Hi i am Sex Female. I am 36 years old. 3 years ago, I am suffering from Brain thrombosis which called Lakwa (One Side weakness) Paralysis. Now is OK. But Monthly Report PT INR continuous 1.79 to 2.30. I have consult by a doctor at Thane. But Problem is PT INR fluctuation Rate. So I request you So what is the prevention of PT INR fluctuation Ratio. This Time Precaution Tab 4 mg Five times & 5 Mg Two times.

DNB (Neurosurgery) , DNB (General Surgery)
Neurosurgeon,
Reply: the fluctuation of the pt inr is common. This is due to the variation in the body metabolism and food habits. In order to maintain a pt inr of 2.5 times the normal which in your case seems to be in range, a fluctuation of the tablets is necessary. If your symptoms have improved and you have continued this medication for almost 3 years, you may do a mri with Mr. Venogram and re-assess whether the thrombus is completely resolved. In case, the thrombus is resolved and you are not prone for repeated thrombotic attacks, the tablets can be tapered and stopped. In order to give complete details and treatment schedule, you need to provide all the previous details of the problems, the symptoms, the case records, the imaging and the management strategy till date. Regards

I had epilepsy problem some twenty five year ago. I gradually reduced the dosage and I was alright. Bud suddenly had an attack yesterday. What should I do.

MD - Psychiatry, MBBS
Psychiatrist, Panchkula
Hello mam. You must contact your doctor without a delay as your condition needs re-evaluation and maybe, your medicines need to be continued. If you want to reduce or stop your medication, it must be done under guidance of your doctor only, as unsupervised change of medication is always harmful. Please take care.

Hello, there are many causes behind backache *any strain or sprain in the muscles or ligaments of the back during lifting of heavy objects is the main cause of the lower back pain. *sitting in the wrong posture for a very long time is another cause. *people suffering from constipation may also suffer from lower back pain. *sometimes sleeping on wrong type of beds can also cause pain in the back. Follow the tips *take rest or move about once in 1-2 hours in case of working in a constant position. *maintain a good posture while reading or working with computers. *take a high fibre content diet like vegetables, fruits, etc.

My wife, 66 years, suffered a intracerebral brain hemorrhage stroke more than a year ago. She is not under any medication for this other than her regular drugs for diabetics and hypertension. She still feels numbness around her right cheeks and continues to feel bitter taste in her saliva. Is this a common phenomena as some doctors say? Is there any medicine that can help to improve without much side effects? (She is too sensitive to most of the common medications.)

My wife is having migraine pain please suggest.

Dietitian/Nutritionist,
Migraine can be managed with lifestyle changes and medications, try muscle relaxation exercises. Relaxation may help ease the pain of a migraine headache. Get enough sleep, but don't oversleep. Get an adequate amount of sleep each night. It's best to go to bed and wake up at regular times, as well. Rest and relax. If possible, rest in a dark, quiet room when you feel a headache coming on. Place an ice pack wrapped in a cloth on the back of your neck and apply gentle pressure to painful areas on your scalp. It is very important keep a headache diary. Continue keeping your headache diary even after you see doctor. It will help you learn more about what triggers your migraines and what treatment is most effective. A dose of riboflavin in appropriate dose as prescribed under supervision, also may prevent migraines or reduce the frequency of headaches. Coenzyme q10 supplements may decrease the frequency of migraines, magnesium supplements may also be used to treat migraines.

Sleep Apnea and Its Link to Heart Failure

DM - Cardiology, MD-Internal Medicine , MBBS
Cardiologist, Delhi
Sleep Apnea and Its Link to Heart Failure

We're all guilty of making fun of snorers, but sometimes this snoring can be a symptom of sleep apnea. Sleep apnea is a condition where breathing is not continuous and may start and stop many times while asleep. This affects the quality of your sleep and in turn can affect many aspects of your health including your cardiovascular health. Here's how:

High Blood Pressure: When a sleep apnea patient's breathing stops, the oxygen levels in the blood also suddenly fall. This can increase your blood pressure. High blood pressure means that the heart muscles need to work harder to pump blood through the body.

Cardiomyopathy: As a result of high blood pressure, the heart walls may become thicker and the heart muscles become stiffer. This is known as Cardiomyopathy. As this condition worsens, the heart becomes weaker and is unable to maintain a regular rhythm. This can eventually lead to heart failure.

Arrhythmia: An irregular heartbeat is also known as Arrhythmia. This may also be related to the changes in the heart's structure that follow the drop in blood oxygen levels. In many cases, Arrhythmia has no visible symptoms and can often go undiagnosed. This can result in the formation of blood clots in the atria which can lead to a stroke.

Sleep apnea is easy to diagnose. If the doctor feels that you show symptoms of sleep apnea, you may be asked to stay on the hospital overnight and undergo a sleep evaluation. This tests a variety of body functions including brain activity, eye movements, heart rate, breathing patterns and blood oxygen levels.

Treatment for sleep apnea depends on the causes for this condition. If you are overweight, regular exercise and a change in diet can help you lose the excess weight and cure sleep apnea. Similarly, if your sleep apnea is triggered by an allergy, treating the allergy can help cure the sleep apnea. Other forms of treatment for sleep apnea may include:

Continuous Positive Airway Pressure (CPAP): This is a machine which pumps air into the body through a nasal mask. By keeping the pressure in the machine higher than normal air pressure the upper airway passages are kept open and hence the quality of your sleep is improved. An auto CPAP machine can modulate the air pressure such that it is higher when you inhale and lower when you exhale.;

Oral devices: Your doctor may suggest oral devices that allow you to keep your mouth open while you sleep. This is easier to use than CPAP machine but less effective. Surgery is the last resort when it comes to treatment for sleep apnea.
