Dear Lybrateuser, - The chest pain could be due to acidity, anxiety or sedentary habit - avoid spicy, fried & junk food - do not skip meals, have meals on time, chew your food well, have 4-5 small meals including snacks rather than three large meals, do not lye down for at least an hour after meals, have dinner 2-3 hrs before bedtime - have 8-10 glasses of water for proper hydration - do regular exercise like jogging, swimming, brisk walking, also do yoga & deep breathing for 10-15 min daily morning to keep your body fit.

HDL low, and LDL high. Very common problem, usually associated with high triglycerides and increased risk of heart trouble. Suggest you correct the over-production of triglycerides and LDL by liver. This will automatically increase HDL as well. It is better to understand biochemistry of body lipids, just cheering for HDL as if it is Indian cricket team, and jeering against LDL will not protect our health.

Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me with all details like your daily eating pattern and medicines you take. I assure you a proper guidance.

Your pulse rate is normal there and a normal ecg and holter confirm the same. So please stop being concerned about your heart rate. For BP please consult your treating cardiologist and start making lifestyle changes like exercising daily, a healthy heart diet.

Atrial Fibrillation Symptoms, Types and Treatment

MBBS, PG Diploma in Clinical Cardiology
Cardiologist, Gurgaon
Atrial Fibrillation Symptoms, Types and Treatment

We all know about the heartbeat, which is produced by the opening and closing of the valves in the heart, which in turn controls our blood flow. There is a regular pattern to this beat, and when, due to various reasons, it becomes irregular, it is known as arrhythmia. Atrial fibrillation, known shortly as AFib, is one of the common types of arrhythmia.

Why AFib: The heart has an electrical impulse system, which controls the opening/closing of its valves. Due to various changes, be it lifestyle, dietary, or regular wear and tear, this electrical system is affected and so the valves do not function properly. This leads to altered rhythm, and when it happens on the right side of the heart, in the valves between the two atria, it is known as atrial fibrillation.

Signs and Symptoms: It is not something which shows up as soon as the onset happens. It is a gradual condition, and many people with AFib may go for months with no symptoms. There could be general symptoms like fatigue and headaches. Gradually, more symptoms like heart palpitations, shortness of breath, dizziness, occasional chest pain, or fainting set in. It is usually when workup for some other disease is being done.

Monitoring the pulse or heartbeat is one of the best ways to keep a check on the condition.

Types: There are different forms of it – paroxysmal, persistent, and permanent. In the first one, there are brief bursts of AFib lasting less than 7 days. With gradual progression, symptoms are more frequent and last longer, converting into persistent AFib, which lasts longer than 7 days. If the condition is longstanding and the doctor (as well as the patient) have decided not to treat it, it is permanent AFib.

Risk Factors: The chances of developing AFib increase with family history, age, obesity, smoking, hyperthyroidism, chronic lung diseases, and sleep apnea.

Living with AFib: It is a longstanding condition, and the following precautions are necessary, which are generally heart-healthy.

  1. Quit Smoking: In addition to the multiple benefits of quitting, managing AFib is one.

  2. Diet Changes: Change to a heart-healthy diet with whole grains, fresh fruits and vegetables, increased fish oils, reduced fat, reduced salt and sugar etc.

  3. Work It Out: Get into an exercise regimen. Discuss with your doctor to identify and agree on the type and level of exercise. This helps manage weight and stress, both essential for controlling AFib.

  4. Manage Alcohol Consumption: Avoid excess consumption of alcoholic beverages. Avoid them completely if they trigger symptoms.

If you have the risk factors for AFib, implementing these changes earlier than the onset of symptoms can help delay progress and reduce severity of the symptoms. If you wish to discuss about any specific problem, you can consult a Cardiologist.

Hello sir. Echocardiograpghy is a test in which we assess your heart's functioning. Size of different chambers, functioning of valves whether obstructed or leaking, any weakness of any part of or whole of heart chambers especially left ventricle, efficiency of pumping commonly expressed in terms of ejection fraction or ef. By this we can estimate the normacy or if any abnormality then its severity, plan treatment and asses the effect of treatment during followup. Thank you and goodluck.
