Dear Lybrate User, *For diabetes follow a diet low in carbohydrate, low in fat and high in fibre. Avoid bakery products. Avoid too much rice and potato. Avoid fried stuff. Avoid chocolate, cakes, ice cream, soft drinks and fruit juices. Eat lots of vegetables. *Practice Yoga, breathing exercise.

Merese bahut saare patient aate hai aur baar baar yahi poochte hai ki doctor saab ye angina pain kya hota hai? Heart attack hota kyun hai? Unka jawaab dene ke liye, unko ye samjhata hoon ki angina jo hota hai ki jab dil ke andar jab khoon ki supply kam ho jati hai aur, dil jo hai it’s like a pump. Pump ke andar aapko chalane ke liye energy chahiye, energy aati hai usko apni supply mine se. Jab vo apni supply dheere dheere khoon ke naliya sikudne ki wajah se kam ho jaati hai toh patient ko angina shuru ho jaata hai. Aur agar suddenly wo nali band ho jati hai to patient ko heart attack ho jata hai.

Ab sawal ye uthta hai ki ye hota kyun hai? Angina ya heart attack dono ek hi beemari ke do roop hai. Dono ke kaaran ek hi hai. Pehla kaaran jo hota hai wo hota ki jisko hum bolte hai non-modifiable factor. Non-modifiable factor hai isko hum bolte hai ki patient ko vanshanugak problem hai. Matlab uske pitaji ki taraf ya mataji ki taraf kisi ko bhi family me agar ye problem rahi hai heart attack ki, angina ki toh usko jo aage santaan aati hai unko hone ke kafi kafi chances rehe hai. Dusra hota hai modifiable factor. Modifiable factor me bahut saare factor hai jaise ki patient agar diabetes hai usko control nahi kar rahe hai properly, patient ko blood pressure hai usko properly control nahi kar hai, patient ko cholesterol bahut jaada rehta hai, hyperlipidaemia jisko bolte hai usko properly control nahi kar rahe hai ye jo factor hote hai iske alava jin do cheezo pe mai bahut jyada mai stress dena chahunga aur samjhana chahunga jo ki bahut aasani se aap bacha bhi sakte ho aur bahut dangerous hai.

Ek sabse badi problem hai smoking ki, smoking ek itni katarnaak cheez hai jo ki aam taur par logbagh samajhte hai ki smoking se cancer hi hota hai. Smoking se cancer to hota hi ye har aadmi jante hai, par har aadmi ye nahi jante hai ki smoking se khoon ki naliya sukad jati hai, khoon gaadha ho jata hai aur khoon ka bahaav kam ho jata hai. Uski wajah se patient ko jo stroke bhi ho sakta hai, jisko hum bolte hai brain attack ya heart attack bhi ho sakta hai. Jisko hum normal bahasha me hum bolte hi hai har log jante hi heart attack kya hota hai. To ye do cheeze hai agar aap smoking karte hai to aap ko bahut jayada chances hai ki heart attack ya brain attack hone ke chances bad jate hai.

Dusra mai bolunga ki humara jo life style hai, life style modification ka bahut bada role hai, heart attack ya angina ko prevention ke liye. To life style hum log ka bahut change hogaya hai. Aaj kal logbagh raat der tak jagte rete hai. Proper need nahi le paate hai, proper relaxation nahi ho paata hai, mind ka relaxation nahi hota, tension bada rehta hai. Yeh sab cheeze aaj kal itni jyada ho gayi hai ki jiski wajah se aap dekh rahe hai ki din pratidin heart attack aur angina ki problems badti jaa rahi hai.

My mother has diabetes and blood pressure problem also. She is continuously suffering from fever now a days in evening. But the fever gets off within 1 to 2 hours. She has swelling in the legs too. She is a school teacher too so she has to work whole day therefore she gets a little time to take rest. So please suggest me some valuable suggestions for her. Thanks.

Avaid fatty and spicy food do exercise and go for ultrasound for cholelithisis according to me you are also suffering from hyper Acidity.

I am 39 male from Mumbai. I am 5'8 tall and weight 88 kg and have a problem from past 10 years or so even when I weighed 80 kgs and here is my brief. Everyday, I wake up feeling good. I have energy and mind is somewhat sharp and happy for half n hour. After an hour of standing or waking up, I feel crashed. I have heaviness in my eyes and feel as if I have a hangover without head ache. Very intolerant and I battle through this everyday. My basic blood tests such as cholesterol are normal. No diabetes. However vitamin d is 6. B12 is normal. I do take Antacids for my GERD and gastritis everyday which is another problem that I deal with. Blood pressure is 125/ 80. After standing up, it might variate. When I take quick 15-20 min nap, it goes away for 5-10 mins and then gradually comes back again. I also play badminton everyday and burn around 600 calories. After playing I sometimes feel better maybe because of fast blood circulation? Back in the days, l used to be fine in the evening but now, I feel like this all the time. I am looking for real diagnosis and would really appreciate if someone can help me out.

Heart Failure - Why Pacemaker Is Required?

The treatment of heart disease has come a long way and one of the more important and fairly common devices used to correct rhythmic problems of the heart is a pacemaker. A pacemaker is a device which is put inside the chest or sometimes even the abdomen to send electrical pulses to the heart to keep it beating at a normal rate.

Pacemaker for heart failure

However, the implications in case of heart failure make the usage of the pacemaker much more complicated. In such cases, the device used is a special type of pacemaker which is also called a bi-ventricular pacemaker. This device sends electrical signals to pump the ventricles of the heart so that they can pump at the same time.

There are many factors to be aware of before undergoing this treatment. Some of the factors to consider are mentioned below.

  • Requirement for a pacemaker in case of heart failure - When a patient has heart failure, their ventricles or lower chambers of the heart aren't able to pump enough blood into it. This could be due to physical problems with the heart or problems with the electrical system within it which cannot function normally. The doctor will perform multiple tests to determine whether a patient is the right candidate for a pacemaker for heart failure.
  • The procedure - In most cases the device is fitted into the chest and it is done without an open heart surgery. Precautions and post operative care for a minor surgery should be followed. The doctor makes a minor incision on the chest and then inserts the device along with three leads which are connected to the ventricles to ensure they keep pumping properly. Sometimes another device known as the ICD or the implantable cardioverter-defibrillator may be put in to shock the heart back to a normal rhythm in extreme cases where the change of sudden death from high heart rate is a possibility.
  • Immediate post operative care - As mentioned above, the surgery involved in putting a pacemaker is a minor one and the patient is usually discharged in next 2-3 days or may be more depending on the comorbidities & complexity of the scenario. However, in most cases, people can go home the next day fairly easily although some monitoring is done for the next few days with daily reporting.

Hello Mr Anwar, If the X ray, ECG and Echo cardio gram are all normal, then you dont need to worry much but you need to be vigilant. Take antacid Gelusil one tsp twice a day for a week and see if there is any improvement. If the pain lasts, you need to consult another doctor who is different from the one you consulted with before, in order to take a second opinion and you will have to repeat the ECG and Echocardiogram.

BP should stay below 140, dose depends on response. You can gradually reduce dose later if BP stays below 120.
