Hi, my daughter is 4. 5yrs and she is suffering from adenoids and due to that she snores and vomits thick sputum. She does not takes much fluids. Can you please suggest some medicine or treatment?

Pediatrician, Pune
If the adenoids are source of recurrent infections, she may require a good dose of antibiotics, her environment need s to be clean with smoke, dust and allergen free, obviously along with good exercise and a healthy nutritious diet, if inspite of best efforts, the problems are persisting, she may require an adenoidectomy if they cause problem with sleep, with mouth breathing, protrusion of teeth.

Stop or decrease solid foods, particularly if baby is younger than 6 months. Most solids foods have fewer calories and nutrients than breastmilk, plus they tend to replace (rather than add to) the higher-calorie, more nutritious breastmilk. Sleep close to your baby (this increases prolactin and frequency of nursing). Learn baby massage — this has been proven to improve digestion and weight gain. Carry baby throughout the day in a baby carrier; get as much skin to skin contact as you can. Both of these things have been shown to improve weight gain. Nurse often – at least every 2 hours during the day and at least once at night. Frequent nursing increases baby’s milk intake. Make sure you’re allowing your baby to completely finish one side before you offer the other by waiting upon her cues that she is finished; i.e. Pulling off herself and looking satisfied, going to sleep, changing from an active suck/swallow to more of a pacifier suck, etc. Always OFFER the second side, but don’t worry if she doesn’t seem to need it. It’s much more important that she be allowed to completely finish one side than that she nurse both sides. By doing so she will be assured of reaching enough of the richer, more caloric hindmilk that helps her to go longer between feedings. Use breast massage and breast compression during breastfeeding. If you’re pumping, use hands on pumping techniques. Pump or hand express for a couple of minutes before nursing. This will remove some of the foremilk so that your baby receives more of the richer, higher calorie hindmilk. If supplements are medically indicated, breastmilk is preferred over formula as a supplement (exceptions to this are rare), and the average fat/calorie content of mom’s milk is higher than that of formula. Mom can pump for 5-10 minutes after nursing (don’t interrupt or shorten the nursing session to do this), and offer this higher-fat hindmilk to baby as needed. This is also an option for moms who normally offer expressed milk when they are separated from baby. My 4 years old daughter has a very weak appetite. She is 12 kg, What is the best tonic to increase her appetite.

MD - Homeopathy
Pediatrician, Mumbai
Low appetite has many reasons, it could be low haemoglobin levels, first find out that. Try and make food interesting for her by giving her variety of food. Tonics are going to give only temporary relief. So do not start any tonic. Give her chapatis in form of chapati noodles you can google and learn some good nutritious recepies.

For new born baby, there is a white mark/coating in the tongue, may be due to milk powder (Nan or Lactogen feeding). How to clear it?

Diploma in Child Health (DCH), MBBS
Pediatrician, Aurangabad
For new born baby, there is a white mark/coating in the tongue, may be due to milk powder (Nan or Lactogen feeding). ...
That is normal. Try to wipe it with a clean, sterile cloth. It it goes it was most likely due to the milk/nan otherwise it can be a fungal infection called oral candidiasis, common in preterm babies.

Dentist,
Care for childs dental health:
Utmost care should be given for children s dental health. Tooth appears six months from birth. Clean tooth with wet gauze and go for a dental visit at soon aftr first year of birth. Taking child to a dentist at young age is the best way to prevent tooth related problems.

Sexual Abuse - Mental and Psychological Effects on Children

MBBS, MD Psychiatry, DPM Psychological Medicine
Psychiatrist, Kolkata
Sexual Abuse - Mental and Psychological Effects on Children

Child sexual abuse is a pressing issue in society today. Statistically, there has been a considerable increase in the number of cases related to juvenile sex abuse over the past decade. Most of the cases report the abuser to be a close family member or a family friend rather than a complete stranger.

Future Consequences-

  1. A sexually abused child may develop severe mental health conditions and have major difficulties coping with as an adult. One of the most common disorders affecting survivors of child sexual abuse is Post-Traumatic Stress Disorder (PTSD). This involves the individual reliving the trauma all his/her life, thus leading to panic, stress and difficulties in living a healthy life in the longer run.

  2. The individuals who were abused as a child normally suffer from a very low self -esteem. They continue to blame themselves for the events that occurred and hence have a rather demeaning view of themselves, leading to adjustment issues in the future.

  3. They may also be diagnosed with clinical depression owing to the previous trauma. This hampers the everyday functioning of the individual. The individual may continue to relive the past traumatic events in the mind that further fuels their depression and contribute to the vicious cycle, eventually.

  4. Most often, survivors of child sexual abuse have a rather impulsive nature owing to the hampered functioning of impulse-control. They have lesser control over their emotions, especially anger and are rather quick to act on it.

  5. Because of all the complications, they might be less socially competent and thus fail to build firm peer relationships. They grow up to be adults with severe trust issues; they are hence, often side-lined because of their cynicism which leads to further depressive symptoms.

  6. Genophobia or the fear of sex is another complication that they may experience as adults. They may avoid sex altogether or engage in sexual activities that are rather violent in nature. Some survivors may develop certain sexual fetishes that can be rather dangerous; the most common being ‘paedophilia’ or the sexual arousal involving prepubescent children.

Conclusion-

The seriousness of the condition is often undermined. In treatment, various therapeutic techniques are used according to the need and age of the patient. Therapists work to promote positive thinking among the sufferers so that it becomes easy for them to accept their past and move on in life with a better outlook. With proper guidance and help, the individuals are able to overcome their mental health issues and live a wholesome life.
