I am 69 years old male inactive from service since last 15 months & settled in Bangalore staying with my daughter. Since early Nov 2016, I was having occasional pain in my left leg (below Hip. That time, I checked with local orthopaedic who diagnosed it as Trochanteric Bursitis and did not suggest any X-Ray. The pain was on & off.In between I checked with neuro physician, Rheumatologist as well as medicine Specialist. Surprisingly,no one suggested for MRI nor x-ray and suggested to take some pain relief tablets/ointment. For the past 5 weeks, due to pain I was practically at bed except for going to washroom. Today I decided to again check with my Rheumatologist and he suggested to have pelvic MRI. The MRI report is attached for your kind perusal. For your information, I was getting treatment for atherosclerosis Scleroderma for the last 20 years and as part of medication, I was taking steroids (wysolone 5 mg/omnacortil 5 mg) once a day. MRI Report is as follows MRI OF HIP JOINTS TECHNIQUE: Multi-planar, multi-sequence imaging of both hip joints was performed. FINDINGS: Geographic signal abnormalities are seen in both femoral heads (anterior and superior portion) demonstrating double line sign with T1 and T2 hypointense outer margin and T2 hyperintense inner margins. Marrow edema in left femoral head and neck. Small effusion is seen on the left. Hip joint space is maintained bilaterally. Normal marrow signals are noted in the acetabulum. Gluteii, iliopsoas, hamstring and short external rotators are intact. STIR hyperintenstiy noted involving the origin of left vastus lateralis muscle - edema. The sacroiliac joints are normal. The pelvic viscera are unremarkable. IMPRESSION: -Avascular necrosis of bilateral femoral head (Ficat and Arlet stage II). -Marrow edema involving left femoral head and neck with mild joint effusion.

Degeneration ofL1-L2, L2-L3 and L4-L5 lumbar intervertebral discPosterior annular tear noted at L1-L2, L2-L3 and L4-LSlevels. At Ll-L2 level there is right paramidline protrusion, No foramina narrowing. At L2-L3, L3-L4 diffuse posterior disc bulge noted ,causing effacement of ventral thecal sac and inferior recess of bilateral neural foramina narrowing at L3-L4 level. At L4-L5 level, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting L4 and traversing LS nerve roots at this level. At LS-Sllevel, no disc bulge or herniation. Conus medullaris appear unremarkable. Impression:- Lumbar spondylosis degenerative changes as mentioned ,predominant at L4-L5 level.- At L4-LSlevel, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting left L4 and traversing Left LSnerve roots at this level. In medical terms, Osteoarthritis is referred to a disease of the joints. It mostly affects the cartilage or the slippery tissue, which helps to cover the ends of bones in a joint. A proper cartilage helps in gliding of the bones one over the other. In osteoarthritis, the cartilage’s top layer gets damaged or breaks down. This leads to rubbing of the bones and swelling, pain or loss of motion. As the time passes by, the joint may lose its shape. There is also a possibility of developing bone spurs from the edges of the joint. This causes pain and damage.
People suffering from osteoarthritis often complain of joint pain and experience reduced motion. Unlike the other kinds of arthritis, this affects only the joints and does not interfere with the working of internal organs. It is the most common form of arthritis. It mainly occurs in older people. In younger ones, osteoarthritis may occur due to joint injuries.
What Causes Osteoarthritis?
It is caused due to ageing. But there are certain other risk factors that have to be kept in mind. This include:
- Excess body weight or obesity
- Old age
- Joint injury
- Improper formation of joints
- Defective genes
- Stress on the joints
What are the symptoms?
It can develop in any joint in the body. But it mainly occurs in the hands, knees, hips, and spine. The symptoms that indicate towards its occurrence are as follows:
- Stiffness in a joint, especially as you get out of bed or sit for a long period of time.
- Swelling or tenderness in the joints.
- A crunching feeling.
- Sound of bone rubbing.
It cannot be diagnosed by a single test. It takes several methods to detect its symptoms. tHerefore, a combination of tests are conducted, including physical examination, X-rays, blood tests or examination of the fluid in the joints.
How Is It Treated?
It requires a combination of treatments to meet a patient's needs, as per his or her lifestyle, and health. The treatment has four main goals:
- To improve function of the joints
- Maintain a healthy body weight
- It is important to control the pain
- Efforts to achieve a healthy lifestyle
The treatment plan or sketch will involve focus on the following things:
