Dear lybrate-user being a sports person you should get your shoulder checked up to get a diagnosis. May be you have a rotator cuff tear and need detailed evaluation and imaging to confirm that diagnosis. Meanwhile apply ice and avoid over head activities.

Back stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are some issues which can be there in chronic diabetics. My advise is always to keep sugar levels under control by regularly checking it and consulting diabetologist. Also proper diet helps. Secondly, keeping yourself physically fit like daily morning walks for 2-3 km in a day, general conditioning exercises, abdominal strengthening ex, adequate calcium and vitamin d intake keeps bone metabolism healthy. Meanwhile please get few blood tests done: cbc, esr, crp, serum electrolytes and serum vitamin d levels. We shall supplement in case there is any deficiency. Hello sir.As your age suggest there would be arthritic changes in the joint. If you are getting pain then stop all exercise and try to get pain free shoulder.Application of hot n cold towel thrice a day will be useful.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There definitely could be degenerative changes setting in your joints at this age. Answers to knee osteoarthritis depends actually upon a lot of things but please tell me duration of pain and what are aggravating factors? how much distance can you walk and what is the analgesic requirement to carry out activities of daily routine? what medications you generally take and what has been your treatment history till now for existing problem. I also need to see your latest knee x rays so that I know what grade of arthritis are we dealing with on your case? normally without seeing anything I can still say that at your age you should take adequate amounts of calcium and vitamin d, do regular exercises for knee, back etc because these are the areas which undergo degeneration with age. Diet has got nothing to do with knee arthritis, but still take adequate calcium rich diet to keep bones healthy and strong do not hesitate to contact me if you need any further assistance.

Lybrate-user first of all you have to make sure that you maintain a proper diet schedule i. E. Have breakfast, lunch and dinner on time. Not taking meals at the right time is one of the most common cause of gastric reflux leading to acidity causing throat pain, difficulty in swallowing etc. So first of all make a proper diet schedule and avoid staying empty stomach for long durations.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Swing 20 times morning and evenings. Keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Myelomalacia is a pathological term referring to the softening of the spinal cord. Hemorrhagic infarction (bleeding) of the spinal cord can occur as a sequela to acute injury, such as that caused by intervertebral disc extrusion (being forced or pressed out). The symptoms associated with myelomalacia. “cervical spondylotic myelophaty (myelomalacia) can cause variety of symptoms and some of them can last for years. Here are some: pain in neck, sub-scapular area, shoulder; sensory changes in lower extremities, motor weakness in the upper or lower extremities; numbness or parenthesis in the upper extremities; gait difficulties, like “spastic gait” that is broad-based, hesitant and jerky; upper extremity hypo- reflexia and atrophy, also bladder and bowel dysfunction may occur.” There is no known treatment to reverse nerve damage due to myelomalacia. In some cases, surgery may slow or stop further damage. As motor function degenerates, muscle spasticity and atrophy may occur. Steroids may be prescribed to reduce swelling of the spinal cord, pain, and spasticity.[citation needed] Research is underway to consider the potential of stem cells for treatment of neurodegenerative diseases. There are, however, no approved stem cell therapies for myelomalacia.

Daily 80 km on bike is the reason behind back pain. Our body is not made for such activities daily. During young age it is ok. You will find it hard as your age increases. You need to take precautions. If possible, go by bus or 4 wheeler. That will provide back support. If not, then check stock up of your bike every 3 months. Do back strengthening exercises.
