Regular exercise for 45 minute and taking food at 4 or 5 hour interval will reduce both the issue easily. Dont try to understand medicine or type of cardiac problem, which will confuse you a lot. Better move to a doctor who can explain your problem politely, that is good. Well defined problem is half cured. Seems you are having so many medicine which is not required when you do exercise regularly. I dont find your age. always inform your age. You may not know the weight and height exactly but age can be.

To control mastrubation first avoid using smart phone. Which affect your sleep, which grab your time and create stress. And it will reduce your physical activity.
