My brother aged 45 has severe back pain along with pain on his right thigh. The mri report says that 1. Regeneration spondulitic changes posteirial limbs at l4 superior end plate. 2. Diffuse disc bulge atl3-4 l4-5 levels with annulus tear posterial with central profusion causing canal narrowing. Identification of preexisting nerves in canal bilaterally. Diffuse disc bulge l5-s1 level with annulus tear posterially with posterio central disc extrusion. Extruded disc fragment migrating inferially in canal and causing severe canal narrowing mild neural foremen narrowing no nerve compression. Significant compression of traversing nerve in canal bilaterally mild indentification of existing nerve root in neural foremen bilaterally. What sort of treatment is needed for him?

Knee pain is a common complaint that affects people of all ages. Knee pain may be the result of an injury or some other medical conditions including arthritis, patellar tendinitis, acl injury, knee bursitis, torn meniscus, gout and infections. Try some home remedies for your pain- - ice is useful for the control of pain and swelling. It can be applied to the knee for 15 to 20 minutes as often as every two to four hours. The swollen knee should be elevated above the level of the heart while icing. -activities that cause pain should be avoided temporarily. -if the knee is swollen or sore, the following positions and activities should be avoided until knee pain and swelling resolve: squatting kneeling twisting and pivoting jogging aerobics, dancing playing sport specific exercises will help you improve range of motion, strength, and endurance. It is especially important to focus on strengthening and stretching your quadriceps since these muscles are the main stabilizers of your kneecap. Core exercises may also be recommended to strengthen the muscles in your abdomen and lower back. Also maintaining a healthy body weight to avoid overstressing your knees are very important. -a full history of your problem should be taken and your movements assessed by a physiotherapist. Hello- Diet Tips for High Uric Acid Patients • Lower fat intake as excess fat reduces normal excretion of uric acid. • Liberal fluid intake to keep the urine dilute. • Restriction or elimination of alcohol. What to avoid • Shell fishes, red meat, minced meat, meat extracts, organ meats, fishes (sardine, roe, makeral). • Whole pulses (masur, rajmah, chana, chole etc.). Other dals are allowed. • Vegetables and fruits - green peas, spinach, french beans, brinjal, cauliflower, mushroom, chickoo, custard apple. Diet rules • Use less salt while cooking. Avoid adding salt while cooking rice or in chapatti dough. • Use adjuncts like tomato, vinegar, cocum, amchur, lime juice, curd, pepper powder to make the food cooked with less salt palatable. • Remove the salt shaker from the table. • Avoid salted nuts/biscuits, papads, pickles, chutneys. • Avoid excess consumption of processed and preserved foods; eg. Bakery products. • Caution - Do not overdo on salt substitutes as that might cause an electrolyte imbalance. Some Brilliant ayurvedic medicines for High uric acid- 1) Kokilakshadi Kashaya 2 tsp twice daily 2) Mahatiktaka ghrita 1 tsp twice daily.

I am 40 years old male. I have lower back pain from the last 15 years It developed gradually. I have done many treatments like physiotherapy, allopathy medicines etc it doesn't worked. I feel a lot of pain in my back especially after sex and in the morning time. I have not done any scan like MRI etc. I have got only basic treatment from doctors but the pain is still there. kindly take ashwagandha powder, 1 tsf in a glass of warm milk ,1 hr after meals twice a day. Take maharajah prasani oil capsules, 2 caps twice a day after meals with half a glass of warm water. Take panchamruta loha guggal tabs, 1 tab, three times a day, after meals with half a glass of warm water. Avoid cold water, curd, excess tea and other cold eatables in your routine. Avoid standing for long hrs and soft mattress for sleeping. Do gentle massage with maha narayana oil over the back up to both legs. Continue the regime for 45 days. Consult some good ayurveda doctor for panchakarma treatment. Good luck

This treatment is being suggested on bases of the information provided. I would like to examine & investigate you in detail. Rule out Hypertension, Diabetes or other metabolic disorder. Any way it may be tried, --. Dolokind Plus (Mankind) [Aceclofenac 100 mg +Paracetamol 350 mg] 1 tab. OD & SOS. X 5 days. --. Caldikind plus (Mankind) 1 tab OD x 10 days. Kindly show me x rays of your shoulder. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Do hot fomentation any way take ibuprofen 200mg od & sos x 5days you will need other supportive medicines also. Do neck & shoulder exercises it may have to be further investigated make sure you are not allergic to any of the medicines you are going to take. Shoulder exercises can be done any time. It should not be done immediately after taking food. Do reply back for private consultation and detailed treatment plan.

dear arvindkumar, As ur back pain is not relieved since 2yrs, so u should go for investigations like xrays and blood investigations- CBC ESR Sr Vit D3 level. for time being do hot fomentation, start doing back muscles strengthening exercises.

Quadriceps Exercises- Lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain.

Knee 2 quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day.

I'm 67 years old and I had mild neck pain for few and now since a month it has shifted to both upper arm it's not a continuous pain please advice me.

It could be due to cervical spondylosis this is quite a common condition rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Any way take ibuprofen 200mg od & sos x 5days caldikind plus (mankind) 1 tab odx10days do neck & back (spine) exercises use of cervical collar will give you relief. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take you will need other supportive medicines also. Do ask for detailed treatment plan. Do not ignore. It could be beginning of a serious problem.

This may be either frozen shoulder or cervical radiculopathy. Do neck and shoulder excercises avoid forward bending of neck. Use thin pillows. Otherwise contact me privately with all reports.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - since you have also mentioned pain in legs, I can tell you that there are certain medications like tab pregabalin or tab gabapentin which are available and can be initiated in small doses for your leg symptoms. - physical therapy initially under supervision of a trained physiotherapist and then to continue at home, initially tens can be a useful modality for you. - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation.
