and Ft4 is in normal range, but my TSH is .006. So how much thyronorm medicine I need to take now. I am 71 kg post section and 29 yrs Note-Sep 2015-may 2016 I was pregnant so Dr. advised me 75 mcg and I am still taking same.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
lybrate-user, With TSH 0.006 mU/L, and T3 & T4 being normal, thyroxine dosage can be reduced to 50 mcg per day. But it is better to check TSH after 6 weeks to see if that dose has worked well or if TSH has increased abnormally. Ideally TSH should remain between 3 to 4 mU/L for good control in a non prgnant state. Thanks.

Kidney stones treatment depends on the types and size of stone, most kidney stones won't require invasive treatment. You may be able to pass a small stone by: drinking water. Drinking as much as 2 to 3 quarts (1.9 to 2.8 liters) a day may help flush out your urinary system. Unless your doctor tells you otherwise, drink enough fluid mostly water to produce clear or nearly clear urine. Pain relievers. Passing a small stone can cause some discomfort. To relieve mild pain, according to your medical history I may prescribe pain relievers such as ibuprofen, acetaminophen or naproxen sodium. Medical therapy. I am a doctor and renal (kidney) dietitian who will aslo prescribe a customized diet plan and medications to help pass your kidney stone quickly and with less pain. Do reply back for private consultation for a detailed treatment plan including dietary therapy.

Hello doctor my father was suffering sugar problem last two years what should I do his age 53.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, I am sure he is already on appropriate treatment, that should be continued. Strive to maintain fasting sugar closer to 100 mg and pp between 150 to 160 mg/100ml.& hba1c%<7. These levels will help in preventing or delaying long term complications of diabetes mellitus. Otherwise, consequences can be fairly bad. Apart from regular medication he should also take a restricted but balanced diet, that gives exact calories matching needs based on ideal body weight, sugar levels and extent of daily physical activity. Daily exercise of 1 hour plus is a must. He should avoid sweets, deep fried foods, junk foods and pastries. Should also refrain from alcohol, sweetened aerated drinks and smoking. Also avoid ghee, butter and high fat milk products like cheese.

What is the present treatment? if on oral anti diabetic drugs sugar is well controlled then continue those. It is possible that beta cells might have got revived after insulin treatment for some time.
