When on treatment with thyroxine, tsh levels should come between 3 to 4 milli units, for better control and disappearance of all symptoms. Since your tsh level is 7.41 milli uits/ml, it shows the dose of thyroxine is not adequate. Dose has to go up. In fact in the beginning itself dose should have been higher. If dose is not increased, then most symptoms may come back.

Suggested you should control your sugar nd bp. In addition if you r overweight try nd reduce it. Do some quadriceps strengthening exe. Roll a towel put it under your knee. Lie down straight. Press the roll of towel under your knee with knee hold it for 20 sec. Repeat it 15 times. Twice a day.

Dear lybrate-uservery good question. You See uric acid is a by product or breaking doennof certain molecules in the body. Especially it is acomponent of break down of cells. Normally our body is a perfect machine which is efficiently able to throw out this uric acid. Uric acid can increase if there is excessive intake of products that produce it like alcohol, spinach, red meat etc. Or if the enzymes that are used to process it are deficient. Or if the kidneys are not working, as uric acid is excreted through kidneys. Acute hyperuricemia can cause severe pain and swelling in a single joint. Most commonly it affects the great toe. Avoiding alcohol, red meat; drinking plenty of water are some of the ways to avoid it. Regards.

Mr. lybrate-user, Oats can be taken by a person with diabetes, there is no problem at all. However, since he has had a severe complication related to diabetes it is essential that blood glucose levels are maintained strictly as advised by his physician. Ideally fasting glucose should be closer to 100 mg, PP 160 to 170 mg & HbA1c% <7. Please see that these levels are constantly maintained. Thanks.

I take medicine for diabetes 500 mg each metphormin morning and evening & 15 mg pioze in morning my diabetic was under control for last five years. Though I take regular walk in morning & five days swimming in evening but now my reading is 125 fasting & after two hours of eating 200 do you want me to increase dose or add another medicine?

Hello please follow the doctor prescribed medicine. Which controls the diabetics and will not allow you increase. But along with medication, you need to concentrate on food also. Make sure you food must have high protein, fiber, multivitamin nutrition. For a diabetics, its good to have more protein rather than carbohydrates, which converts into glucose and may have a chance to increase the diabetes. I am type 1 diabetic patient. I am taking insulin 26 in morning and 16 at night. Today morning at 6.30 I checked my sugar it was 47 and after some time at 10 again I checked my sugar it was 155 without eating anything. How it is increased.

lybrate-user, When glucose level goes too low (hypoglycemia), then there is counter regulatory mechanism activated in the body. This leads to release of a hormone Glucagon from alpha cells in the pancreas. That hormone causes breakdown of glycogen in the liver, producing glucose, which in turn increases blood glucose. Body can not tolerate low glucose state for long time. Hence, this counter regulatory mechanism is activated. Thanks.

I am diabetic since 1998. My age is 66 years. At present I take Glypride 2 mg BD. Roimet 850 mg BD. Poiz 15 mg OD (after dinner). The BS level of Yesterday are 139 Morning and 208 after lunch. The BS level is almost same since last three months. Kindly suggested to maintain the BS level properly.

Dear , your sugars are slightly higher than the recommended levels. You can try improving your diet & exercise regimens as you are already on maximum doses of your medications. There is one new tablet invokana 100 mg which is available now. It is expensive. Approx 40- 45 rs per tablet. You can try taking it once a day after breakfast & see. This will be in addition to the other tablets you are already taking. If this is fine with you then probably your sugars might come down. Or else you need to start insulin.

Hi Thanks for the query. Diabetes occurs either due to insulin deficiency, insulin resistance or both. This is particularly true in case of T2 DM. T1 diabetes occurs due to total absence of Insulin. In T2 DM other major contributing factors are being over weight and inactive, plus having a lot of junk food and aerated sweetened drinks (colas). Diabetes can only be controlled with proper modern medication both oral and with Insulin Injections. Plus strict diet control and regular exercise to avoid weight increase. Treatment is lifelong. Adapting a modified lifestyle is the most important aspect of treatment. Ayurvedic and homeopathic medicines do not help in controlling blood glucose on their own. Those may be taken as additional therapies. Uncontrolled diabetes can cause serious long term complications affecting every single system. Mr. Lybrate-user, you can take vitamin supplements since your diet may not be adequate. Any vitamin supplement capsule is fine. Daily one would be good enough, like multivitamin combinations. There are a number of brands. Plus it better if you take some liquid tonic about 1/2 hour before food, which will stimulate appetite, a preparation like neogodine elixer 2 tsf 3 times a day. Thanks.

Mr. Lybrate-user, the problem as described appears to be plantar fasciaitis. This occurs due to inflammation of plantar fascia. Occasional pain killers are useful. But most importantly certain exercises like standing on toes, walking a few steps on toes, stretching the ligament by pulling the foot towards you while keeping the lower limb straight, with a belt and holding it in that position for 20 counts, doing the same for a number of time daily, can help. Spot jogging on toes can also help. Using soft foot wear is also useful.

For diabetes follow a diet low in carbohydrate, low in fat and high in fibre. Avoid bakery products. Avoid too much rice and potato. Avoid fried stuff. Avoid chocolate, cakes, ice cream, soft drinks and fruit juices. Eat lots of vegetables. Walk for at least 30 minutes a day for 5 days in a week. Medicine and insulin will work better if you follow diet and exercise. Check your blood sugar both in fasting and 2 hours after meal. If still your blood sugar is high than your medication needs to be changed.

Keep your sugar and bp under control. Perform 1hr exercises or go for a walk for1 hr daily. Avoid oil fried food and sweets. Check out your cholesterol levels by doing lipid profile once in 6 months and see that the results are within normal range. Be active by avoiding sedentary life style.

Hi, thanks for writing in to us. I have read through your query in detail. It looks like there is no problem in your brain if your mri scan is normal. However many people continue to have symptoms of vertigo due to problems in the inner ear. The inner ear contains the vestibular system which participates in maintaining balance and gait. There are tiny hair cells in the ear which are surrounded by fluid. Changes in the characteristics of these hair cells and fluid might give rise to chronic vertigo. Chronic ear infection is also to be tested. Please consult an ent specialist and get test done for vertigo and nystagmus. Hope your blood investigations are normal. Regards,

Mr. lybrate-user, Thanks for the query. If you have fasting blood glucose 200 mg and HbA1c% 9, then your diabetes is totally uncontrolled. Ideally fasting glucose should be < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. It appears you need to change or up grade the present medication in order to control blood glucose and reduce possibility of developing long term complications associated a almost every single system. Another thing I notice is that your BMI is 26.78 kgs/M2 (normal <23 kgs/M2), showing over weight, which also is a contributory factor for high blood glucose. In this communication we are only allowed to give general guidelines but no prescriptions. Therefore, it is better if you come in a private conversation with all the necessary details, then I will be able to guide. Thanks.

Mr. Lybrate-user, the sugar levels mentioned are really very high and serious. They suggest that she must have been having diabetes since a year or more. Immediate initiation of treatment is a must. It looks like insulin is needed, plus oral drugs also. In addition some necessary physical exercise and diet control also are required. She has to stop sweets completely, avoid aerated sweetened drinks like colas, deep fried foods, junk foods, pastries and high fat milk products. She should consume more of green leafy vegetables, salads and permitted fruits (100 g a day).

Mr. lybrate-user, The particular tablet which is being advertised heavily in TV, is just a combination of some herbal remedies, based on encient ayurvedic texts. There is very little scientific evidence to support. Unless a patient is on modern medicine to treat diabetes or to control blood glucose, there is no way to control it. The medicine mentioned by you can be at best can be taken as a supportive therapy. I have absolutely no belief on the various claims made. Thanks.

Check thyroid level. Depending on the level of prolactin, you may need additional testing and drugs to correct the level.
